Although a telephone-based intervention is feasible and acceptable for this population, peer helping in the context of a coping skills intervention does not enhance spiritual well-being relative to coping skills alone.
Background: Fatigue often interferes with functioning in patients with advanced cancer, resulting in increased family caregiver burden. Acceptance and commitment therapy, a promising intervention for cancer-related suffering, has rarely been applied to dyads coping with advanced cancer. Aim: To examine the feasibility, acceptability, and preliminary efficacy of acceptance and commitment therapy for patient-caregiver dyads coping with advanced gastrointestinal cancer. Primary outcomes were patient fatigue interference and caregiver burden. Design: In this pilot trial, dyads were randomized to six weekly sessions of telephone-delivered acceptance and commitment therapy or education/support, an attention control. Outcomes were assessed at baseline and at 2 weeks and 3 months post-intervention. Setting/participants: Forty patients with stage III–IV gastrointestinal cancer and fatigue interference and family caregivers with burden or distress were recruited from two oncology clinics and randomized. Results: The eligibility screening rate (54%) and retention rate (81% at 2 weeks post-intervention) demonstrated feasibility. At 2 weeks post-intervention, acceptance and commitment therapy participants reported high intervention helpfulness (mean = 4.25/5.00). Group differences in outcomes were not statistically significant. However, when examining within-group change, acceptance and commitment therapy patients showed moderate decline in fatigue interference at both follow-ups, whereas education/support patients did not show improvement at either follow-up. Acceptance and commitment therapy caregivers showed medium decline in burden at 2 weeks that was not sustained at 3 months, whereas education/support caregivers showed little change in burden. Conclusions: Acceptance and commitment therapy showed strong feasibility, acceptability, and promise and warrants further testing. Trial Registration: ClinicalTrials.gov NCT04010227. Acceptance of cancer has long been recognized as playing a critical role in psychological adjustment to the illness, but its associations with distress outcomes have not been quantitatively reviewed. Informed by coping theory and third wave conceptualizations of acceptance, we first propose an integrated model of acceptance of cancer. Then we examine the strength of the relationships between acceptance of cancer and general and cancer-specific distress in cancer patients and potential moderators of these relationships. CINAHL, Embase, MEDLINE, PsycINFO, PsycARTICLES, and Web of Science databases were searched. Random-effects metaanalyses were conducted on 78 records (N=15,448). Small-to-moderate, negative, and significant relationships were found between acceptance of cancer and general distress (r=−.31; 95% CI:−.36 to −.26, k=75); cancer-specific distress (r=−.18; 95% CI:−.21 to −.14, k=13); depressive symptoms (r=−.25; 95% CI:−.31 to −.19, k=41); and anxiety symptoms (r=−.22; 95% CI:−.30 to −.15, k=29). Age, marital status, and stage of cancer were identified as significant moderators. Findings suggest that acceptance of cancer may be important to target in interventions to reduce general and cancerspecific distress in cancer patients. Future research should focus on developing multifaceted measures of acceptance and identifying theory-based psychological and social processes that lead to greater acceptance.
Background: Childhood chronic physical illness is associated with a greater vulnerability for emotional problems (i.e. depression and anxiety) in childhood. However, little is known about life-long effects of childhood chronic physical illness on mental health. The present study aims to systematically review evidence for associations between eight chronic physical illnesses with childhood onset (arthritis, asthma, cancer, chronic renal failure, congenital heart disease, cystic fibrosis, type 1 diabetes, and epilepsy) and adult emotional problems. Methods: A database search of MEDLINE, PsycARTICLES, PsycINFO, and ScienceDirect was undertaken, and random effects meta-analyses were used to synthesise evidence from eligible studies. Results: In total, 37 studies were eligible for the systematic review (n = 45,733) and of these, 34 studies were included in the meta-analyses (n = 45,358). There were overall associations between childhood chronic physical illness and adult depression (OR = 1.31; 95% CI [1.12, 1.54]) and anxiety (OR = 1.47; 95% CI [1.13, 1.92]). Separate meta-analyses for childhood asthma, type 1 diabetes and cancer were also conducted, with cancer being significantly associated with adult depression (OR = 1.19; 95% CI [1.00, 1.42]). Conclusions: The effects of childhood chronic physical illness on the risk of emotional problems persist beyond childhood and adolescence. Mental health prevention and intervention strategies targeting children with chronic physical illnesses can have long-term benefits.
Work values act as guiding principles for individuals' work-related behavior. Economic self-sufficiency is an important predictor for psychological well-being in adulthood. Longitudinal research has demonstrated work values to be an important predictor of economic behavior, and consequently of self-sufficiency. Socialization theories designate parents an important role in the socialization of their children to cultural values. Yet, extant literature is limited in demonstrating the role families play on how youth develop agentic pathways and seek self-sufficiency in transition to adulthood. This study presents a meta-analytic review investigating the intergenerational transmission of work values, which is frequently assessed in terms of parent-child value similarities. Thirty studies from 11 countries (N = 19,987; Median child age = 18.15) were included in the analyses. The results revealed a significant effect of parents on their children's work values. Both mothers' and fathers' work values, and their parenting behavior were significantly associated with their children's work values. Yet, similarity of father-child work values decreased as child age increased. Our findings suggest a moderate effect, suggesting the influence of general socio-cultural context, such as generational differences and peer influences, in addition to those of parents on youth's value acquisition. Our systematic review also revealed that, despite its theoretical and practical importance, social science literature is scarce in comprehensive and comparative empirical studies that investigate parent-child work value similarity. We discuss the implications of our findings for labor market and policy makers.
