InleidingDe NHG-Standaard Obstipatie geeft richtlijnen voor diagnostiek en behandeling van obstipatie bij kinderen en volwassenen in de huisartsenpraktijk, en vervangt de Farmacotherapeutische richtlijnen Obstipatie bij kinderen en Obstipatie bij volwassenen. De klacht obstipatie, oftewel 'moeite met de stoelgang', komt bij kinderen en volwassenen veel voor. Tot op heden ontbrak een eenduidige definitie en behandelstrategie van obstipatie, wat mogelijk geleid heeft tot over-of onderdiagnostiek en -behandeling. Deze NHG-Standaard geeft nu een definitie voor obstipatie die voor kinderen en volwassenen hanteerbaar is in de praktijk, waardoor eenduidigheid kan ontstaan tussen verschillende behandelaars over deze 'symptoomdiagnose'. Er zijn diverse overeenkomsten in de benadering van obstipatie bij kinderen en volwassenen: anamnese en lichamelijk onderzoek zijn vaak voldoende voor de diagnose, zowel voor kinderen als voor volwassenen is het belangrijk dat zij het aandranggevoel herkennen en toegeven aan de defecatiereflex, en de medicamenteuze behandeling is, op de dosering na, nagenoeg hetzelfde. Een belangrijk onderscheid in de aanpak is dat het bij volwassenen vooral belangrijk is onderliggende (ernstige) oorzaken van de klachten aan te tonen of uit te sluiten. Bij kinderen is ophoudgedrag een van de belangrijkste oorzaken van obstipatie. In het beleid bij kinderen dient veel aandacht te zijn voor goede voorlichting en begeleiding aan kinderen en ouders. De passages in deze NHG-Standaard die over kinderen gaan, sluiten aan bij de multidisciplinaire richtlijn Obstipatie bij kinderen van 0 tot 18 jaar van het Kwaliteitsinstituut voor de Gezondheidszorg CBO. Met name bij kinderen kan fecale incontinentie een van de kenmerken van obstipatie zijn, maar deze NHG-Standaard zal niet ingaan op solitaire fecale incontinentie. Dit is een andere aandoening, waarbij sprake is van onvrijwillig verlies van ontlasting zonder obstipatie. Voor obstipatie in de palliatieve fase wordt verwezen naar bestaande (palliatieve) richtlijnen, zoals de Farmacotherapeutische richtlijn Pijnbestrijding (http://www.nhg. org, zie ook http://www.oncoline.nl). Voor diagnostiek en beleid bij obstipatie in het kader van het prikkelbaredarmsyndroom verwijst deze standaard naar de NHG-Standaard Prikkelbaredarmsyndroom. De aanpak van obstipatie bij kinderen en volwassenen komt grotendeels overeen, en de standaard behandelt deze gezamenlijk. De extra aandachtspunten die relevant zijn bij kinderen bespreekt de standaard apart.
