Background: Breast cancer remains the leading cause of death for women globally, including in Indonesia. Breast cancer screening plays a vital role in reducing deaths caused by breast cancer. However, breast cancer screening rate is still low and studies on determinants for breast cancer screening is limited in Indonesia. This study aimed to identify the determinants of breast cancer screening among women in Indonesia. Methods: This population-based study was conducted among 827 women who lived in either rural and urban areas, using a stratified sampling design where were based on province and locality combinations. Data were analysed using a binary logistic regression model to assess the associations between independent and dependent variables. Results: As many as 827 women with an average age of 29.91 (± 11.14) years old participated in this study. The overall breast cancer screening among women was 18.74%. Knowledge of breast cancer risk factors, signs, and symptoms (adj.OR = 1.75, 95%CI: 1.20 – 2.56), age of 35 to 39 years old (adj.OR. = 1.52, 95% CI: 1.02 – 2.26), and household income of ≥6,000,000 IDR (≥457 USD) (adj.OR. = 5.19, 95%CI: 1.43–18.84) were associated with breast cancer screening attendance. In contrast, Christian women had a significantly lower breast cancer screening rate that women from other religions (adj. OR. = 0.45, 95%CI: 0.24 – 0.85). Conclusion: The overall breast cancer screening attendance was poor among Indonesian women population. Age, household income, religion, and knowledge of breast cancer risk factors were identified as the determinant factors for breast cancer screening.
BackgroundComprehensive reports of malaria in Menoreh Hills, Central Java, Indonesia, a unique district cross-boundaries area under three districts and two provinces have been published previously. However, no study was performed to identify the hotspots of malaria in this cross-boundaries area, Kaligesing and Bagelen Subdistricts in Purworejo, Jawa Tengah Province and Kokap Subdistrict in Kulon Progo, Yogyakarta Province, using a longitudinal spatial data.MethodsMonthly reports of malaria cases at primary health centres during 2005–2015 were collected and processed with ArcGIS and SaTScan to identify the malaria distribution at the village level. Malaria distribution was analysed using global spatial autocorrelation (Moran index) in ArcGIS. Cluster analysis was conducted using SaTScan purely spatial clustering and purely temporal clustering. Cluster characteristics resulted from three different approach were compared and analysed.ResultsDuring the last 11 years, 3812 malaria cases were reported and the number of high case incidence (HCI) villages were increased continuously. Malaria spatial distribution in Menoreh Hills was clustered spatially. Using three different approaches of time period ranges, consistent conclusion was found i.e. most likely clusters always occurred in the Purworejo district while the secondary clusters appeared later in the cross-boundaries districts.ConclusionSpatiotemporal analysis of an 11 years surveillance data showed that hotspots of malaria cases in Menoreh Hills were continuously located in Purworejo district. The success of malaria elimination in the cross boundaries area of Menoreh Hills might be depended on the success in malaria case management and surveillance in this hotspot area.
Talasemia adalah penyakit kelainan darah akibat kekurangan atau penurunan produksi hemoglobin. Jumlah penderita talasemia di Yayasan Talasemia Indonesia cabang Banyumas terus meningkat. Tujuan penelitian ini untuk mendeskripsikan karakteristik penderita talasemia di Yayasan Talasemia Indonesia cabang Banyumas tahun 2012. Penelitian kuantitatif dengan desain penelitian cross sectional ini dilakukan terhadap 64 sampel yang diambil dengan teknik proportionate stratified random sampling . Data dikumpulkan meliputi melalui wawancara terstruktur menggunakan kuesioner. Metode analisis yang digunakan adalah analisis deskriptif univariat. Hasil penelitian menunjukkan bahwa penderita talasemia rata-rata berusia 12,28 tahun, berjenis kelamin laki-laki (51,6%), sedang sekolah SD (40,6%), bukan angkatan kerja (92,2%), talasemia β-mayor (90,6%), tidak splenektomi (84,4%), jaminan kesehatan masyarakat (Jamkesmas) (73,4%), kadar feritin ≥ 2000 ng/mL (90,4%), kelasi deferioprone, vitamin C, dan deferioksamin (81,2%), tinggal di Kabupaten Banyumas (79,7%), trans-fusi darah di Rumah Sakit Umum Daerah (RSUD) Banyumas (95,3%), rata-rata terdiagnosis usia 3,7 tahun, usia rata-rata mulai transfusi darah adalah 3,78 tahun. Frekuensi transfusi darah 1 bulan 1 kali (87,5%), dan frekuensi terapi kelasi zat besi mengonsumsi vitamin C 30 kali dalam 1 bulan (95,3%).Kata kunci: Hemoglobin, kelainan darah, talasemia AbstractThalassaemia is blood disorder caused by deficiency or decrease in production/formation of hemoglobin. Number of thalassaemia patients in Yayasan Talasemia Indonesia (Indonesian Thalassemia Foundation) Banyumas branch countinue to increase. The research purpose is to describe the characteristics of thalassemia patients in YTI Banyumas. The research type is a