Os resultados pós-operatório s foram bem mais satisfatórios que as tentativas prévias com terapêuticas não cirúrgicas. Studies have described this illness in patients with neurological an d electroneurographic evidenc e of posterior tibial nerve compression 3 8 1 0 .From the pertinent literature, there seems t o be agreemen t tha t this syndrome constitute s a disorder primarily amenable to surgical intervention. The present work was carried out in a series of seven prospectively observed patients, in whom clinical management was unsatisfactory. The latter consisted of systemic administration of non-steroid anti-inflammatory agents, local steroid infiltrations, and a variety of podiatric procedures. Patients eventually underwent surgical decompression of the posterior tibial nerve within the tarsal tunnel. CASE REPORTSCase 1 . A 43-year-old woma n presente d wit h a 12-yea r histor y o f progressive burnin g pai n ove r the postero-lateral aspec t o f the right leg , irradiated t o the medial an d plantar region s o f the right foot . Thes e symptoms were exacerbated o n the upright position, and while deambulating. Relief was achieved during rest. Neurological examination disclosed mild deficit on plantar flexion of the right foot and hallux with hypoesthesia on the medial and dorsal aspects of the right foot. Painfu l symptom s could be triggered by pressure applied to the medial retromalleolar region. Electroneurographic testing revealed marked slowness in neuroconduction of the dista l segment s o f both posterio r tibia l nerves , mor e pronounce d o f the right side . Thes e abnormalitie
Intramedullary tumors are rare. The authors reviewed 24 cases operated between 1996 and 2006. The study assessed the clinical characteristics and surgical results based upon the neurological function. Method: Medical records of patients with intramedullary astrocytoma and ependymoma were reviewed. The minimal follow up time was 6 months and, at the end of this period, a comparative analysis of the neurological function was performed based using the McCormick scale score. results: Most patients had astrocytoma (75%). Male gender was more prevalent (58.3%). The most common type of tumor was graded as I or II, and in three cases these were malignant. The total resection of the tumor was achieved in 20.8% of the cases. The statistical analysis did not show a statistically significant difference between preoperative and postoperative grades at McCormick scale. conclusion: The authors concluded that microsurgery to intramedullary tumors did not significantly alter the neurological function after six months. Os autores analisaram 24 casos operados entre 1996 e 2006. O estudo analisou as características clínicas e o resultado da cirurgia quanto à função neurológica. Método: Foram analisados pacientes com astrocitomas e ependimomas intramedulares. O tempo mínimo de acompanhamento foi de 6 meses e ao final deste período foi realizada a avaliação comparativa da variação do estado neurológico baseado na escala de McCormick. resultados: A maioria dos pacientes era de astrocitoma (75%). O gênero masculino foi mais prevalente (58,3%). A maioria dos tumores era de grau I ou II, 3 casos eram malignos. A ressecção total do tumor ocorreu em 20,8% dos casos. A avaliação estatística demonstrou que não houve diferença significativa entre o estado neurológico na escala de McCormick pré-operatória e pós-operatória. conclusões: Os autores concluem que a microcirurgia para ressecção dos tumores intramedulares não ocasionou variação funcional significativa nos pacientes após seis meses da cirurgia. Resumo Objetivo: Os autores realizaram uma avaliação comparativa entre a abordagem anterior e posterior do nervo acessório para transferência nervosa para o nervo supraescapular. Método: Foram estudados pacientes submetidos à neurotização do nervo supraescapular utilizando como doador o nervo acessório, no período de janeiro de 2007 a janeiro de 2010. Os registros cirúrgicos foram avaliados para analisar as vantagens e desvantagens das duas formas de abordagem desses nervos na cirurgia. Foram avaliados os marcos anatômicos utilizados para a localização dos nervos durante a cirurgia e as dificuldades ocorridas nas duas formas de abordagem. Resultados: Foram estudados ao todo 29 pacientes, sendo que 22 deles foram abordados pela via anterior e sete por via posterior. A localização do nervo supraclavicular foi considerada mais fácil pela via posterior, enquanto a localização do nervo acessório foi feita mais rapidamente pela via anterior. Em nenhuma das cirurgias foi necessária a interposição de enxerto nervoso. Não ocorreram complicações cirúrgicas nas duas técnicas. Conclusão: A análise comparativa das duas abordagens cirúrgicas demonstra que a anatomia desses dois nervos é importante fator na dificuldade técnica apresentada na cirurgia. Novos estudos devem analisar se há uma melhora dos resultados funcionais com a abordagem pela via posterior.
Introduction: Aneurysmal bone cyst is a benign and destructive bone lesion of unknown etiology. Current approaches to treatment include surgical en bloc excision, curettage, selective arterial embolization and radiotherapy. A giant aneurysmal bone cyst in the lumbar vertebra is reported, along with a brief review of the literature on the topic. Method:A 15-yearold boy presents with lumbar pain and a 3 months history of paraparesis. Diagnostic imaging revealed an expansile lytic lesion in the spinous process and lamina of the fourth and fifth lumbar vertebra. Surgical treatment consisted of excisional biopsy and a segmental instrumented posterior fusion from L2-S1. Histopathology was consistent with an aneurysmal bone cyst. After one year, the patient improved his neurological function with only a L5 bilateral paresis. Discussion: Treatment of aneurysmal bone cysts of the spine is challenging when it occurs in close proximity to neural and vascular structures. Fusion and stabilization with instrumentation are often necessary. One vertebra is involved in most cases and extensive lesions are uncommon. Complete tumor resection and fusion of lumbar spine, using stabilization with instrumentation provide both cure and preservation of lumbar spine stability. IntroduçãoO hematoma intradiploico é uma condição clínica rara, tendo sido descrito originalmente por Chorobski em 1934.2,3 Os autores relatam um caso de um paciente com um grande hematoma intradiploico com extensão intra e extracraniana, que resultou em trombose da porção posterior do seio sagital superior. Existem poucos relatos na literatura sobre essa lesão expansiva de características não neoplásicas do crânio, que deve ser diferenciada de outras entidades clínicas.1,3 A maioria dos relatos realizados na literatura é de hematomas de pequeno volume e em pacientes pediátricos. Os autores descrevem um caso atípico de um hematoma intradiploico de grande volume em um paciente adulto e que ocasionou trombose de seio sagital superior, tendo sido o primeiro relato na literatura de estudo por angiografia digital de artérias carótidas e vertebrais, que demonstraram à trombose do seio secundário a compressão deste pelo hematoma crônico.
