Рандомизированное контролируемое мультицентровое исследование, позволившее оценить безопасность раннего закрытия временной илеостомы у больных раком прямой кишки после выполнения основного вмешательства. Материал и методы. У пациентов различных клинических центров без клинических признаков несостоятельности сформированных анастомозов на 8-й день после основного вмешательства с помощью КТ-проктографии или ректоскопии оценивали анатомическую целостность межкишечного соустья. Критерии исключения из исследования: факторы, препятствующие нормальной регенерации тканей (сахарный диабет и прием препаратов стероидного ряда). Пациенты с подтвержденной состоятельностью анастомоза и отсутствием критериев исключения (n=65) распределены на две группы: в 1-й (n=31) закрытие илеостомы произведено в первые 8-13 дней после основного вмешательства, во 2-й (n=34)-после 12 нед. Результаты лечения всех участников включены в анализ. Результаты. Анализ послеоперационных осложнений не выявил количественных различий в группах-6,45% в 1-й против 5,88% во 2-й группе (p=0,08), однако в 1-й группе отмечена меньшая продолжительность реконструктивного вмешательства-в 1-й группе в среднем 50 (27-126) мин, во 2-й-71 (31-134) мин. В среднем время операции в 1-й группе сократилось в 1,42 раза (95% CI 1,30-1,52; p=0,02). Выводы. Раннее закрытие илеостом у пациентов после оперативного лечения колоректального рака технически выполнимо, не вызывает увеличения числа послеоперационных осложнений и может быть рассмотрено в качестве альтернативы тактике отсроченного закрытия стомы. Однако необходимо более глубокое изучение проблемы с целью оценки частоты осложнений, ассоциированных с наличием илеостомы, а также качества жизни таких пациентов.
