Zusammenfassung Hintergrund: Die F?rderung k?rperlicher Aktivit?t ist ein wichtiger Bestandteil der station?ren, multidisziplin?ren Behandlung psychosomatischer Patienten. Ein langfristiger Behandlungserfolg l?sst sich nur durch eine ebenso langfristige?Adh?renz bez?glich k?rperlicher Aktivit?t erzielen. Ziel: Diese Studie untersuchte, ob die langfristige Adh?renz bez?glich k?rperlicher Aktivit?t durch Nordic Walking verbessert werden kann und ob?Selbstwirksamkeit und Ver?nderungsbereitschaft Pr?diktoren f?r eine langfristige Adh?renz bez?glich k?rperlicher Aktivit?t sind. Methode: Als Teilprojekt einer randomisierten, kontrollierten Interventionsstudie mit station?ren, psychosomatischen Patienten fand eine retrospektive Datenauswertung statt. Die Kontrollgruppe wurde nach dem Standardprogramm behandelt, die Interventionsgruppe absolvierte zus?tzlich w?hrend mindestens 3 Wochen 3-mal w?chentlich ein begleitetes Nordic-Walking-Training in der Gruppe. Das Aktivit?tsniveaus wurde bei Eintritt und 3 Monate nach Austritt, die Selbstwirksamkeit und Ver?nderungsbereitschaft bei Eintritt und bei Austritt ermittelt. Ergebnisse: Die Studie umfasste 138 Patienten (18???65 Jahre). Das Aktivit?tsniveau wies 3 Monate nach Austritt keinen signifikanten Unterschied zwischen den Gruppen auf. Auch innerhalb der Gruppen unterschied es sich zwischen Eintritt und 3 Monate nach Austritt nicht signifikant. Selbstwirksamkeit und Ver?nderungsbereitschaft waren keine Pr?diktoren f?r eine langfristige Adh?renz bez?glich k?rperlicher Aktivit?t. Schlussfolgerungen: Nordic Walking ist f?r psychosomatische Patienten grunds?tzlich eine geeignete Form k?rperlicher Aktivit?t, f?hrt als Therapieform im Rahmen eines station?ren Aufenthaltes jedoch nicht zu einer verbesserten langfristigen Adh?renz bez?glich k?rperlicher Aktivit?t. Eine ambulante Nachbetreuung zur Unterst?tzung der Adh?renz wird empfohlen.
The effects of exercise therapy in patients with psychosomatic disorders are modest. Therapeutic Nordic walking (tNW) might be a clinically meaningful and cost-effective additional treatment modality. This study aimed to investigate the effects of a tNW program on improving coping ability, compared to the usual care in highly disabled patients with diverse psychosomatic disorders in a hospital setting, and the willingness-to-pay for this treatment modality. Primary outcomes were self-efficacy, readiness to change health behavior, physical performance and attitude to walking. Economic evaluation was conducted with the willingness-to-pay questionnaire. Cohens' d and 95% confidence intervals were calculated for baseline, discharge and three month follow up using intention-to-treat analysis. Differences between groups were tested by t-test or nonparametric ANOVA. A total of 150 patients were included in this study. Results show no significant differences between the experimental and control group for any of the outcomes. Ratings of stages of change within the transtheoretical model were made by therapists or patients, they showed small effects within both groups (precontemplation: 0.2-0.4 points; preparation: 0.1-0.2; action: 0.4-1.1; maintenance: 0.4 for every comparison). There was no additional effect of the tNW program compared to the standard program in the outcome willingness-to-pay for tNW. However, since clinical experience and treatment expectancy of both patients and health care providers with this treatment modality play an important role in the treatment process, tNW might be a low-budget treatment option for some patients.
Zusammenfassung Hintergrund Bei der Anwendung von Kryotherapie gehört die Continuous Flow Cold Therapy (CFCT) zu den moderneren Anwendungsformen. Ziel Diese Literaturübersichtsarbeit untersuchte die Wirkungen von Kältetherapie bei Personen nach einer Kniegelenkprothesenoperation. Methode Die Literatursuche fand in den Datenbanken Cochrane, Embase und PubMed im Zeitraum Januar 2018 bis Juli 2018 statt. Eingeschlossen wurden randomisierte kontrollierte klinische Studien in deutscher oder englischer Sprache. Die Zielparameter umfassten Schwellung, maximale Beweglichkeit, Schmerz und Blutverlust. Die Bewertung der Studien erfolgte mithilfe der PEDro-Skala. Ergebnisse Es wurden 8 prospektive Studien mit 832 Probanden eingeschlossen. Der PEDro-Score lag im Median bei 4,5 von 10 Punkten. Die Messungen waren sehr heterogen. Nur wenige Studien fanden signifikante Unterschiede zwischen der Interventions- und der Kontrollgruppe für die Zielparameter Schwellung, Schmerzen oder Beweglichkeit. Statistisch signifikante Unterschiede waren kaum klinisch relevant. Beim Blutverlust ergaben sich signifikante und klinisch relevante Unterschiede. Schlussfolgerungen Die Ergebnisse zeigten keinen Nutzen der kontinuierlichen Kühlung gegenüber Kühl- oder Eispackungen. CFCT-Geräte riefen keine schädigenden Effekte hervor und können daher weiterhin genutzt werden.
