Это-«большая» ревматология, область высоких достижений, где нашли практическое применение последние открытия медицинской науки и новейшие разработки фармакологической индуст-рии. Сегодня основой успешного лечения ревматических заболеваний являются точное знание главных элементов их патогенеза, использование методик ранней диагностики, в том числе высокоточных лабораторных и инструментальных тестов, обязательная оценка тяжести болезни с помощью стандартизированных индексов, а также применение целенаправленной (таргетной) патогенетической терапии [1, 2].
Аbstract. The current regulatory framework of the Ministry of Health and the Ministry of Defense of the Russian Federation on the regulation of the provision of medical assistance to military personnel affected by the infection caused by the SARS-CoV-2 coronavirus is considered. The state of the problem of information support for the provision of medical care to military personnel of this category is assessed. The developed methodology and the draft regulatory document for organizing the collection of medical information in the register of military personnel affected by the infection caused by the SARS-CoV-2 coronavirus are presented, and a regulation on this register is developed. The content, scope and algorithm of creating an information-analytical system for registering this pathology among military personnel, as well as the need to introduce new medical document management technologies into the medical service of the Armed Forces of Russia based on personified records of military personnel and the medical and psychological assistance provided to them, are substantiated. It is shown that for the smooth functioning of the register within the framework of a justified list of tasks, the presence of high-speed secure communication channels between the participating military units, institutions and medical service management bodies will be required. Consequently, the improvement of the information support system for managing the medical support of troops in peacetime and wartime is an important component of increasing the effectiveness of managing medical support for military.
Objective: to justify the main directions of optimizing the medical care system for HIV-infected military personnel of the Ministry of Defense of Russia on the basis of studying the clinical, epidemiological and social characteristics of. servicemen.The materials consisted of 188 case histories of servicemen undergoing examination and treatment in the only specialized department for military medical examination and treatment of the Ministry of Defense of Russia. In the processing and analysis of data we used modern methods of descriptive statistics.Results of the study. HIV infection rate of the servicemen in Russian Federation is at a consistently low level. HIV-positive servicemen are characterized by: sexual heterosexual way of infection; high social status (income, education, housing conditions), low prevalence of associated pathology. Access of most HIV-positive military personnel to dispensary observation (76,6%) and receiving antiretroviral therapy (66,1%) in the centers for the Prevention and Control of AIDS and Infectious Diseases has already been implemented, despite the lack of a regulatory framework of the Russian Ministry of Defense. A high level of adherence to dispensary observation and receiving antiretroviral therapy (about 95%) was noted in HIV-positive military personnel. Ensuring the legitimate access of HIV-infected military personnel to antiretroviral therapy practically does not require an increase in funding.
