ObjectiveThe purpose of the present study was to describe an OrBitoZygomatic (OBZ) surgical variant that implies the drilling of the orbital roof and lateral wall of the orbit without orbitotomy.MethodsDesign : cross-sectional study. Between January 2010 and December 2014, 18 patients with middle fossa lesions underwent the previously mentioned OBZ surgical variant. Gender, age, histopathological diagnosis, complications, and percentage of resection were registered. The detailed surgical technique is described.ResultsOf the 18 cases listed in the study, nine were males and nine females. Seventeen cases (94.5%) were diagnosed as primary tumoral lesions, one case (5.5%) presented with metastasis of a carcinoma, and an additional one had a fibrous dysplasia. Age ranged between 27 and 73 years. Early complications were developed in four cases, but all of these were completely resolved. None developed enophthalmos.ConclusionThe present study illustrates a novel surgical OBZ approach that allows for the performance of a simpler and faster procedure with fewer complications, and without increasing surgical time or cerebral manipulation, for reaching lesions of the middle fossa. Thorough knowledge of the anatomy and surgical technique is essential for successful completion of the procedure.
Las fístulas del líquido cefalorraquídeo a través de la pared posterior del clivus son muy raras hasta el 2015 sólo se han reportado 16 casos. Estas son un serio problema y una condición potencialmente fatal cuyo manejo requiere de un abordaje multidisciplinario. Se presenta el caso de una mujer de 56 años con un cuadro de 6 meses de evolución, de aparición espontánea, caracterizado por rinorrea, y cefalea holocraneana intermitente de leve a moderada intensidad. En este caso se reparó la fístula con un injerto de cartílago septal reforzado con un colgajo de mucosa nasoseptal y pegamento de fibrina, se obtuvo un resultado exitoso.
Estadísticamente los melanomas malignos intracaneales en la población es de aproximadamente 0.005 casos/ 100,000 habitantes, con una prevalencia hacia la población masculina. Se han descrito principios para el abordaje diagnóstico, así como para el tratamiento quirúrgico y adyuvante oncológico. Presentamos el caso de un paciente de 39 años de edad, con evolución caracterizada por cefalea, lateralización de la marcha, dismetría, disdiadococinesia derechas, ataxia troncal y Romberg. Iniciando el protocolo diagnóstico, se identifica por resonancia magnética presencia de una lesión en fosa posterior de mayor afección cerebelosa derecha, heterogénea, con edema importante. Por lo cual es programado para manejo quirúrgico, logrando resección total de la lesión y con reporte posterior por el servicio de patología clínica como melanoma. Se realizan pruebas de inmunohistoquímica siendo positivas S100 y HMB 45. En los estudios de extensión por oftalmología, dermatología y gastroenterología con realización de panendoscopía no fue posible localizar otro foco de crecimiento tumoral. El estudio de tomografía por emisión de positrones no demostró positividad en otro lado, salvo el crecimiento intracraneal previamente conocido. Conclusión: las pautas del diagnóstico de melanoma de sistema nervioso central hacen indispensable investigar exhaustivamente posibles sitios de origen y descartar lesiones metastásicas; respecto al manejo, se debe considerar la ablación total siempre que sea posible; así como, uso de terapia oncológica agregada.
