Presentación de la menarca en madres e hijas. ¿Existe un adelanto?Resumen INTRODUCCIÓN: la menarca es un acontecimiento tardío en el desarrollo puberal y constituye el hito más significativo. Se ha descrito un adelanto progresivo en la edad de presentación de la menarca.OBJETIVO: determinar la edad de aparición de la menarca y su relación con la edad de la menarca materna. MATERIAL Y MÉTODOS: estudio retrospectivo, analítico, realizado entre mayo del 2014 y septiembre del 2015. Se estudiaron a niñas que presentaron su menarca con edad ginecológica menor a tres años. Se registró la edad de la menarca, seguro de salud y lugar de residencia de la niña y edad de la menarca de su madre, por el método de recuperación de datos. Se calcularon el índice de masa corporal (IMC) y el IMC/edad-Z score de la niña.RESULTADOS: se incluyeron 250 niñas que presentaron menarca a una edad promedio de 11.6 ± 1.1 años; con un adelanto significativo de un año respecto a la de su madre: 12.6 ± 1.6 años (p < 0.00001). Se observó una correlación negativa estadísticamente significativa entre la edad de la menarca de las niñas y su IMC/edad-Z score (p < 0.00001); se mantuvo independiente del lugar de residencia (p = 0.3903) y del seguro de salud (p = 0.6067).CONCLUSIÓN: en nuestra población, la menarca ocurrió un año antes en promedio en las hijas que en sus madres. Se observó una tendencia a la disminución de la menarca promedio en niñas con aumento del índice de masa corporal. No encontramos asociación entre la edad de la menarca y el nivel socioeconómico o lugar de residencia.
Prevalencia de vulvovaginitis y su relación con hallazgos físicos en niñas evaluadas por sospecha de abuso sexual infantilPrevalence of vulvovaginitis and relation to physical findings in girls assessed for suspected child sexual abuse
RESUMENIntroducción. La vulvovaginitis representa el 25% de las consultas en ginecología pediátrica. Objetivo. Evaluar las etiologías de las vulvovaginitis en función de la edad y el estadio de Tanner mamario. Material y métodos. Estudio descriptivo y transversal realizado entre el 1 de enero y el 31 de diciembre de 2011. Se analizaron pacientes con vulvovaginitis en función de dos variables: la edad (GI: 0 a 8,9 años; GII: 9 a 15,9 años y GIII: 16 a 18 años) y el estadio de Tanner mamario (I; II-III; IV-V). Resultados. Se incluyeron 229 pacientes; 78 niñas en GI, 134 en GII y 17 en GIII; en relación con el estadio de Tanner mamario, se agruparon 81 niñas en TI, 36 en TII-III y 112 TIV-V. Shigella y Oxiurus se presentaron con mayor frecuencia a temprana edad. Candida albicans, otras especies de Candida, Gardnerella y Ureaplasma urealyticum se observaron en niñas mayores. Oxiurus predominó en la etapa prepuberal y Candida albicans, en la pospuberal. Conclusiones. En relación con la etiología de las vulvovaginitis, la influencia hormonal es más relevante que la edad cronológica de la paciente.
INTRODUCCIÓNSe define como vulvovaginitis al cuadro inflamatorio que involucra las mucosas vulvar y vaginal.1 Representa el 25% de las consultas en ginecología pediátrica. 2 El bajo estímulo estrogénico del período prepuberal condiciona el adelgazamiento del epitelio vulvovaginal, pH alcalino local y labios menores finos, factores que favorecen la presencia de vulvovaginitis. En nuestra Unidad de Ginecología InfantoJuvenil, corresponde al 23% de las consultas. En las prepúberes, la causa más frecuente es la vulvovaginitis inespecífica (VVI) por flora endógena que, ante la falta de integridad de las mucosas y alteraciones fisicoquímicas, se torna agresiva. Las vulvovaginitis específicas (VVE) se asocian a un agente etiológico particular, agrupados según su origen respiratorio, entérico y de transmisión sexual.3 Se ha publicado escasa bibliografía que relacione las distintas etiologías con el desarrollo puberal. 4 El objetivo de este trabajo es describir la etiología de las vulvovaginitis en función de la edad cronológica y del estímulo estrogénico en niñas asistidas por consulta espontánea en nuestra unidad.
MATERIAL Y MÉTODOSe incluyeron las pacientes que consultaron a la Unidad de Ginecología Infanto-Juvenil del Hospital Sor María Ludovica de La Plata por presentar vulvovaginitis entre el 1 de enero y el 31 de diciembre de 2011. Se realizó la anamnesis y el examen físico y ginecológico en presencia de un adulto responsable. El estímulo estrogénico se evaluó mediante el grado de desarrollo mamario por el método de Marshall y Tanner.5 Se excluyeron las pacientes con patologías crónicas provenientes de otros servicios y aquellas con sospecha de abuso sexual. Se tomó una muestra de la secreción intravaginal con hisopo a quienes presentaron flujo genital en el momento de la evaluación.
