Objetivo: Relatar um caso de um paciente com fratura exposta de cotovelo conduzida com tratamento cirúrgico definitivo na abordagem inicial em um cenário limítrofe para possível amputação ou fixação externa para controle de danos locais. Detalhamento do caso: Masculino, 36 anos, vítima de acidente automobilístico. O paciente apresentava uma lesão extensa de partes moles e exposição óssea, nervosa e vascular. Foi realizado o diagnóstico de fratura de cotovelo através das radiografias, sendo conduzido a fixação definitiva das fraturas com placas e parafusos. Posteriormente, fez-se o seguimento com procedimentos adicionais a fim de realizar o tratamento para rigidez residual pós-traumática. Após 2 anos de seguimento, o paciente alcançou um resultado amplamente funcional e encontra-se satisfeito com o desfecho atual. Considerações finais: O manejo desses casos complexos continua um desafio para a comunidade médica em geral e, principalmente, para os cirurgiões ortopedistas. Em algumas oportunidades, a primeira abordagem pode ser a única chance de salvação do membro do paciente mesmo quando o contexto é desafiador. Portanto, apresentamos um relato onde demonstramos um resultado aceitável nesse contexto a fim de encorajar os cirurgiões a sempre considerar essa opção terapêutica.
