Depression and anxiety symptoms in chronic pain are associated with adverse clinical outcomes, and appear highly related to patient's illness perceptions as well as with marital adjustment. This study aimed to investigate the predictive value of pain variables, marital adjustment and illness perceptions on depression and anxiety in patients with chronic pain. Two hundred patients were recruited from a pain unit in a public hospital in the north of Portugal. Patients completed a questionnaire that assessed illness perceptions (IPQ-Brief), marital adjustment (revised dyadic adjustment scale), depression and anxiety symptoms (hospital anxiety depression scale) and pain variables (pain intensity and pain disability index). Depression and anxiety symptoms were associated with pain intensity, pain-related disability, marital adjustment and illness perceptions. Results from hierarchical regression showed that illness perceptions contributed significantly to depression and anxiety symptoms over and above the effects of pain intensity, pain-related disability and marital adjustment, after controlling for gender. In multivariate analyses, pain intensity, pain-related disability and marital adjustment were uniquely related to depression and anxiety symptoms, whereas specific illness perceptions were uniquely related to depression symptoms (identity, treatment control, emotional response and coherence) and to anxiety symptoms (identity, emotional response and concern). Perceptions of greater symptomatology (identity) and of emotional impact, and lesser perceptions of treatment control and understanding of chronic pain (illness comprehensibility) were significantly associated with increased depression symptoms. Perceptions of greater symptomatology (identity), emotional impact and greater concern were associated with anxiety symptoms. These findings indicate that the contribution of illness perceptions was greater than that made by traditional covariates, and may therefore be a useful basis for future psychological interventions.
Este estudo analisou as diferenças entre jovens e idosos ao nível das atitudes e dos conhecimentos face ao envelhecimento e face à sexualidade na terceira idade; a relação entre as variáveis em estudo; e os preditores das atitudes negativas face ao envelhecimento e das atitudes negativas face à sexualidade na terceira idade. Cento e cinquenta e três jovens universitários e 42 idosos participaram no estudo. Os resultados indicaram que os jovens apresentam um menor nível de conhecimentos acerca da sexualidade na terceira idade do que os idosos, e os idosos possuem atitudes mais negativas face ao envelhecimento e atitudes menos permissivas acerca da sexualidade na terceira idade do que os jovens. Verificou-se que quanto maior a atitude negativa face ao envelhecimento, maiores são as atitudes negativas face à sexualidade na terceira idade. Os preditores do idadismo foram a idade mais elevada e o menor contacto diário com idosos. Os preditores das atitudes negativas face à sexualidade na terceira idade foram a idade mais elevada e as atitudes negativas face ao envelhecimento. É importante intervir junto da população ao nível dos conhecimentos, das atitudes negativas face ao envelhecimento e modificação das atitudes negativas face à sexualidade na terceira idade.
This study highlights the importance of the worries during pregnancy and relationship intimacy in shaping pregnant women's emotional distress, and identifies both as targets of intervention.
Background: Young women make up most cases of HIV infection in Portugal (e.g., 26% in 2012), but their sexual behaviour, ability to recognize sexually transmitted disease (STD) risk in partners, and knowledge and practice of safe sex are underresearched. Methods: We studied these issues in a group of 177 women from Northern Portugal, together with their social, educational, and religious background. The women filled out several self-report questionnaires developed in the USA that have been adapted and validated for use in Portugal. They assess HIV knowledge, risk perception, and sexual risk behaviour. Results: We documented good knowledge about HIV transmission and prevention, although there are still some myths; 79.9% of the women knew that condoms prevent HIV transmission, but only 46% of them declared to use them regularly. Thus, knowledge does not imply adequate preventive behaviour. Women lacking higher education had poorer knowledge of HIV biology and of partner HIV risk. Despite being aware of HIV cases in their communities, most women deemed themselves at little risk, especially those lacking higher education, with many arguing that monogamy protects them, and, importantly, many preferring to ignore their partners' current and past behaviour when assessing their exposure. Conclusion: These results point to important intervention targets for campaigns to curb STD infections among young women. reconhecer o risco de IST em parceiros e o conhecimento e a prática de sexo seguro são pouco investigados. Méto-dos: Estudaram-se essas questões num grupo de 177 mulheres do Norte de Portugal, juntamente com seus antecedentes sociais, educacionais e religiosos. As mulheres em estudo preencheram vários questionários de auto relato desenvolvidos nos Estados Unidos da América e que foram adaptados e validados para uso em Portugal. Foi avaliado o conhecimento sobre o VIH, a perceção de risco e o comportamento sexual de risco. Resultados: Documentaram-se bons conhecimentos sobre a transmissão e a prevenção do VIH, embora haja alguns mitos; 79,9% das mulheres sabem que os preservativos impedem a transmissão do VIH, mas apenas 46% declara usá-los regularmente. O conhecimento não implica o comportamento preventivo adequado. As mulheres que não possuíam escolaridade de nível superior tinham um conhecimento mais pobre no que se refere à biologia do VIH e ao risco de VIH no parceiro. Apesar de estarem cientes dos casos de VIH em suas comunidades, a maioria das mulheres considerou-se com pouco risco, especialmente aquelas que não possuem ensino superior, argumentando que a monogamia as protege e, principalmente, muitas preferem ignorar o comportamento atual e passado de seus parceiros ao avaliar a sua exposição. Conclusão: Os resultados identificam importantes alvos de intervenção para as campanhas de prevenção de ISTs na população feminina.
