Antecedentes: La Macroglobulinemia de Waldenström (MW) es un desorden proliferativo en el cual está presente un Linfoma Linfoplasmocitoide (LLP) en la médula ósea y asociado a una gammapatía monoclonal IgM. Con una incidencia global de 3 casos por millón de personas-año, representa aproximadamente el 2% de todas las neoplasias hematológicas. Este es el primer caso documentado de MW en Honduras con confirmación molecular de la mutación MYD88 p.L265P. Descripción del caso: Paciente de 58 años de edad evaluado en 2017 con informe de biopsia que reporta infiltración linfoplasmocitaria en glándula salival del labio inferior, refiriendo 3 meses de epistaxis y gingivorragia, anemia severa, leucopenia, episodios de lipotimia y pérdida de peso. Al examen físico presentó agrandamiento bilateral de glándula parótida y submandibular, múltiples adenopatías cervicales, alteración en glándula tiroides, hepatomegalia leve y esplenomegalia masiva. Se encontraron valores elevados de TSH, microglobulina y LDH; eletroforesis y citometría de flujo de médula ósea sugestiva de desorden de IgM monoclonal. Se logró confirmación molecular mediante la detección de la mutación L265P en el gen MYD88. El paciente recibió esquema de primera línea con Ciclofosfamida, Adriamicina, Vincristina, Prednisona y Rituximab(CHOP-R), luego por recaída recibió mismo esquema por haber mostrado buena respuesta. Posteriormente, tras una nueva recaída recibió tratamiento con Chlorambucil presentando nuevamente recaída. Se diagnosticó con MW refractaria y es candidato para recibir Ibrutinib; no disponible en nuestro medio. Conclusión: La escasa casuística de enfermedad de MW en Honduras dificulta el diagnóstico oportuno y tratamiento óptimo en nuestro medio.
Introduction:In the earliest cases of COVID-19, a higher percentage of severe and fatal cases was observed in patients with cancer, including those with haematological malignancies. However, patients with chronic myeloid leukaemia (CML) had better prognoses, suggesting that tyrosine kinase inhibitors (TKIs) may have a therapeutic effect against SARS-CoV-2. This study describes the clinical and epidemiological characteristics of patients with CML receiving the TKIs tested for SARS-CoV-2 in Tegucigalpa, Honduras.Methodology: An Analytical cross-sectional study was conducted. The sample included patients with Philadelphia chromosome-positive (Ph+) CML, who had been tested at least once for COVID-19 at the Emma Romero de Callejas Cancer Centre (CCERC). Sociodemographic and clinical variables were both analysed. Epi Info 7.2.4.0 and Stata/ MP 16.0 were used to collect and analyse data. The COVID-19 positivity percentage and the association between severity and the TKI used were determined using Fisher's exact test and odds ratio (OR). Data were gathered from clinical records with approval of CCERC institutional management.Results: One hundred and forty-nine patients with Ph+ CML were included; 20.1% were COVID-19-positive; 56% were male; mean age was 46 years; 81% were receiving imatinib, with a mean treatment duration of 6 years; 55% achieved a BCR -ABL molecular response ≤ 0.1% (IS). Twenty-one percent had comorbidities. COVID-19 was asymptomatic in 38.7% of patients, mild in 35.5% and severe in 9.7%. One patient died, a fatality rate of 3.2%. No statistical association was found between disease severity and treatment with imatinib versus second-line TKI (OR: 0.833, p: 0.8493, 95% CI: 0.098-10.998). Conclusion:Despite high COVID-19 positivity in CML when compared with the literature, this study found a lower fatality rate. The type of TKI used or molecular response at the time of infection was not associated with case severity. Determining the effectiveness of imatinib or other TKIs as a COVID-19 treatment requires randomised clinical trials.
Antecedentes: La enfermedad de Gaucher es causada por la acumulación del glicolípido glucosilceramidasa en el sistema macrofagocítico debido a la deficiencia de la enzima glucocerebrosidasa. Se han descrito tres formas clínicas de la enfermedad. La prevalencia mundial es de 1 caso en 40,000- 100,000 nacimientos, aunque en poblaciones de ascendencia judío askenazi puede ser tan alta como 1 en 850 individuos. El diagnóstico definitivo se obtiene a través de observación directa de células Gaucher en médula ósea y la confirmación diagnóstica de las mutaciones en el cromosoma 1q21.31. Descripción del Caso: Femenina de 30 años con historia de 4 meses de hepatoesplenomegalia, leucopenia, neutropenia y trombocitopenia. Células de Gaucher se observaron en biopsia de médula ósea. Se confirmaron las variantes heterocigotas en los Exones 4 y 10 del Cromosoma 1q2. La paciente fue tratada con 5 mg de ácido fólico y multivitaminas. Es candidata para Terapia de Reemplazo de la Enzima con imiglucerasa, sin embargo, éste no está disponible en Honduras por su alto costo. Paciente continúa en seguimiento 5 años después del diagnóstico. Conclusión: La incidencia en Honduras es desconocida. Otros casos podrían permanecer sin ser diagnosticados debido a la complejidad del diagnóstico en el primer nivel de atención. Las enfermedades metabólicas son un reto para el sistema de salud debido al acceso limitado a la atención médica de calidad y a la escasez de médicos especialistas capacitados. Una de las fortalezas de este caso es el estudio laboratorial completo que incluye confirmación genética de la enfermedad.
