Đặt vấn đề: U xơ tử cung được ghi nhận ở 0,1 - 10,7% phụ nữ mang thai và tỷ lệ này ngày càng tăng lên do phụ nữ có xu hướng trì hoãn việc sinh con đến lứa tuổi muộn hơn. Các hormone liên quan tới thai kỳ ảnh hưởng tới kích thước của u xơ tử cung và u xơ tử cung cũng có nhiều tác động đến thai kỳ. Ca lâm sàng: Sản phụ 30 tuổi, tiền sử phát hiện u xơ tử cung cách 2 năm, có thai lần đầu. Siêu âm ghi nhận một thai trong buồng tử cung, nhân xơ nằm ở thành trước cổ tử cung, thể tích tăng dần tới cuối thai kỳ với kích thước 112 x 99 x 128 mm. Bệnh nhân được mổ lấy thai chủ động kèm bóc u xơ vào tuần thai 38 do thai ngôi ngang và u xơ nằm sâu ở trong tiểu khung cản trở đường ra của thai nhi. Quá trình hậu phẫu của bệnh nhân ổn định, ra viện sau 5 ngày điều trị tăng co bằng oxytocin và kháng sinh dự phòng. Kết luận: U xơ tử cung và thai nghén thường không có triệu chứng lâm sàng. Tuy nhiên một số trường hợp u xơ có thể gây ra các biến chứng trong thai kỳ. Sản phụ cần được quản lý chặt chẽ trong suốt quá trình mang thai.
Mục tiêu: Mô tả đặc điểm lâm sàng và kết quả điều trị các trường hợp chửa ngoài tử cung phối hợp với chửa trong buồng tử cung tại Bệnh viện Phụ sản Trung ương. Đối tượng và phương pháp nghiên cứu: Nghiên cứu hồi cứu mô tả cắt ngang trên 55 trường hợp chửa ngoài tử cung phối hợp với chửa trong buồng tử cung được điều trị tại Bệnh viện Phụ Sản Trung ương từ tháng 01/2017 đến tháng 12/2021. Kết quả nghiên cứu: 87,3% các bệnh nhân mang thai dựa vào các biện pháp hỗ trợ sinh sản, 1/5 số bệnh nhân có khối chửa ngoài tử cung vỡ tại thời điểm phẫu thuật, 83,6% các trường hợp được chẩn đoán tại thời điểm tuổi thai từ 5 - 8 tuần, hầu hết bệnh nhân có khối chửa ngoài nằm tại vòi tử cung, 82% các trường hợp có thai trong buồng tử cung phát triển bình thường sau điều trị. Kết luận: Chửa ngoài tử cung kết hợp với chửa trong buồng tử cung ngày càng trở nên phổ biến hơn do sự phát triển của các biện pháp hỗ trợ sinh sản. Thai trong buồng tử cung phát triển bình thường sau phẫu thuật.
