Introduction Fin décembre 2019, un nouveau coronavirus responsable de nombreuses infections des voies respiratoires inférieures est apparu à Wuhan, en Chine. Ce virus, nommé SARS-CoV-2, s’est depuis propagé dans le monde entier, étant responsable d’une pandémie mondiale. Les rapports initiaux concernant la symptomatologie liée à ce virus mettaient à l’avant plan les plaintes respiratoires. L’objectif de notre travail était de décrire la fréquence et les caractéristiques des manifestations respiratoires chez les professionnels de la santé (PS) atteints de la COVID-19. Méthodes Il s’agissait d’une étude rétrospective descriptive des PS de l’hôpital Charles Nicolle consultants le service de médecine du travail et de pathologie professionnelle durant la période allant du premier septembre au 31 décembre 2020 pour des symptômes évocateurs d’infection à COVID-19. Les patients considérés comme COVID-19 positifs sont ceux ayant eu une RT-PCR COVID-19 positive sur un prélèvement nasopharyngé. Résultats Notre population constituée de 494 PS était à majorité féminine (78 %). L’âge moyen était de 41 ans [21–61]. Des comorbidités chroniques associées étaient retrouvées chez 266 patients (53,8 %). L’hypertension artérielle, l’asthme et le diabète étaient les comorbidités les plus retrouvées. La catégorie d’infirmier représentait 32 % des cas. Ils travaillaient dans les services de chirurgie dans 36 % des cas. Nos patients avaient une ancienneté professionnelle moyenne de 14,9 ans [1–27]. Les plaintes respiratoires liées à l’infection SARS-CoV-2 étaient la toux (67,9 %), la dyspnée (44,4 %) et les douleurs thoraciques (35,6 %). Des signes généraux ont été également rapportées tel que l’asthénie (80,2 %), la fièvre (38,6 %) et l’anorexie (2,6 %). Un traitement symptomatique a été prescrit à tous nos patients à la première consultation. Un suivi téléphonique a été fait pour tous les patients positifs durant la période de confinement allant de 10 à 14 jours. Au total, sept (1,4 %) patients ont présenté une forme sévère de COVID-19 et ont été hospitalisés, dont 3 (0,6 %) sont décédés. Conclusion Les manifestations respiratoires sont fréquentes en cas d’infection au SARS-CoV-2. Elles peuvent être le symptôme initial de la maladie ou rester isolée chez certains patients paucisymptomatiques.
Introduction La médecine du travail est essentiellement une médecine préventive permettant de préserver la santé des travailleurs notamment en milieu de soins et particulièrement en cette période de pandémie COVID-19. Dans ce cadre, une campagne de vaccination a été organisée au sein des services de médecine du travail des hôpitaux, destinée pour le personnel de soins des secteurs public et privé non encore vacciné. Notre étude avait pour objectif de déterminer l’apport de la médecine du travail hospitalière dans cette mesure préventive. Méthodes Étude descriptive transversale ayant intéressé le personnel de soins non vacciné de deux centres hospitalo-universitaires publics (hôpital Charles Nicolle et hôpital Habib Thameur) et trois cliniques privées qui s’est présenté au service de médecine du travail de l’hôpital Charles Nicolle au cours de la campagne. Résultats Au total, nous avons colligé 824 personnels non vaccinés. Notre population ayant reçu le vaccin anti-COVID 19 était composée de 454 personnels (55,1 %). Le personnel avait un âge moyen de 39,66 ± 10,03 ans et était à prédominance féminine (72,7 %) avec un sex-ratio H/F de 0,36. Ils travaillaient dans 72,2 % dans le secteur public et occupaient des services à caractère médical dans 34,9 % des cas. La catégorie professionnelle prédominante était celle des ouvriers (27,4 %) suivie de celle des infirmiers (20,1 %). Conclusion Notre étude a mis en évidence un taux de participation à la vaccination supérieur à la moitié du personnel au sein du service de médecine du travail. Ce taux pourrait refléter le rôle important que joue cette discipline dans l’information et la sensibilisation du personnel à la vaccination anti-COVID 19. Par ailleurs, un renforcement de ces actions est nécessaire afin d’aboutir à une participation quasi-totale du personnel non vacciné.
