Wprowadzenie. Udar niedokrwienny mózgu spowodowany jest zatrzymaniem lub zmniejszeniem perfuzji naczynia tętniczego. Udary są główną przyczyną ciężkiego i przewlekłego inwalidztwa, co wpływa na całkowite koszty ponoszone przez społeczeństwo. Leczenie udaru niedokrwiennego polega na zastosowaniu terapii, która prowadzi do poprawienia perfuzji mózgu w obszarze objętym niedokrwieniem, a także terapii zmniejszającej skutki udaru. Wczesna rehabilitacja pacjentów odgrywa istotną rolę w procesie leczenia. Stosuje się różne metody rehabilitacji, uwzględniając stan ogólny chorego, ciężkość udaru i możliwości pacjenta. Cel pracy. Przedstawienie różnych metod rehabilitacji pacjentów po udarach niedokrwiennych mózgu oraz ich wpływ na poprawę funkcjonowania chorych. Skrócony opis stanu wiedzy. Obecnie rehabilitacja medyczna dysponuje wyspecjalizowanym sprzętem, którego stosowanie daje dobre rezultaty w przywracaniu sprawności pacjentom. Celem rehabilitacji jest między innymi poprawa funkcji motorycznych w zakresie kończyn górnych i dolnych, zmniejszenie afazji, poprawa wydolności mięśni oddechowych, zmniejszanie niedowładu mięśni gardła i krtani, ograniczenie lub likwidacja dolegliwości bólowych, a także poprawa samopoczucia chorego. Podsumowanie. Zapewnienie ciągłości rehabilitacji jest ważne w profilaktyce ponownego udaru mózgu, czy też w powstaniu zespołu rzekomego udaru mózgu, kiedy w mózgowiu nie dochodzi do nowych zmian ogniskowych, a mimo to pojawia się nasilenie niedowładów, powstają kolejne powikłania i pogorszenie stanu funkcjonalnego pacjenta.
STRESZCZENIESATYSFAKCJA PERSONELU MEDYCZNEGO Z WYKONYWANEJ PRACY, ZE SZCZEGÓLNYM UWZGLĘDNIENIEM PIELĘGNIAREK Wprowadzenie. Satysfakcja z pracy zarówno lekarzy jak i pielęgniarek, oraz pozostałego personelu medycznego, prowadzi do lepszej jakości opieki oraz poprawia satysfakcję pacjenta. Istnieje wiele czynników determinujących zadowolenie z pracy. Główne powody niskiej lub braku satysfakcji zawodowej to nieadekwatne do pracy wynagrodzenie, środowisko pracy, relacje z przełożonym, godziny pracy oraz niski prestiż zawodu. Istotne jest stałe monitorowanie satysfakcji zawodowej pracowników oraz podejmowanie działań zmierzających do jej podnoszenia. Cel pracy. Celem pracy jest ocena poziomu satysfakcji zawodowej wśród personelu medycznego. Materiał i metoda. Badanie przeprowadzono wśród personelu medycznego -73 osób losowo wybranych i pracujących w szpitalach państwowych w Lublinie, Warszawie i Tarnobrzegu. Zastosowano metodę sondażu diagnostycznego, jako narzędzie badawcze wykorzystano kwestionariusz ankiety własnego autorstwa. Wyniki. Zdecydowana większość -83,5% personelu medycznego lubi swoją pracę. Osoby, które lubią swoją pracę, 84,9% odczuwają jednocześnie satysfakcję z jej wykonywania. Prawie wszyscy ankietowani nie są zadowoleni z otrzymywanego wynagrodzenia za pracę -94,5%. Warunki pracy w miejscu zatrudnienia badanych są z reguły zadowalające. Zdecydowana większość, bo 89% respondentów zadeklarowało, że praca, którą wykonują, wywołuje u nich zmęczenie. Ponad połowa uważa swoją pracę za niebezpieczną. Badani w 56,1% poleciliby wykonywany zawód przyszłym pokoleniom studentów. Wnioski. Personel medyczny deklaruje, że lubi swoją pracę i odczuwa satysfakcję z jej wykonywania. Zdecydowana większość personelu medycznego nie jest zadowolona z otrzymywanego wynagrodzenia. Istnieje zależność między relacjami z przełożonym a tym czy pracownik lubi swoją pracę. The higher the doctors' and nurses' work satisfaction rates are, the better the quality of patient care. There are many factors influencing job satisfaction. Low salaries, working environment, relations with superiors, working hours and low prestige, especially of the nursing job is the main causes of dissatisfaction. Measuring the nurses' professional satisfaction and constant improvements in this field are necessary. Aim. The aim of the work was to assess the level of job satisfaction among the medical staff. Material and methods. The research was conducted on a group of health care employees of different hospitals in Lublin, Warszawa and Tarnobrzeg -73 people selected at random. A diagnostic survey using the authors' own questionnaires was applied.Results. The vast majority (83.5%) of the medical staff likes their job. Choroba Huntingtona to dziedziczona autosomalnie dominująco choroba neurozwyrodnieniowa charakteryzująca się występowaniem postępujących objawów ruchowych, poznawczych i psychiatrycznych. Choroba Huntingtona jest schorzeniem powodującym degradację w sferze fizycznej, psychologicznej oraz socjalnej pacjenta. W zależności od tego, na jakim etapie choroby znajduje się pacjent, przedklinicznym czy klinicznym, występują u niego mniej lub bardziej nasilone problemy zdrowotne. Postępujące objawy ruchowe uniemożliwiają samodzielne funkcjonowanie, natomiast depresja, zaburzenia psychiatryczne i funkcji poznawczych są powodem izolacji oraz unikania kontaktów z otoczeniem. Obecnie nie istnieje skuteczne leczenie tej choroby, a leki stosowane w terapii mają wiele działań niepożądanych. Prowadzi ona do postępującej niesprawności, zależności chorego od osób sprawujących nad nim opiekę oraz w konsekwencji do śmierci. Health problems of patients with Huntington's disease and their impact on daily activity
