Latar belakang. Data global menunjukkan pneumonia pada anak usia di bawah 5 tahun (balita) menyebabkan kematian 15,5%.Neutrophil to lymphocyte count ratio (NLCR) atau rasio jumlah neutrofil-limfosit merupakan parameter inflamasi sistemik dan stresuntuk pasien kritis. Dari beberapa penelitian didapatkan hubungan antara derajat keadaan klinis dengan peningkatan NLCRTujuan. Membuktikan nilai NLCR awal masuk sebagai faktor risiko luaran pasien pneumonia anak yang dirawat di bangsal anakRSUP dr KariadiMetode. Penelitian kohort prospektif pada anak pneumonia usia 1 bulan – 5 tahun yang dilaksanakan di ruang perawatan anak(bangsal anak, HCU, PICU) RS dr Kariadi, Semarang. Subjek yang memenuhi kriteria inklusi dilakukan pemeriksaan darah rutin,kemudian dilakukan pemantauan klinis pada hari perawatan ke 10. Pada spesimen darah diperiksa darah rutin dan dilakukanperhitungan NLCR. Analisis statistik menggunakan uji chi square dan Mann-Whitney UHasil. Didapatkan 40 subjek dengan pneumonia. Rerata NLCR lebih tinggi pada luaran yang buruk dari pada luaran yang baik(3,42± 4,063) vs (1,73 ± 1,568) tetapi tidak berbeda bermakna (p= 0,118). Titik potong NLCR 1,335 meningkatkan risiko luaranperburukan pada pneumonia anak sebesar 1,727 kaliKesimpulan. Tidak terdapat perbedaan nilai NLCR awal antara luaran perbaikan dan perburukan. Background Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis. Of all TB patients, 40-50% are children. C-reactive protein (CRP) is produced during the inflammation process and is an indicator of active TB disease. High sensitivity CRP (hs-CRP) test has higher accuracy and sensitivity to detect CRP at lower levels. Objective To compare hs-CRP in children with TB infection, pulmonary TB, and extra-pulmonary TB. Methods This cross-sectional study of children with tuberculosis was conducted at Dr. Kariadi Hospital and the Semarang Community Health Center, Semarang, Central Java, from January 2020–February 2021. Inclusion criteria were patients aged 1–18 years with suspected TB (contact with adult TB patient or clinically suspected to have TB). Results From 95 study subjects, 19 had TB infection, 51 had pulmonary TB, and 25 had extra-pulmonary TB. There was a significant increase in hs-CRP level based on prolonged fever (P<0.001), enlarged lymph glands (P=0.004), joint swelling (P=0.006), low WHZ or BMI for age (P=0.048), positive bacteriological evidence (P<0.001), and negative/not done tuberculin skin test (P=0.001). There was a significant difference of hs-CRP level based on TB status, with the highest hs-CRP level in extra-pulmonary TB [14.3 mg/l (0.16–321.5)], followed by pulmonary TB [0.8 mg/l (0.3–129.1)], and TB infection [0.7 mg/l (0.3–20.2)]. The highest hs-CRP level for extra-pulmonary TB was found in abdominal TB [84.5 mg/l (0.6–321.5)]. Conclusion Children with extra-pulmonary TB have significantly higher hs-CRP than children with TB infection or pulmonary TB.
Latar belakang. Insiden tuberkulosis (TB) paru terkonfirmasi bakteriologis pada anak mulai meningkat di Indonesia.Tujuan. Menganalisis faktor risiko TB paru terkonfirmasi bakteriologis pada anak. Metode. Penelitian kasus-kontrol menggunakan data catatan medik pasien TB anak di RSUP dr. Kariadi Semarang periode tahun 2014-2015. Kelompok kasus adalah pasien TB anak yang ditemukan kuman dari pemeriksaan sputum dengan BTA dan atau tes cepat molekuler (TCM). Kelompok kontrol adalah pasien TB yang terdiagnosis secara klinis. Analisis menggunakan Chi-square dan multivariat.Hasil. Sebanyak 74 pasien TB anak dilakukan analisis, 37 subyek di masing-masing kelompok. Gambaran Rontgen paru berat dan riwayat kontak TB merupakan faktor risiko (OR 3,08; CI: 1,11-8,50, p=0,027) (OR 2,85; CI:1,08-7,55, p=0,032). Jenis kelamin, usia, status gizi, gejala demam, batuk dan tes tuberkulin, bukan merupakan faktor risiko TB terkonfirmasi bakteriologis. Kesimpulan. Riwayat kontak dan gambaran Rontgen toraks yang berat merupakan faktor risiko TB paru terkonfirmasi bakteriologis pada anak.
