queratometría media, paquimetría en el punto más delgado y cilindro refractivo, en dos momentos. Se empleó el análisis de varianza (ANOVA) de medidas repetidas para un factor con ajuste del intervalo de confianza por Bonferroni y valor p. Resultados: Se obtuvo un aumento estadísticamente significativo entre el valor promedio de queratometría media de la muestra en el momento del diagnóstico (49.6 D) y el valor promedio de queratometría media a los 3 años de seguimiento (52.4 D) (p = 0.02; α = 0.05); una disminución estadísticamente significativa entre la paquimetría promedio de la muestra en el momento del diagnóstico (482 µm) y la paquimetría promedio de la muestra a los 3 años de seguimiento (459 µm) (p = 0.03; α = 0.05); y un incremento estadísticamente significativo entre el cilindro refractivo promedio de la muestra en el momento del diagnóstico (4.6 D) y el cilindro refractivo promedio de la muestra a los 3 años de seguimiento (5.1 D) (p = 0.01; α = 0.05). Conclusiones: Las modificaciones de queratometría, cilindro refractivo y paquimetría sugieren progresión de la enfermedad en este grupo de edad, en un periodo de 3 años.
Por su apariencia antiestética y su naturaleza progresiva, el pterigion es una de las más antiguas afecciones oculares descritas. Desde tiempos remotos los más famosos médicos ya lo conocían y hacían descripciones precisas de sus diversas formas. Los estudios iniciales realizados por el médico hindú Susruta, considerado como el primer cirujano oftalmólogo, datan de mil años a.C. Describió con precisión el pterigion, su tratamiento, así como su fácil recidiva.Hipócrates (469 a.C.) sugirió el tratamiento con zinc, cobre, hierro, bilis, orina y leche materna. Celso (50 d.C.) y Galeno (131 d.C.) trataron el pterigion con soluciones de vino blanco, vinagre, azú-car, así como su pinzamiento y exéresis (1).En la medicina árabe algunos puntos son llamativos, como el tratamiento para el sangrado postoperatorio del pterigion con una mezcla de sal y alcaravea mascada y luego presionada a través de una pieza de ropa por donde se filtraban las gotas.En el texto oftalmológico «Memorial de los oculistas de Ali Ibn Isa», se utiliza la palabra «tan-wim», que significa literalmente «poner a dormir», cuando se refiere a las de cirugías oftalmológicas en pacientes que no pueden estarse quietos. Todas las traducciones de los textos al latín interpretan dicha palabra como dormir, o dar somníferos, o paciente dormido. Todas las citas en las que se usa la palabra «tanwim» se refieren a operaciones oftalmológicas largas y dolorosas (pterigion, pannus, etc.).Khalifah, del período tardío de la medicina árabe, nos deja en sus tratados ilustraciones de instrumentos oftálmicos, en un número superior que cualquier otro libro medieval (2). Encontramos en este manuscrito 36 ilustraciones en dos páginas, donde se explican algunos instrumentos de la cirugía de pterigion como:Miqrad: tijeras estrechas usadas para cortar el pterigion de la conjuntiva.Kaz: tijeras con una hoja más estrecha que la primera usadas para cortar el pterigion del limbo.Sananir: ganchos usados para levantar el pterigion y escindirlos.Asah: (la oja de un myrtle) usado para elevar el pterigion y pelarlo.El tiempo impuso retos superiores a esta historia. En el año 1872 Arlt describe la autoplastia conjuntival, proceder que ha pasado por distintas etapas de uso y desuso. Es una técnica factible de realizar en nuestro medio (3).Thoft introdujo la utilización de autoinjertos conjuntivales para la reconstrucción de la superficie ocular. La técnica se basa en el uso de membrana mucosa conjuntival normal con su red vascular superficial intacta y con las células de la superficie sanas de forma que se tiene una fuente normal de células caliciformes, de superficie columnar y de células madre para reponer la membrana lesionada.Con los nuevos caminos que año tras año fue tomando la cirugía de pterigion, se fueron observando diversas variaciones en el uso de la superficie de autoinjerto conjuntival, entre ellas: transposición conjuntival ortotópica libre, autoinjerto rotatorio conjuntival, transposición del colgajo conjuntival inferior, técnica de colgajo conjuntival mínimo y pedículo con...