quantitative approach with cross sectional research design. Sample of 64 people taken by proportionate stratified random sampling. Data collected includes characteristics of the respondents based on the variables of people, places, and time was obtained through interviews using a questionnaire. The univariate descriptive analysis were use to describe the characteristics of the respondents. The results showed that thala-ssaemia patients at YTI Banyumas have several characteristics: average age of 12,28 years, male (51,6%), elementary students (40,6%), the laborforce (92,2%), b-thalassemia major (90,6%), no splenectomy (84,4%), Jamkesmas (73,4%), ferritin levels ≥ 2000 ng/mL (90,4%), sailor deferio-prone, vitamin C, and deferioksamin (81,2%), live in Banyumas (79,7%), blood transfusions in hospitals Banyumas (95,3%), the average age of diagnosis of 3,7 years, the average age of starting a blood transfusion was 3,78 years. AbstrakSampai dengan tahun 2013 dilaporkan ada 13 kasus leptospirosis di Kabupaten Banyumas.Terjadinya peningkatan kasus selama dua tahun terakhir perlu mendapat perhatian dari berbagai pihak agar kasus leptospirosis bisa segera ditangani. Tujuan penelitian ini adalah memetakan kasus leptospirosis dan menganalisis faktor risiko lingkungan dan perilaku yang memengaruhi leptospirosis. Penelitian ini menggunakan desain studi observasional kasus kontrol. Pengumpulan data dilakukan dengan wawancara, observasi, dan pengukuran. Analisis data menggunakan analisis spasial, analisis univariat, dan bivariat. Kasus adalah penderita leptospirosis berjumlah 13 orang dan kontrol adalah tetangga kasus yang tidak menderita leptospirosis berjumlah 52 orang. Hasil pemetaan menunjukkan kasus leptospirosis di Kabupaten Banyumas termasuk daerah aliran sungai dengan radius 600 meter ke sungai; seluruh kasus dekat dengan sawah (jarak < 1 km); sebagian besar memiliki vegetasi ≥ 3 jenis dan berada di daerah dengan curah hujan tinggi. Faktor lingkungan yang terbukti berhubungan dengan leptospirosis adalah kondisi jalan yang buruk sekitar rumah (OR = 4,90; CI 95% = 1,35 _ 17,10). Faktor perilaku yang berhubungan dengan leptospirosis adalah kebiasaan mandi/mencuci di sungai (OR = 4,35; 95% CI = 1,21 _ 15,60), riwayat peran serta dalam kegiatan sosial yang beresiko (OR = 12,00; 95% CI = 1,45 _ 99,09) dan penggunaan alat pelindung diri (OR = 7,50; 95% CI = 1,00 _ 62,18). The increasing incidents in last 2 years should get an attention from all institutions. The study conducted was to map incidence of leptospirosis and analyze the environmental and behavior risk factors which associated leptospirosis case. This research was observational by case-control approach. The data was collected through interviews, observation and test. For analyzing the data, the researcher applied spatial analysis, univariate, and bivariate analysis. It was 13 leptospirosis cases and 52 people as the controls; they are the neighbors who are free from leptospirosis.The mapping showed that leptospirosis cases in Banyumas were along the riverin 600 meters radius, all cases with the distance of 0 _ 1 km into the rice field, had vegetation ≥ 3 species and those were in areas with high rainfall intensity. Environmental risk factor associated with leptospirosis was a bad road conditions around the house (OR = 4,90; CI 95% = 1,35 _ 17,10). Behavior risk factors werethe bathing/washing habit in the river (OR = 4,35; 95% CI = 1,21 _ 15,60, a history of participation in social activities (OR = 12,00; 95% CI = 1,45 _ 99,09)and the use of personal protective equipment (OR = 7,50; 95% CI = 1,00 _ 62,18).
Background: Acute otitis media (AOM) is the most common disease in young children requiring antibiotic treatment. Information on AOM-related determinant risk factors in primary school children is still limited, particularly in Indonesia. This study aims to identify risk factors related to AOM in primary school children in Banyumas Regency, Central Java, Indonesia.Design and Methods: This is an analytical study with a case-control design in Banyumas Regency, Indonesia. 3574 children from 6 regions of the Banyumas Regency were recruited for the screening of AOM detection, and confirmation of AOM diagnosis was determined by Otolaryngologist. One hundred and twenty-five cases and 125 control were involved in this study. Data collection was carried out using a structured questionnaire focusing on several variables such as household cooking fuel, house environment, smoking exposure, knowledge of parents, and nutrition status. Univariate, bivariate using chi-square and multivariate with regression logistic was conducted for data analysis. Results: This study highlights the risk of household firewood use (p=0.003), poor nutritional status (p=0.009), and a family history of ear infections (p=0.015) with an increased risk of otitis media.Conclusions: Household firewood use, poor nutritional status and family history of ear infection are factors associated with the occurrence of acute otitis media. It is necessary to provide public health education to prevent exposure to fuel at risk for children and to improve their nutritional status.
