A treatment protocol is demonstrated, consisting of an osteotomy, either proximal or distal, of the bone defect with subsequent segmental transport via an anteromedially (tibia) or laterally (femur) mounted AO external fixation over an unreamed interlocking nail (monorail system). Twenty patients were treated by this method with indications as follows: 13 had a segmental bone defect of the tibia, 3 of the femur. Three patients showed post-traumatic and postinfectious leg-length discrepancies and one was treated for hypertrophic non-union of the femur. Defect distance varied between 5 and 18.5 cm and average time for transport was 19,42 days/ cm for the tibial shaft, 15,93 days/cm for the femur. Two patients developed deep infection, which required change of treatment, removing the monorail system and application of an Ilizarov apparatus. Despite complications using the monorail system, all patients healed and no amputations were required. The monorail system can be used as an alternative to the Ilizarov method under certain criteria of patient selection; these criteria are shown by an algorithm for segmental bone defects without infection, respecting the soft-tissue status with or without neurovascular compromise.
Eine 12 Jahre alte Shetlandponystute wurde aufgrund einer seit 3 Wochen anhaltenden Lahmheit der rechten Vordergliedmaße vorgestellt. Die Stute zeigte eine mittelgradige Hangbeinlahmheit mit verkürzter Vorführphase der betroffenen Gliedmaße. Aufgrund der klinisch-orthopädischen und der bildgebenden Untersuchung konnten eine geringgradige Osteoarthritis und Dysplasie der rechten Schulter diagnostiziert werden. Ein Jahr später erfolgte die erneute Vorstellung der Stute. Im Stand wurde eine Entlastungshaltung der rechten Vordergliedmaße beobachtet und es lag eine hochgradige Lahmheit vor. Das rechte Schultergelenk war bei Palpation druckdolent und Provokationsproben in diesem Bereich lösten deutliche Abwehrreaktionen aus. Bei der röntgenologischen Untersuchung wurde eine hochgradige Omarthrose diagnostiziert und die Stute aufgrund der infausten Prognose euthanasiert.Omarthrosen werden relativ selten als Erkrankungen des Bewegungsapparats diagnostiziert und scheinen häufig sekundär im Zusammenhang mit primären Erkrankungen bzw. Verletzungen zu stehen. Dazu gehören [7]: ▶ repetitive Gelenkstraumata ▶ intraartikuläre Frakturen ▶ subchondrale Knochenzysten ▶ Osteochondrosis dissecans (OCD) ▶ septische Arthritiden ▶ Luxationen Schultergelenksdysplasien sind in der Humanmedizin [13] durch Gelenkinstabilität und Osteoarthritis charakterisiert. In der Veterinärmedizin sind sie vornehmlich beim Hund beschrieben [3]. In einer radiologischen Studie wiesen Boswell et al. das vereinzelte Auftreten von Schultergelenksdysplasien auch beim Shetlandpony nach [2]. Es wurde demonstriert, dass Shetlandponys im Vergleich zu anderen Ponyrassen eine flachere Cavitas glenoidalis der Scapula aufweisen. Dieser Unterschied sei prädisponierend für die Entwicklung einer Osteoarthritis als Folge einer primä-ren scapulohumeralen Dysplasie [2]. Darin begründet sich wahrscheinlich die erhöhte Inzidenz von Schultergelenkslahmheiten dieser Rasse, verglichen mit der Pferdepopulation [4]. Die Erkrankung ist durch einen charakteristischen Bewegungsablauf der betroffenen Gliedmaße gekennzeichnet. Mehrheitlich wird eine Hangbeinlahmheit mit Verkürzung der späten Vorführphase der erkrankten Gliedmaße bereits im Schritt beobachtet. Die Lahmheit tritt häufig akut und in seltenen Fällen auch bilateral auf [14]. Die im Rahmen der klinisch-orthopädi-schen Untersuchung durchgeführten Provokationsproben hinsichtlich Extension und Flexion der proximalen Gliedmaße lösen deutliche Abwehrreaktionen aus. Begleitend wird eine Atrophie der Schultergelenksmuskulatur beobachtet [4, 12, 14]. Die klinischen und röntgenologischen Befunde scheinen bei Shetlandponys, Miniaturpferden und Falabella von ähnlicher Ausprägung zu sein. Zu den radiologischen Veränderungen zählen die Ausbildung periartikulärer Osteophyten und periartikulärer Enthesiophyten, sowie Anzeichen einer Abflachung der Schultergelenkspfanne. Diese radiologischen Befunde scheinen spezifisch für Schultergelenksarthosen ohne definitive Ätiologie zu sein, jedoch könnte die Dysplasie und die damit verbundene Instab...