Objetivo: Describir los hallazgos clínicos y tomográficos de un caso con toxoplasmosis ocular atípica. Caso clínico: Varón de 32 años que inició con disminución de la agudeza visual en el ojo izquierdo, con edema papilar y vítreo transparente, sin estrella macular. Presencia de IgM positiva para toxoplasmosis. Se muestran resultados de laboratorio, tomografía de coherencia óptica papilar y características de las fibras del nervio óptico. Una vez iniciado el tratamiento por toxoplasma, se obtuvo una mejoría clínica significativa, verificada con imágenes del fondo de ojo. Conclusiones: El edema papilar por toxoplasmosis con vítreo transparente es poco frecuente en la práctica. Sin embargo, es un cuadro que debe tenerse en cuenta en el diagnóstico diferencial como una presentación atípica de la toxoplasmosis ocular.
Establecer una comparación entre la técnica PRK convencional, asistida con alcohol (PRKaa), y la trans-PRK de un solo paso, en cuanto a variables clínico-refractivas y quirúrgicas. Método: Se realizó un estudio observacional, prospectivo, longitudinal, en 72 pacientes candidatos a cirugía refractiva corneal con excímer láser, con el diagnóstico de astigmatismo miópico compuesto, en el Centro Oftalmológico Exiláser, Cuenca, Ecuador, de septiembre a diciembre de 2019. Los pacientes fueron intervenidos por técnicas de superficie (trans-PRK o PRKaa). El procesamiento de los datos se realizó en el programa SPSS, versión 21.0. Para la comparación de los datos de ambas técnicas quirúrgicas se empleó la prueba de chi cuadrado, donde se consideró p < 0.05 estadísticamente significativo. Resultados: Se obtuvo como equivalente esférico promedio para PRKaa −0.14 y para trans-PRK −0.11, a los 3 meses del procedimiento (p = 0.34). El promedio de agudeza visual sin corrección obtenido fue en PRKaa de 0.93 y en trans-PRK de 0.96, sin diferencias estadísticamente significativas (p = 0.63). El tiempo quirúrgico para PRKaa fue de 1,080.85 s y para trans-PRK fue de 720.31 s, menor en esta última con diferencia estadísticamente significativa (p < 0.001). En trans-PRK hubo menor dolor inmediatamente después de la cirugía, comparado con PRKaa (p < 0.05). El día promedio de cierre epitelial en PRKaa fue el 6.27 y en trans-PRK el 3.62 (p = 0.02). conclusiones: La PRK-transepitelial de un solo paso y la PRKaa realizada en pacientes candidatos a cirugía refractiva producen resultados muy similares a los 3 meses de la cirugía en cuanto a AVSC, equivalente esférico obtenido y mínimas complicaciones postoperatorias. La trans-PRK ofrece mayores ventajas al paciente, referentes a menor tiempo quirúrgico, rápido cierre epitelial y menor dolor en el postoperatorio inmediato.
Purpose:The aim of the study was to describe the clinical and tomographic findings of a case with atypical ocular toxoplasmosis. Observations: This is the case of a 32-year-old patient who presented with decreased visual acuity in the left eye, papillary edema, and transparent vitreous without a macular star positive IgM test for toxoplasmosis. Laboratory test results, papillary OCT, and the characteristics of optic nerve fibers are shown. After Toxoplasma gondii was treated, significant clinical improvement was seen in the patient that was confirmed on the images of the eye fundus. Conclusions: Papillary edema due to toxoplasmosis with clean vitreous is not a common finding in the routine clinical practice. However, it should be taken into account while performing differential diagnosis as an atypical presentation of ocular toxoplasmosis.