A obesidade é um problema de saúde pública considerando a sua elevada prevalência, a dificuldade no controlo e o elevado índice de reincidência. Metodologia: É um estudo com desenho correlacional e foi desenvolvido num Centro Hospitalar da Região Norte de Portugal (CHAA), com uma amostra não aleatória de 100 participantes, de ambos os géneros, com obesidade mórbida e candidatos à cirurgia bariátrica. O objectivo deste estudo consiste em avaliar os níveis de ansiedade, depressão e auto-conceito em obesos que vão ser submetidos à cirurgia bariátrica, mais concretamente, caracterizar o estado emocional (ansiedade, depressão) dos utentes com obesidade mórbida candidatos à cirurgia bariátrica, verificar a relação entre os níveis de ansiedade, depressão e o auto-conceito, analisando se existem relações significativas entre as dimensões do auto-conceito (aceitação/rejeição social, auto-eficácia, maturidade psicológica, impulsividade/actividade) e a ansiedade e depressão. Pretende-se ainda verificar a relação existente entre o IMC e a ansiedade, depressão e avaliar a relação entre o IMC) e o autoconceito. Para a realização do presente estudo foi administrado um Questionário Sócio-Demográfico e Clínico, o Hospital Anxiety and Depression Scale (HADS) de Zigmond e Snaith (1983) e o auto-conceito através do Inventário Clínico de Auto-Conceito (ICAC) de Vaz Serra (1986). Resultados: O estudo sugere uma associação estatisticamente significativa entre os níveis de ansiedade, depressão e o autoconceito em indivíduos com obesidade mórbida. Verificou-se ainda que a ansiedade está presente nos indivíduos com obesidade mórbida mas, de acordo com as análises efectuadas, não parece haver correlações significativas entre a ansiedade, a depressão, o auto-conceito e o IMC. Conclusão: Apesar de alguns estudos revelarem que os indivíduos obesos apresentam uma maior probabilidade de manifestar algum grau de psicopatologia, nem todos os sujeitos apresentam graus significativos de perturbações psicológicas. Esperamos contribuir para ampliar o conhecimento e compreensão dos aspectos psicológicos desta doença, associada às dimensões em análise -depressão, ansiedade e autoconceito que tem sido estudado, sobretudo, do ponto de vista médico, nutricional e estético, mas ainda de forma incipiente pela psicologia, contribuindo, deste modo, para o aprofundamento desta área e o desenvolvimento de futuros planos de intervenção na praxis da Psicologia Clínica e da Saúde. Chronic pain is a health problem that significantly influences patients' lives, causing functional, social, socioeconomic, and emotional changes that impact quality of life (QoL). The aim of this study was to evaluate which variables (e.g., psychological morbidity, illness representations, pain, and coping) contribute to QoL and to analyse the moderating role of illness-and wellness-focused coping in the relationship between pain interference and Qol, in chronic pain patients. A sociodemographic and clinical questionnaire, the Brief Illness Perception Questionnaire, the Pain Catastrophizing Scale, the Hospital Anxiety and Depression Scale, the Chronic Pain Coping Inventory, the Short Form Health Survey, and the Brief Pain Inventory were completed by 103 patients with chronic pain. Greater use of wellness-focused coping and being professionally active were associated with better physical QoL. Cognitive representations and illnessfocused coping contributed to physical QoL, and psychological morbidity contributed to mental QoL. Illness-focused coping and wellness-focused coping moderated the relationship between pain interference and physical QoL, but not with mental QoL. Since pain interference was positively related to psychological morbidity, and the latter was negatively related to QoL, it is extremely important to evaluate and promote patients' coping strategies that are focused on well-being to improve QoL. Results from this study underline the relevance of a multidisciplinary approach to chronic pain and the need to account for psychological morbidity and coping strategies in intervention programs to promote QoL in chronic pain patients.
