Trinta estudantes, jovens e saudáveis, de Odontologia, foram submetidos, uma vez ao dia, durante três dias consecutivos, (intervalos de 24 horas) a dois testes comuns de memória: 1 — o reconhecimento de faces famosas em fotografias, e 2 — a identificação do ano em que eventos importantes ocorreram. Os estudantes foram divididos em três grupos de dez estudantes cada um, e submetidos a ambos os testes acima mencionados sempre o teste de número 1, precedendo o teste de número 2. No primeiro dia, logo após os testes terem sido aplicados pela primeira vez, um dos grupos (Grupo C) foi passivo (todos os seus integrantes) de aplicação de uma tira de borracha, simulando uma força ortodôntica (orthodontic-like force — OLF), entre o primeiro e o segundo pré-molares superiores direitos. No segundo dia, uma hora antes da aplicação dos testes, outro grupo (Grupo B) foi passível de uma experiência inusitada (NE — novel experience) ao permanecer por 10 minutos em um ambiente onde nenhum de seus componentes tinha estado anteriormente. Os testes foram aplicados da mesma forma e ordem que no primeiro dia, em intervalos de 24 horas, no segundo e terceiro dias a todos os três grupos. (Grupo A — controle). Os resultados obtidos mostraram resultados melhorados estatisticamente significativos no teste de reconhecimento de faces famosas para o Grupo B (NE) tanto no segundo dia — P < 0.01, como no terceiro dia — P < 0.05. Os resultados obtidos na aplicação do teste de eventos significativos mostrou resultados melhorados estatisticamente significativos tanto no Grupo B (NE) como no Grupo C (OLF) onde P < 0.01, mas apenas no terceiro dia. Os resultados indicam que ambas as diferenças avaliadas — experiência inusitada e esforços alterados sobre os dentes — podem melhorar o desempenho de humanos em testes de memória, sugerindo o envolvimento em ambos os casos de mecanismos neuronais endógenos que modulam a percepção da dor e a memória.
Purpose: Magnetic resonance imaging (MRI) is useful for interpreting temporomandibular joint (TMJ) disorders including disc displacement. This study aimed to evaluate the relationship between TMJ sounds and disc displacement with and without reduction by clinical examination and MRI.Methods: A sample of 58 patients was clinically examined when after being referred to MRI due to signs and symptoms of unilateral or bilateral TMJ disc displacement. The diagnosis obtained using MRI was compared with the clinical findings for the presence or absence of articular sounds.Results: Most patients (n=39; 67%) had joint sounds manifested as clicking; the other 19 patients (33%) did not present sounds. Among the 39 patients with sounds, bilateral clicking was observed in 29 (74%), and unilateral clicking in 10 (26%) patients. Unilateral clicking was equally observed on the right and left sides, each occurring in 5 five patients (13%). Conclusion:The results suggest that articular sounds do not occur exclusively in TMJ with disc displacement with reduction. The presence or absence of joint sounds will not suffice as the only aspect criterion to be considered in the differential diagnosis of disc displacement with and without reduction.Key words: Temporomandibular joint; disc displacement; joint sounds; magnetic resonance imaging ResumoObjetivo: A ressonância magnética nuclear (RMN) é de grande utilidade para o diagnóstico das desordens temporomandibulares, inclusive o deslocamento de disco é muito comum. O objetivo deste estudo foi avaliar a relação entre ruído articular e deslocamento de disco com e sem redução por meio de avaliação clinica e ressonância magnética nuclear.Metodologia: O estudo examinou 58 pacientes clinicamente, que foram submetidos à RMN quando apresentavam de sinais e sintomas de deslocamento de disco uni ou bilateral. O diagnóstico obtido pela RMN foi comparado com os achados clínicos relativos à presença ou não de ruído articular.Resultados: Dos 58 indivíduos estudados, 39 (67%) apresentaram ruído articular do tipo click, os restantes 19 (33%) não apresentaram ruídos. Dos 39 com ruído, a ocorrência bilateral foi de 29 (74%) e unilateral em 10 (26%) casos, sendo 5 (13%) do lado direito e 5 (13%) do lado esquerdo.Conclusão: Os resultados demonstraram que os ruídos articulares não estão restritos a articulações com deslocamento de disco com redução. A presença ou ausência de ruído articular não devem ser os únicos aspectos considerados para o diagnóstico diferencial entre deslocamento de disco com e sem redução. É realizada uma discussão sobre as informações disponíveis a partir de pesquisas atuais, publicadas na literatura. São também analisadas as suas consequências na determinação de avanços clínicos no reconhecimento das várias etiologias capazes de desencadear desordens temporomandibulares e dores oro-faciais. Uma ênfase especial é dada à necessidade de uma base biológica ampla e correta, e à apresentação - tão cedo quanto possível no curso pré-clínico - de informação relativa a pesquisas clínicas selecionadas, de maneira a prover um real embasamento a um profissional que deve, crítica, analítica e cientificamente avaliar e coordenar tratamentos em pacientes com desordens temporomandibulares e dores oro-faciais. Um diagnóstico absolutamente correto é a precípua, e quem sabe o único meio conhecido de ser evitada a iatrogenia.
Este trabalho, orientado para o aluno iniciante em "Desordens temporomandibulares e dor oro-facial; Diagnóstico" foi elaborado para proporcionar uma parcela de informação revisada e atualizada em complementação a uma série de conferências sob o título de "Dor no consultório - o que fazer?" (Diretório Acadêmico Othon Silva - FO/UFRGS - 1990) Considerando o fato de que uma boa parte das dores somáticas profundas na região oro-facial têm origens dentárias, achamos justificável a abordagem de: 1 - um resumo dos fatores envolvidos na etiologia e percepção das dores polpares e periodontais, e 2 - uma descrição suscinta dos mecanismos que embasam a transmissão das dores de origem dentária e sua modulação, tanto periférica como central. Não são aqui discutidas, devido a suas características muitas vezes multidisciplinares, especialmente nos casos de dores crônicas, as oportunidades terapêuticas múltiplas atualmente à disposição do clínico. Algumas serão só citadas. Para facilitar ao aluno interessado a oportunidade de um maior acesso às fontes de referência atualizadas, aquelas consideradas mais importantes são citadas ao final do trabalho e numeradas no texto.
