Introdução: A sarcoidose é uma doença heterogênea de etiologia desconhecida cujas lesões características são granulomas não caseosos. O acometimento cardíaco pela sarcoidose ocorre em apenas uma pequena parcela dos pacientes que possuem sarcoidose sistêmica. A apresentação clínica da sarcoidose cardíaca (SC) varia desde uma condição descoberta incidentalmente até insuficiência cardíaca (IC), bradiarritmias, taquiarritmias e morte súbita. Apesar da escassez de dados, a terapia de imunossupressão tem sido preconizada para o tratamento da SC clinicamente manifesta. A terapia com dispositivos, principalmente com cardiodesfibriladores implantáveis, é frequentemente recomendada para pacientes com doença clinicamente manifesta. 2,7,8 Objetivos: O objetivo desse estudo é revisar sobre a sarcoidose cardíaca, compreendendo epidemiologia, fisiopatologia, manifestações clinicas, diagnóstico e tratamento. Métodos: Os bancos de dados Pubmed, Diretrizes e UpToDate foram pesquisados eletronicamente utilizando os descritores sarcoidose; sarcoidose cardíaca nos idiomas inglês e português. Discussão e Conclusão: trata-se de uma doença de manejo desafiador, os principais objetivos da terapêutica incluem a prevenção da progressão da doença e a consequente piora da disfunção ventricular esquerda, bloqueios atrioventriculares, arritmias e risco de morte súbita. Parte do tratamento preconizado tem alta probabilidade de efeitos adversos, os benefícios do tratamento são limitados e, ainda, muitos pacientes possuem o diagnóstico provável, e não definitivo, sendo que os médicos geralmente precisam decidir se devem ou não iniciar a terapia diante de um diagnóstico incerto.
