La défaillance circulatoire occupe une position centrale dans le sepsis sévère et l'état de choc septique puisqu'elle en conditionne l'existence. Se distinguant de la défaillance micro circulatoire qui fait l'objet d'un axe de recherche fondamentale et clinique prometteur, nous n'envisagerons que la défaillance macro circulatoire. Celle-ci concerne l'ensemble des paramètres hémodyna-miques, tant la volémie vraie ou relative que la fonction myocardique systé-mique ou pulmonaire. Cette défaillance macro circulatoire peut ainsi revêtir tous les aspects hémodynamiques, sans compter l'addition des comorbidités cardio circulatoires de patients de réanimation toujours plus âgés. Monitorer une telle défaillance est dans ce contexte une nécessité. Mais comment choisir, devant le large panel des moniteurs proposés, celui qui répondra le mieux à nos questions ? L'échocardiographie a connu ces dernières années un essor certain, d'abord du fait de l'apparition d'appareils de plus en plus accessibles, mais aussi parce que l'expertise des réanimateurs dans ce domaine a atteint un niveau suffi sant pour que l'utilisation de l'échocardiographie en réanimation devienne routinière. Malheureusement, quand bien même cet outil aurait été complet et toujours accessible, son utilisation comme un monitorage continu de la défaillance circulatoire n'a aucune cohérence. Son atout réside en fait dans sa capacité à faire un bilan hémodynamique rapide de façon non invasive, et ainsi de choisir le moniteur le plus adapté pour la surveillance continue indispensable à la prise en charge hémodynamique d'un état de choc septique.
