Patients with persistent AF had significantly lower LA voltage compared with patients with paroxysmal AF even after adjustment for differences in indexed LA volume. LA voltage represents an independent covariate of clinical manifestation of AF.

Among clinical variables, shorter duration of persistent AF and higher voltage recorded around the LA predicted long-term maintenance of SR after single ablation.

The purpose of the study presented here was to use the AGREE instrument for the implementation of the methodological analysis of the quality of processing selected clinical practice guidelines focused on problems of pressure ulcers, and furthermore to find differences between particular recommended clinical procedures in the field of prevention of the decubitus development and recommended preventive strategies. The clinical practice guidelines (CPG) were searched for in electronic databases (G-IN , Medline, Embase, ProQuest, Science Direct, Web of Knowledge) based on the following key words: pressure ulcers, pressure sore, decubitus, prevention, clinical practice guidelines. After subsequent classification and analysis of five foreign recommended clinical procedures and two Czech standards, the CPG methodological quality was evaluated and recommended preventive nursing strategies of prevention and taking care of pressure ulcers were compared. In terms of methodology, the clinical practice guideline Risk Assessment & Prevention of Pressure Ulcers of the group Registered Nurses Association Ontario (RNAO) exerted the highest quality of processing. Most CPG evaluated mentioned similar recommendations of nursing interventions in the prevention and taking care of pressure ulcers. Differences were particularly found in recommendations of scales for the determination of the measure of risk of the pressure ulcers development, where the scale Braden, and under Czech conditions the scale Norton, were preferred. Our study demonstrated considerable differences in processing of foreign CPG and Czech local standards of taking care of pressure ulcers, mainly in the compilation and implementation of recommendations-participation of multidisciplinary teams including patients, recommendations supported by research demonstrations, availability of professional as well as lay public.

This study investigated long-term outcome of catheter ablation of right anteroseptal atrio-ventricular (AV) accessory pathways (AP) located close to the His bundle. Methods: Between April 2003 and June 2011, 26 patients (6 females, age 35 ± 13 years) underwent catheter ablation of right anteroseptal AP. These APs represented 10% of all 248 APs ablated within the given reference period. Elimination of AP conduction in both directions and preservation of normal AV conduction were the ablation procedure endpoints. Results: First ablation was effective in 18 (69%) patients. After repeat ablation, AP was permanently eliminated in 22 (85%) patients (one, two, and three ablation procedures in 16, 5, and 1 patient, respectively). Ablation failed in 4 patients (1 procedure in 3 patients, 2 procedures in 1 patient). During 56 ± 27 (4-102) month follow-up period since the last ablation, no late AP conduction recovery was found, and no late advanced AV block occurred. Post-ablation AV node Wenckebach point was present at the pacing rate of 176 ± 26 (130-230) beats per minute. Of the two engaged operators, more experienced operator successfully accomplished first, second, and third ablation procedure in 14/16 (88%), 5/5 (100%), and 1/1 (100%) patients, respectively, the latter operator attained successful ablation in 4/10 (40%) and 1/2 (50%) patients at the first and second ablation procedures, respectively. Conclusion: Ablation of right anteroseptal AP close to the His bundle is feasible and safe. Late advanced AV block was not observed. Individual operator's experience influenced ablation efficacy. SOUHRNCílem práce byla analýza dlouhodobých výsledků katetrové ablace pravostranných anteroseptálních atrioventrikulárních (AV) přídatných drah (PD) v blízkosti Hisova svazku. Metody: Katetrovou ablaci pravostranné anteroseptální PD podstoupilo od dubna 2003 do června 2011 26 pacientů (6 žen), průměrného věku 35 ± 13 let. Tyto PD představovaly 10 % ze všech 248 PD řešených ablací za dané referenční období. Cílem výkonu byla eliminace vedení PD v obou směrech a zachování normálního AV vedení.

Cíl: Cílem práce je charakteristika levosíňových tachykardií (LST) recidivujících po předchozí katetrové nebo chirurgické ablaci fibrilace síní (FS) a výsledky katetrové ablace LST. Metodika: Ablaci LST podstoupilo 40 pacientů (11 žen, 58 ± 10 let) ve 42 výkonech. Po předchozí ablaci paroxyzmální, perzistentní nebo permanentní FS bylo 13, 3 a 22 pacientů a po chirurgické kryoablaci byli dva pacienti. Stabilní LST byly mapovány a cíleny ablací selektivně, nemapovatelné vícečetné střídající se LST byly cíleny substrátovým přístupem. Cílovými momenty výkonu bylo ukončení LST a dosažení nevyvolatelnosti žádné arytmie. Výsledky: Vícečetné tvary LST byly od počátku přítomny v 11 (26 %) výkonů, zatímco stabilní vstupní LST byla zmapována ve 31 (74 %) výkonech. Ve čtyřech případech nebylo možné stanovit z obrazu komplexního reentry jedno kritické místo LST a použila se substrátová ablace. V jiných dvou případech nebyl mapováním stanoven jasný reentry okruh, protože LST vycházela z myokardu koronárního sinu. V dalších dvou případech byl nalezen ektopický zdroj poblíž antra pravé dolní plicní žíly. V ostatních 23 případech byly zmapovány různé formy reentry okruhů. V 7 případech šlo o makroreentry využívající celou levou síň (LS), v 11 případech o lokalizované reentry zahrnující méně než polovinu LS a v 5 případech šlo o mikroreentry z rozhraní antra plicních žil. LST se podařilo ukončit u 39 pacientů ve 41 výkonech, z nichž ve 33 výkonech bylo dosaženo nevyvolatelnosti arytmie a v 8 výkonech nebyla vyvolatelnost testována. Po dobu sledování 16,4 ± 9,5 měsíců je 35 (88 %) pacientů bez recidivy arytmie. Závěr: LST vznikající po předchozí katetrové nebo chirurgické ablaci FS mají převážně reentry mechanismus a jsou odstranitelné selektivní nebo substrátovou mapovací a ablační strategií s vysokou dlouhodobou účinností.
