OBJETIVO: Comparar la eficacia y seguridad de la dosis de 50 microgramos de misoprostol, administrado en pincelación cervical, con dosis convencionales de oxitocina, vía intravenosa (iv.) continua, en pacientes con amenorrea prolongada y cervix desfavorable.
DISEÑO DEL ESTUDIO: Se hizo un estudio prospectivo aleatorizado y controlado. La variable primaria de salida fue el intervalo entre la iniciación de la inducción y la expulsión del feto. Las variables secundarias son los intervalos en los cuales se puede subdividir la variable primaria: intervalo entre iniciación de la inducción y la iniciación del parto, la realización de la amniotomia y la iniciación del período expulsivo; así como: la aparición de hemorragias, hiperactividad uterina, sufrimiento fetal, índice de Apgar del recién nacido y peso fetal al nacer.
RESULTADOS: Se estudiaron 89 pacientes, distribuidas así: 47 en el grupo que recibió misoprostol y 42 en el grupo que recibió oxitocina. No se encontraron diferencias significativas entre los dos grupos estudiados. La fase latente del parto fue dos horas más corta en el grupo de misoprostol, (x = 8.16 ± 1.05 h.) vs. oxitocina (x = 10.29 ± 1.5 h.), diferencia que no fue estadísticamente significativa (p: 0.1401).
CONCLUSION: Siendo la administración de misoprostol de más fácil aplicación, menor costo, y tan eficaz como la oxitocina iv. en la inducción del parto, en casos de amenorrea prolongada, resulta aconsejable su uso.
Se presentan los datos obtenidos mediante la implantación de un modelo de Historia Clínica Perinatal y la realización manual de su análisis, en 8.181 pacientes hospitalizadas en el Departamento de Obstetricia y Ginecología del Hospital Universitario del Valle. Este artículo es solo descriptivo y por ello solo permite algunas pocas conclusiones y recomendaciones, expuestas al final del mismo.
Se estudia el efecto de los fármacos estimulantes de los adrenoceptores beta administrados por vía sublingual, sobre la contractilidad uterina durante la menstruación. Los fármacos estudiados fueron el Th-1165-a (Partusisten) y la Orciprenalina (Alupent).Se registró la contractilidad uterina mediante un catéter abierto de polietileno con esponja terminal. Se analizaron cuantitativamente los registros antes y después de la administración sublingual de los fármacos mencionados. También se analizó el efecto de ellos sobre la osmolaridad urinaria mediante recolección continua de la orina.Con ambos fármacos se objetivó una disminución estadísticamente significativa de la intensidad, tono, frecuencia y actividad uterina en todas las pacientes. En las pacientes que presentaban dismenorrea esencial, concomitantemente con la disminución de la contractilidad uterina hubo abolición subjetiva del dolor pélvico.La Orciprenalina produjo además, disminución significativa de la osmolaridad urinaria.Se concluye sugiriendo la posible utilidad terapéutica de estos fármacos en la clínica ginecológica en pacientes con dismenorrea esencial.

El conocimiento de las alteraciones producidas por el desprendimiento prematuro de la placenta normalmente insertada (Abruptio Placentae), sobre la actividad contráctil del útero humano, es aún insuficiente.
