Стандартная медикаментозная терапия у пациентов с хронической ишемией мозга атеро-склеротического генеза (дисциркуляторная энцефалопатия 1-й степени) изменяет реактивность тромбоцитов в отношении агонистов — аденозиндифосфата (АДФ), адреналина и фактора активации тромбоцитов (ФАТ), что отражает многополярное влияние применяемых лекарственных средств на внутриклеточные сигнальные системы тромбоцитов, ассоциированные с пуриновыми Р2Y12- и P2Y1-рецеп-торами, ФАТ-рецепторами и α2-адренорецепторами. При наличии гиперреактивности тромбоцитов в отношении одного или нескольких агонистов на момент завершения стационарного лечения у пациентов сохраняется риск тромбогенеза, нарушений микроциркуляции мозга и развития аутоиммунного воспаления, то есть тех патологических процессов, которые предопределяют прогрессирование хронической ишемии мозга. Гипореактивность тромбоцитов, выявленная в отношении одного из агонистов, не исключает возможность сохранения или восстановления проагрегантной активности тромбоцитов после завершения лечения, поскольку их функциональный статус определяется взаимодействием нескольких рецепторов, связывающих в том числе АДФ, адреналин и ФАТ.