Introduction L’augmentation de la transmissibilité du virus SARS-CoV-2 à cause de l’émergence de nouveaux variants résistants à la vaccination soulèvent la question de réinfection. L’objectif de cette étude était de décrire les cas de réinfection COVID-19 après vaccination complète. Méthodes Étude descriptive rétrospective, ayant porté sur les personnels de santé (PS) qui se sont présentés à la consultation de médecine du travail pour prise en charge d’une réinfection COVID-19 après une vaccination anti-SARS-CoV-2 complète, durant la période allant du premier septembre 2020 au 31 août 2021. Résultats Nous avons recensé quinze cas de PS correctement vaccinés contre le SARS-CoV-2 ayant présenté deux épisodes d’infection COVID-19. L’âge moyen était de 41 ± 10 ans avec un sex-ratio de 0,25. Aucun antécédent d’immunodépression n’a été rapporté. Treize cas ont reçu une seule dose de vaccin après leur premier épisode d’infection. Deux cas ont reçu les deux doses du vaccin avant les deux épisodes d’infection. Les types de vaccins étaient à ARNm chez 11 cas, à base de virus tué chez trois cas à type d’adénovirus chez un cas. Tous les épisodes d’infection au SARS-CoV-2 ont été confirmés par une RT-PCR sur un prélèvement nasopharyngé positive. Le délai moyen entre les deux épisodes était de 164,53 ± 78,22 jours. Une sérologie du SARS-CoV-2 a été pratiquée chez 8 patients au décours du premier épisode. Elle était négative chez 5 patients. Le délai entre la vaccination et l’apparition des symptômes variait entre 2 à 79 jours (médiane à 35 jours). La notion de contact avec un cas positif a été notée dans onze cas lors des deux épisodes. Les principaux symptômes décrits pendant les deux infections étaient les céphalées ( n = 10), la toux ( n = 9), l’asthénie ( n = 12) et les arthromyalgies ( n = 12). L’anosmie a été prédominante lors du deuxième épisode ( n = 6). Un traitement par corticothérapie a été indiqué chez deux patients pendant le premier épisode et aucun durant le deuxième. La durée moyenne d’arrêt de travail était de 13,4 ± 3,9 jours pendant le premier épisode d’infection et de 10,66 ± 1,29 jours durant le deuxième. Conclusion La confirmation de la réinfection lors de la récurrence des symptômes nécessite une identification par séquençage génomique du variant responsable. L’échappement des nouveaux variants à la combinaison de l’immunité naturelle et post-vaccinale appuie l’idée de la nécessité du respect des mesures barrières par tous en attendant d’atteindre l’immunité collective.
Introduction La pandémie de COVID-19 est une crise sanitaire mondiale. Compte tenu des résultats faux négatifs de la RT-PCR, technique diagnostique de référence, le scanner thoracique joue un rôle pivot dans le diagnostic positif de l’infection COVID-19 et la réparation de la maladie en milieu de soins. L’objectif de notre travail était de décrire les caractéristiques démographiques, professionnelles et cliniques des professionnels de la santé (PS) atteints de la COVID-19 avec une confirmation scannographique. Méthodes Il s’agissait d’une étude descriptive rétrospective des PS de l’hôpital Charles Nicolle consultants le service de Médecine du Travail et de Pathologie Professionnelle durant la période allant du premier septembre au 31 août 2021 pour des symptômes évocateurs d’infection à COVID-19 avec une confirmation scannographique. Résultats Nous avons recensé 1318 infections à SARS-COV2. La confirmation diagnostique était faite par RT-PCR dans 94,76% des cas, par un test rapide antigénique dans 3,19% des cas, par une sérologie anti SARS-COV2 dans 1, 36% des cas et par un scanner thoracique dans 0,53% des cas ( n = 7). Il s’agissait de trois infirmiers, deux médecins et deux ambulanciers. Ils étaient cinq femmes et deux hommes. L’âge variait entre 22 et 59 ans. Ils avaient des antécédents médicaux dans quatre cas. Tous les patients étaient symptomatiques: fièvre ( n = 6) toux ( n = 5), courbatures ( n = 5), asthénie( n = 3), céphalées ( n = 2), dyspnée( n = 2), maux de gorge ( n = 2), vertiges ( n = 1), lombalgies ( n = 1), anosmie ( n = 1) et agueusie ( n = 1). Les RT-PCR pratiquées chez six personnels étaient négatives et un patient était diagnostiqué d’emblée par un scanner thoracique. Le scanner thoracique a trouvé une atteinte minime ( n = 3), modérée ( n = 2), étendue ( n = 1) et sévère ( n = 1). L’extension des lésions au scanner montrait ( n = 2) patients ayant une atteinte ≥ 50% contre ( n = 5) patients ayant une atteinte < 25%. Le délai moyen entre le début des symptômes et la réalisation du scanner était de 3 à 10 jours. La durée de l’arrêt du travail variait entre 12 à 49 jours.
