Summary Ten subjects were treated orally with acitretin (Ro 10–1670) over a period of 3 months. Before, during and after treatment, semen and blood analyses were performed to investigate drug‐induced impairment of spermatogenesis and the hypothalamic — pituitary — gonadal axis. Our study concludes that acitretin in therapeutically effective doses does not influence spermatogenesis, sperm morphology, sperm motility and the hypothalamic — pituitary — gonadal axis.
Zusammenfassung Zehn Probanden erhielten über einen Zeitraum von drei Monaten Acitretin (Ro 10–1670) per os. Vor, während und nach der Behandlung wurden Samen‐ und Blutanalysen durchgeführt, um den Effekt von Acitretin (Ro 10–1670) auf die Spermatogenese und die Hypothalamus — Hypophysen — Gonaden Achse zu untersuchen. In unserer Studie konnte gezeigt werden, daß Acitretin in therapeutischen Dosen die Konzentration, Motilität und Morphologie der Spermatozoen und die Hypothalamus — Hypophysen — Gonaden Achse nicht beeinflußt.
Summary Varicocele is an important cause of male subfertility which may lead to varying degrees of pathological semen quality. Removal of varicocele can be an effective andrological treatment if associated with subfertility. Therefore, in a retrospective study, two possible methods of treatment, high ligation and transcatheter sclerotherapy, were compared with regard to semen parameters and conception rates. Three months after treatment, only high ligation showed a significant improvement of sperm concentration, morphology and motility. Sperm concentration and motility, however, similarly increased in both groups after six to nine months. Sclerotherapy showed a better conception rate of 25% than the surgical group with 14%. Sclerotherapy should be the treatment of first choice if associated with subfertility.
Zusammenfassung Eine Varikozele kann zu unterschiedlich ausgeprägten pathologischen Spermiogrammbefunden führen und somit eine männliche Subfertilität bedingen. Die Therapie der Varikozele stellt daher ein außerordentlich wichtiges andrologisches Behandlungskonzept dar. In einer retrospektiven Studie wurden zwei etablierte Therapieformen — die hohe Ligatur und die retrograde Sklerosierung — im Hinblick auf Spermaparameter und die Schwangerschaftsraten miteinander verglichen. Drei Monate nach Therapie konnte lediglich bei der Gruppe mit hoher Ligatur eine signifikante Verbesserung der Spermatozoenkonzentration, ‐morphologie, und ‐motilität nachgewiesen werden. Nach 6–9 Monaten stiegen jedoch Spermatozoenkonzentration und ‐motilität in beiden Gruppen vergleichbar an. In der Gruppe der Sklerotherapie fand sich eine höhere Schwangerschaftsrate von 25% im Vergleich zur operativen Gruppe mit 14%. Die Sklerotherapie stellt daher die Therapie der Wahl zur Behandlung einer Varikozele dar, wenn sie mit Subfertilität assoziiert ist.
