El propósito de esta publicación es la de describir los fundamentos de los estudios diagnósticos, la de proporcionar elementos de juicio para valorar la validez, puntualizar sobre las medidas de utilidad o rendimiento de las mismas y resaltar la importancia de los estudios diagnósticos en la práctica clínica usual. Dentro de los fundamentos de las pruebas diagnósticas se explican los principios básicos sobre las pruebas diagnósticas, la estructura de estas y se detalla cómo se analizan los resultados de la prueba diagnóstica de interés y los resultados de la prueba diagnóstica de referencia. Se evalúa la validez de una prueba diagnóstica examinando tres criterios importantes que sustentan la solidez metodológica de este tipo de estudio: Representatividad, Constatación y Determinación. Por otro lado, se estudia el primer tipo de medidas de rendimiento de una prueba diagnóstica: Medidas de probabilidad (sensibilidad, especificidad, valor predictivo positivo, valor predictivo negativo). Cuando los resultados de una prueba diagnóstica son expresados en una escala cuantitativa continua es necesario escoger “el mejor punto de corte” que se corresponda con una alta sensibilidad y especificidad, para así distinguir los enfermos de los sanos. Con este fin, podemos utilizar las propiedades de la curva COR (acrónimo de Característica Operativa del Receptor) para elegir el mejor punto de corte. En la presente publicación se describe y pormenoriza la construcción de la curva COR y se puntualiza sobre la mejor manera de analizarla y sacarle provecho. Por último, se ilustra cómo un estudio diagnóstico cambia el paradigma de manejo de una enfermedad.
Introducción. Un biomarcador se define como una alteración molecular presente en el desarrollo de la patogénesis del cáncer, que puede ser utilizada para el diagnóstico temprano de la enfermedad. La medición del biomarcador se hace por medio de diversas técnicas, como bioquímica, inmunohistoquímica o biología molecular, en diferentes tipos de muestras, como tejido, sangre periférica y orina. El biomarcador ideal será aquel que sea válido y específico a la vez, que sea no invasivo, barato y fácilmente detectable. El uso de biomarcadores para la detección temprana del cáncer debe seguir un desarrollo ordenado y sistemático antes de introducirlos en la práctica clínica. Métodos. Se realizó una búsqueda exhaustiva en las bases de datos de PubMed y Embase, seleccionando los artículos pertinentes para revisarlos acorde a la temática específica de interés. Resultados. Se propone la sistematización del desarrollo de biomarcadores en cinco grandes fases, las cuales tienen la característica de ser ordenadas desde las evidencias más tempranas hasta las fases finales de su estudio. Conclusiones. El correcto desarrollo de biomarcadores hace posible la introducción de intervenciones terapéuticas en el ámbito de la prevención secundaria del cáncer.
La utilidad de una prueba diagnóstica se cuantifica mediante el cálculo de dos medidas: Medidas de probabilidad. Medidas de razones de probabilidad. Las medidas de probabilidad son la sensibilidad, la especificidad, valor predictivo positivo y el valor predictivo negativo. La sensibilidad y la especificidad de una prueba diagnóstica se usan para escoger la mejor prueba a utilizar, entre varias pruebas diagnósticas disponibles; sin embargo, estas dos medidas no pueden ser utilizadas para estimar la probabilidad de determinada enfermedad en un paciente en particular. En la práctica clínica usual, es esencial saber cuál es la probabilidad que un paciente con un resultado positivo en una prueba diagnóstica presente la enfermedad; y cuál es la probabilidad que un paciente con un resultado negativo en una prueba diagnóstica no presente la enfermedad. Los valores predictivos positivos y negativos nos brindan la respuesta a esta cuestión. Sin embargo, los valores predictivos positivo y negativo dependen tanto de la sensibilidad y especificidad así como de la prevalencia de la enfermedad en la muestra del estudio. Las medidas de razones de probabilidad son la segunda forma de describir el rendimiento o utilidad de una prueba diagnóstica. Las razones de probabilidad poseen dos propiedades importantes: Resumen el mismo tipo de información que la sensibilidad y la especificidad. Pueden utilizarse para calcular la probabilidad de la enfermedad después de una prueba positiva o negativa. El propósito de esta publicación es definir el definir el concepto de razones de probabilidad, describir sus principales fortalezas y explicar cómo se calculan las razones de probabilidad cuando la prueba de interés expresa sus resultados en forma dicotómica (positivo / negativo), en más de dos categorías o de forma ordinal.
Introducción. Debido a que el cáncer de seno es una enfermedad asociada a una significativa tasa de morbilidad y mortalidad cuando se diagnostica en el período sintomático, se han hecho enormes esfuerzos orientados hacia la prevención primaria de esta enfermedad. Métodos. Se realizó una búsqueda de todos los experimentos clínicos aleatorizados que evaluaran la eficacia de la terapia endocrina para la reducción del riesgo de desarrollar cáncer de seno. La calidad metodológica de los estudios seleccionados fue valorada utilizando la herramienta de la Colaboración Cochrane para medir el riesgo de sesgo en ensayos aleatorizados. Se evaluó la heterogeneidad de los estudios primarios elegibles utilizando los estadísticos T², I², H². El sesgo de publicación fue evaluado mediante el test de Harbord y mediante la gráfica de funnel plot. La medida de efecto utilizada en este metaanálisis fue el riesgo relativo (RR) con el cálculo de los intervalos de confianza (IC) del 95%. Resultados. Encontramos doce experimentos clínicos aleatorizados que reclutaron a 68.180 mujeres, las cuales fueron asignadas al azar para recibir algún tipo terapia endocrina para reducir el riesgo de desarrollar cáncer de seno o placebo. La terapia endocrina en conjunto redujo el riesgo proporcional de cáncer de seno (invasivo más in situ) en un 42 %, resultado estadísticamente significativo RR 0,58 (IC95% 0,50 – 0,69). Conclusiones. La terapia endocrina es el manejo estándar de prevención en mujeres sanas con riesgo de desarrollar cáncer de seno no hereditario.
Introducción. En el presente artículo se propone un nuevo índice de desempeño global de una prueba diagnóstica: el índice T, que corresponde a la proporción de aciertos (Pa) menos la proporción de error (Pe) dividido entre la proporción de aciertos (Pa). Índice T= [(a+d) / N] – [(c+b) / N] × 100 ___________________________ [(a+d) / N] Métodos. Describir los fundamentos teóricos y prácticos del índice T. Comparar el índice T frente al índice de Youden para poder establecer si dos o más modalidades diagnósticas difieren en su capacidad de discriminar, correctamente, entre los individuos verdaderamente enfermos de los sanos. Comparar el índice T frente al índice de Youden para poder establecer si el índice T puede ser utilizado para escoger “el mejor punto de corte” cuando la prueba diagnóstica de interés expresa sus resultados en forma cuantitativa continua. Resultados. Comparado frente al índice de Youden, el índice T permite distinguir entre las pruebas diagnósticas con alto desempeño diagnóstico de aquellas con pobre desempeño diagnóstico. Además, el índice T, de manera confiable, permite escoger “el mejor punto de corte” cuando una prueba diagnóstica utiliza una variable cuantitativa continua para expresar sus resultados. Conclusiones. Al contrastar los resultados del índice T versus el índice de Youden y los resultados del índice T versus la curva COR, podemos afirmar que el índice T es un índice exacto y eficiente.
