Introduction: Von Hippel-Lindau disease (VHL) is autosomal dominant tumor syndrome that debuts mostly in young adults , patients with this disease are linked to the triggering of various types of benign and malignant neoplasms in multiple locations, systems and organs, in particular affecting more the nervous system and other internal organs. Approximately this tumor syndrome shows an incidence rate of 1 in 36,000 live births with a penetrance greater than 90%. The molecular basis of VHL disease is the impairment of VHL protein function and the consequent clustering of hypoxia-inducible factors with subsequent consequences on cell differentiation and metabolism. Objective: to present current information related to Von Hippel-Lindau disease, description, genetics, molecular basis, classification and manifestations of the disease. Methodology: a total of 33 articles were analyzed in this review, including review and original articles, as well as clinical cases, of which 26 bibliographies were used because the other articles were not relevant to this study. The sources of information were PubMed, Google Scholar and Cochrane; the terms used to search for information in Spanish, Portuguese and English were: Von Hippel-Lindau, VHL, tumor suppressor gene, pheochromocytoma, hemangioblastomas. Results: VHL has an incidence of approximately 1 in 36,000 live births, with a penetrance of over 90%. These present tumors are initiated by inactivation of biallelic VHL and are related to pathologic activation of hypoxic gene response pathways. Conclusions: Von Hippel-Lindau disease is an autosomal dominant disorder that is generated by mutations in the VHL tumor suppressor gene. Within the field intrafamilial variation may evidence correctly shaped genotype-phenotype connections for renal cancer and pheochromocytoma risks. Visceral cysts (renal, pancreatic and epididymal) are frequent, however organ function involvement is rare. They usually occur with hemangioblastomas of the central nervous system and retina, as well as renal cancers. Unusually it includes non-functioning pancreatic endocrine cancers, adrenal and extra-adrenal pheochromocytomas, endolymphatic sac tumors, as well as head and neck paragangliomas. Introducción: La Tromboembolia Pulmonar (TEP) constituye una causa importante de hospitalización, morbilidad y mortalidad. Al momento actual representa una de las enfermedades cardiovasculares más frecuentes. Caso clínico: Paciente femenina de 73 años con diversas comorbilidades, que ingresa por un cuadro de dificultad respiratoria severa e inestabilidad hemodinámica, se diagnóstica de neumonía, hipertensión pulmonar y además en una Tomografía Axial Computarizada se evidencia masa tumoral con hemorragia alveolar. Evolución: Durante la estancia hospitalaria, la paciente se mantiene con necesidades altas de oxígeno e inestabilidad hemodinámica a pesar de maniobras de soporte. La hemorragia alveolar que presenta, condiciona el tratamiento trombo profiláctico. El estudio de imagen indica masa tumoral asociado a hemorragia alveolar y la probabilidad muy elevada de desarrollar una trombosis venosa profunda (TVP) y posterior tromboembolia pulmonar (TEP). Motivo por el cual se realiza eco FAST el cual evidencia trombosis venosa profunda en vena femoral común derecha, oclusión total de arteria femoral superficial con flujo conservado en arteria femoral profunda. Pulsos distales ausentes con flujo arterial en tibial anterior monofásico con diástole alta IR: 0.6, e índice Tobillo brazo 0.5, posterior ecocardiograma con dilatación de cavidades derecha, PSAP de 54 mmHg y signo de Mc Connel. Paciente empeora su cuadro con una tromboembolia pulmonar masiva que resulta en fallecimiento. Conclusiones: La embolia pulmonar es una patología que se presenta 1 por cada 1000 casos. El individualizar cada caso puede ser decisivo para salvaguardar la vida de un paciente.
La insuficiencia cardíaca es un síndrome clínico causado por trastornos estructurales o funcionales asociados a una disminución del gasto cardiaco y aumento de la congestión. La pancreatitis aguda se caracteriza por la activación desmedida de la tripsina, provocando la auto digestión del tejido pancreático, en un 20% de los casos puede culminar en falla multiorgánica debido al síndrome de respuesta inflamatoria sistémica. Presentamos el caso de una paciente femenina de 70 años de edad con múltiples comorbilidades que presenta un cuadro compatible con insuficiencia cardiaca ingresada por emergencia con inestabilidad hemodinámica y la clínica clásica de un edema agudo de pulmón; durante el periodo de hospitalización se evidencia deterioro en el cuadro presentándose signos de pancreatitis grave y falla multiorgánica.