Introduction: General anaesthesia is carried out using anaesthetic agents that among others depress the circulatory system and CNS. Central and peripheral thermoregulation occurs, which is due to mild hypothermia as well as changes in skin moisture. An important element of therapy in the perioperative period is adequate intravenous fluid therapy, which affects the final effects of treatment. The aim: Evaluation of skin moisture as a result of changes in central and peripheral thermoregulation during general anaesthesia and evaluation of the hydration status of patients. Material and methods: The study included 180 patients undergoing general anaesthesia for elective surgery, aged 20-85 years of age. Before general anaesthesia, patients were evaluated for ASA-related risk of anaesthesia (ASA 1 - healthy patients, ASA 2 and 3 - patients with neurological and circulatory disorders). Patients were divided into 2 groups, the study group (90 people) were patients who were given no intravenous fluids before the surgery and the control group (90 patients) were those receiving doses of 500 ml crystalline intravenously one hour before anaesthesia. The research was carried out at the Operational Block of the Provincial Hospital in Tarnobrzeg, from November 2013 to November 2014. Skin moisture was measured using a CM 825 Corneometer: before general anaesthesia, after induction for anaesthesia, 15 min after surgical incision of the skin and after awakening the patient. Results: Both in the test and control groups, statistically significant differences were found in subsequent skin moisture measurements. Pairwise comparisons indicate statistically significant differences between each pair of measurements. In both groups of patients, there is a clear decrease in skin moisture after induction of anaesthesia compared to the measurement performed before general anaesthesia. The skin moisture values are reduced in subsequent measurements, however the difference is much lower. Conclusions: 1 Measurement of skin moisture can be used as one of the parameters to assess perioperative stress and changes in body temperature during general anaesthesia, which indirectly determines the functioning of the hypothalamus. 2 By measuring the skin moisture it is possible to indirectly assess the hydration status of patients as well as the decrease of the metabolism during general anaesthesia. 3 The decrease in skin moisture during general anaesthesia is the effect of changes in central and peripheral thermoregulation.
Introduction. Carotid endarterectomy (CEA) is a method of treatment of carotid stenosis, which significantly reduces the risk of ischemic stroke. This procedure may affect the patient's neurocognitive functioning. The aim of the study was to evaluate the occurrence of neurocognitive disorders and to determine the functional status of the patients undergoing CEA. Material and methods. The study group consisted of 102 people who underwent CEA. Studies were performed preoperatively and repeated on the fourth postoperative day. The following scales were used: the Mini-Mental State Examination (MMSE), Activities of Daily Living Scale (ADL), Instrumental Activities of Daily Living (IADL) and the Hamilton Depression Rating Scale (HAM-D). Results. The average number of points obtained by patients in the MMSE, both before and after surgery is 26 points to 30 that may be obtained, which shows a subtle cognitive impairment. Almost every patient showed fitness both in terms of basic (ADL), as well as complex (IADL) activities before and after surgery. Conclusions. Low or average level of cognitive performance is observed in most subjects, both before and after CEA. There are many different factors that can affect the cognitive functions. The functional status of patients stood without significant changes.
Introduction. Alzheimer’s disease is characterized by progressive dementia process. There’s no medicine available to hinder that process. In case, the ill individual is unable to diagnose early symptoms of the disease, they call a doctor in the moment when a neurotic atrophy is irreversible. The patient is gradually losing self-reliance in everyday activities, the ability to take care of themselves, loses touch with the reality and surrounding world, begins to experience distressing symptoms and becomes isolated from the society. All these factors affect the life quality of an individual suffering from Alzheimer’s disease.Aim. The present article provides an analysis of literature on the quality of life in people suffering from Alzheimer’s disease.
