La proliferación epitelial y la reabsorción ósea son de las principales consecuencias del crecimiento del quiste periapical. Los factores de crecimiento son importantes mediadores de la comunicación intercelular entre el tejido conectivo y el epitelio. Información acerca de los factores de crecimiento que están involucrados en el desarrollo de los quistes periapicales pueden ayudarnos a entender los mecanismos que estimulan la proliferación epitelial y la formación de un quiste. Los principales mecanismos de expansión quística son: proliferación epitelial, acumulación de contenidos celulares, crecimiento hidrostático, factor de resorción ósea y actividad enzimática. Dentro de los procesos moleculares involucrados se encuentran la unión del factor de crecimiento epidermal a los receptores de membrana celular, lo que induce a una serie de eventos bioquímicos como señales de transducción, de activación de proteincinasas, fosforilación de proteínas reguladoras, transcripción genética y síntesis de proteínas. También se encuentran implicadas en la reabsorción ósea perirradicular las prostaglandinas PGE2 y PGI2 , leucotrienos y colagenasas. La IL- 1 es la citosina más activa que actúa en la expansión quística a través de su acción en un amplio espectro de funciones celulares como proliferación de fibroblastos, producción de prostaglandinas en la cápsula quística y osteólisis, interactúa con otras interleucinas como la IL-4, IL-6, IL-8, IL-10, IL-12, el interferón-gamma (IFN-γ) y el factor de necrosis tumoral-alfa (TNF-α) promoviendo, activando y diferenciando a los osteoclastos favoreciendo la secreción de prostaglandinas por los fibroblastos y los osteoblastos.
RESUMEN: Los criterios histológicos para determinar el grado de displasia, la clasificación de Broders y el frente de invasión tumoral (FIT) son parámetros subjetivos no cuantificables que pueden indicar el grado de evolución de displasias y carcinomas. Un factor importante a considerar durante la valoración histológica, es la variabilidad del diagnóstico entre patólogos. El objetivo es estandarizar los criterios y determinar la variabilidad intra e inter observador en el diagnóstico de displasias y COCE. Este estudio tuvo como objetivo determinar la concentración de las proteínas totales y pH en muestras de saliva humana, en pacientes de 15-25 años con tratamiento de ortodoncia. Se seleccionaron pacientes de la Clínica de Ortodoncia del Posgrado de la Facultad de Odontología de la UNAM, con y sin aparatología ortodóntica. Se les pidió no comer, beber, fumar y realizar higiene bucal dos horas antes de la recolección de saliva estimulada y no estimulada; las muestras se tomaron bajo las mismas condiciones, por el mismo investigador entre las 8 y 10 a.m. Las muestras fueron almacenadas a menos 20ºC, posteriormente se utilizó espectrofotometría mediante la técnica de Bradford para la determinación de proteínas totales y la medición del pH salival se realizó, utilizando un potenciómetro. Dentro de los resultados se encontró que existieron diferencias estadísticamente significativas entre el grupo control y el experimental en el pH de la saliva total no estimulada (t de Student P <0.05) no encontrando diferencia estadísticamente significativa entre la concentración de proteínas en ambos tipos de salivas.
