Saúde Pública, 30 (5): 460-70, 1996 460Resumo São apresentados os resultados da avaliação da vacina antimeningocócica BC de procedência cubana realizada com dados da vigilância epidemiológica de meningites do Estado de Santa Catarina, Brasil, dos anos de 1990-92. A eficá-cia para os casos confirmados por exames bacteriológicos foi de 59% (IC de 95%: 23 a 78%) para os menores de 4 anos e de 78% (IC de 95%: 54 a 90%) para as crianças de 4 a 7 anos. Com os casos confirmados como causados por sorogrupo B, as eficácias estimadas foram maiores, mas se ampliaram os intervalos de confiança. O efeito protetor da vacina em relação à mortalidade em crianças menores de 4 anos foi estimado em 76% (IC 95%: 41 a 91%). Outros dados apresentados sugerem que as mais baixas eficácias encontradas, quando se analisam os menores de 4 anos, decorrem da seletividade da confirmação diagnóstica que exclui os casos mais graves que morrem a poucas horas do início dos sintomas, associada à modificação do curso da doença (atenuação) em vacinados. Uma pesquisa de comportamento de risco ao HIV foi conduzida entre conscritos (17-20 anos) do Exército em 1998. A amostra (30.318) foi selecionada em três estratos: 1) municípios das regiões Norte e Centro-Oeste (N/CO); 2) municípios da região Sul (S); 3) municípios dos estados do Rio de Janeiro e São Paulo (RJ/SP), estimando-se a prevalência de infecção pelo HIV em cada estrato. O objetivo deste trabalho foi o de apreciar os resultados por nível sócio-econômico (NSE). A análise estatística utilizou um índice de comportamento sexual de risco para HIV e modelos de regressão logística. O estrato N/CO apresentou os piores indicadores - os relativos ao NSE, ao comportamento sexual de risco ou aos problemas relacionados às infecções sexualmente transmissíveis (IST) - assim como a maior proporção de infectados por HIV. Os melhores indicadores foram encontrados no estrato RJ/SP. O Sul mostrou resultados intermediários. O grau de escolaridade teve papel relevante. Os conscritos com primeiro grau incompleto apresentaram o pior índice de comportamento sexual de risco, o qual se mostrou importante preditor dos problemas relacionados às IST, incluindo a infecção por HIV.
Este artigo analisa a validade de técnicas diagnósticas ou de medidas de fatores de risco, através dos seus erros de classificação, e seus efeitos sobre indicadores de risco, próprios de grupos, bem como sobre o risco relativo e a razão de produtos cruzados, obtidos em estudos epidemiológicos analíticos. É demonstrado como os erros de classificação podem distorcer os resultados de inquéritos e estudos de cortes ou de casos e controles. Porém, são propostas formas simples de correção dos indicadores obtidos, sempre que se conhecem estimativas de sensibilidade e especificidade das técnicas utilizadas.
This paper analyses validity of techniques, either for diagnosis or for measures of risk factors, by means of their misclassifications and their effects upon risk measures of groups as well as upon relative risk and odds ratio obtained in analytical studies. It is presented how misclassifîcation can distort results of cross-sectional and cohorts or case-controls studies. Nevertheless simple ways of correcting the results are suggested provided sensibility and especificity estimates of the techniques employed are known

Existem quatro crenças altamente difundidas em relação às doenças cardiovasculares: 1 -que são doenças próprias das pessoas mais ricas; 2 -que são doenças de países desenvolvidos; 3 -que são doenças não-redutíveis, isto é, são uma fatalidade ligada ao envelhecimento das pessoas; São discutidas quatro crenças muito comuns em relação4 -que são doenças da urbanização.Esse conjunto de crenças leva não só a um imobilismo sanitário, como também tem levado a um outro conjunto de crenças causais, que vão desde o excesso de preocupação pelas responsabilidades dos ricos até o estresse do cotidiano urbano, passando pelo medo e tensão frente ao novo, ao desconhecido.Tal conjunto de crenças poderia ser facilmente testado, se as estatísticas dos países subdesenvolvidos não fossem tão falhas e se, nas áreas rurais e entre os grupos menos privilegiados das populações, não fossem os recursos médicos mais escassos. Aliás, esses são motivos que podem determinar os achados que parecem confirmar aquelas crenças. Por isso, esses dados devem ser submetidos a severas críticas.
HIV-1-infected patients co-infected with A(H1N1)pdm09 surprisingly presented benign clinical outcome. The knowledge that HIV-1 changes the host homeostatic equilibrium, which may favor the patient resistance to some co-pathogens, prompted us to investigate whether HIV-1 infection could influence A(H1N1)pdm09 life cycle in vitro. We show here that exposure of A(H1N1)pdm09-infected epithelial cells to HIV-1 viral particles or its gp120 enhanced by 25% the IFITM3 content, resulting in a decrease in influenza replication. This event was dependent on toll-like receptor 2 and 4. Moreover, knockdown of IFITM3 prevented HIV-1 ability to inhibit A(H1N1)pdm09 replication. HIV-1 infection also increased IFITM3 levels in human primary macrophages by almost 100%. Consequently, the arrival of influenza ribonucleoproteins (RNPs) to nucleus of macrophages was inhibited, as evaluated by different approaches. Reduction of influenza RNPs entry into the nucleus tolled A(H1N1)pdm09 life cycle in macrophages earlier than usual, limiting influenza's ability to induce TNF-α. As judged by analysis of the influenza hemagglutin (HA) gene from in vitro experiments and from samples of HIV-1/A(H1N1)pdm09 co-infected individuals, the HIV-1-induced reduction of influenza replication resulted in delayed viral evolution. Our results may provide insights on the mechanisms that may have attenuated the clinical course of Influenza in HIV-1/A(H1N1)pdm09 co-infected patients during the recent influenza form 2009/2010.
