Surgery is indicated in clinical stage II and cases of subclavian steal syndrome. Endarterectomy by mid-sternotomy is the operation of choice for obliteration of the innominate artery. The lethality rate is 6%. Occlusion of the left proximal subclavian artery can be treated by transthoracic endarterectomy or extrathoracic bypass procedure. The bypass procedure is preferred because of low operative lethality rate (0.8% versus 7% in transthoracic procedure). Occlusions of the right proximal subclavian artery are treated by supraclavicular endarterectomy.
Zusammenfassung 259 Operationen wurden an 236 Kranken wegen Veränderungen der extrakraniellen Hirnstrombahn von 1959 bis zum 10. 2. 1972 durchgeführt. 100 Eingriffe betrafen die Aortenbogenstämme, 148 die Karotisgabel und 11 den Vertebralisursprung. Die Gesamtletalität von 4,3 % ging im wesentlichen zu Lasten von Karotiseingriffen im Stadium III (drei Todesfälle bei vier Operationen). Nichtletale zerebrale Komplikationen traten in rund 10 % auf. 87 % der Kranken, die wegen anfallsweiser oder leicht provozierbarer Symptome im Stadium II operiert wurden, waren bis zur Entlassung beschwerdefrei oder wesentlich gebessert. Von 132 Kranken, die wegen zerebrovaskulärer Beschwerden vor 2 bis zu 12 Jahren operiert wurden, hatten 69 % keine Beschwerden und 2 % klagten über neue Symptome. Bei 15 inzwischen verstorbenen Patienten standen kardiale Ursachen (12 Fälle) ganz im Vordergrund. Die eigenen Erfahrungen und eine Literaturanalyse zeigen, daß mit der Verbesserung der Indikationsstellung, der Narkose-und Operationstechnik eine Risikosenkung und Verbesserung der Ergebnisse möglich ist.
