A sepse é um processo dinâmico, que rapidamente evolui para estágios graves. Com isso, tecnologias de diagnóstico rápido, como o Matrix Associated Laser Desorption-Ionization -Time of Flight (MALDI-TOF) têm o potencial de melhorar a condução do tratamento, trazendo melhora no prognóstico do paciente. Objetivo. Esse trabalho teve como objetivo verificar a utilidade do MALDI-TOF como ferramenta diagnóstica nos protocolos de sepse em comparação com a cultura. Método. Foi realizada uma revisão integrativa nas bases de dados: SciELO, BVS, Web of Science, PUBMED e Google Acadêmico. Resultados. Observou-se na literatura encontrada que a implementação do MALDI-TOF no protocolo de sepse reduz o tempo para diagnóstico etiológico, o tempo até a terapia efetiva, o tempo de permanência em UTI e diminui os custos hospitalares. Além disso, ele é utilizado para diagnósticos de microorganismos raros. Conclusão. A inclusão do MALDI-TOF tem potencial para trazer benefícios na condução do paciente com sepse.
Prezados congressistas e demais leitores, estes são os Anais do II Congresso Nacional de Educação Contra o Tabagismo - CONECT, celebrado entre os dias 21 e 23 de outubro de 2022 de forma virtual. É com grande satisfação que a Comissão Científica do II CONECT, em parceria com a renomada Editora Pasteur, organizaram os Anais contendo os resumos aprovados no congresso, com o escopo de alertar os ouvintes e leitores quanto aos inúmeros riscos do tabagismo, que é um importante fator de risco para a perda da qualidade de vida e também para o desenvolvimento de várias doenças, sendo um hábito evitável que traz inúmeros impactos negativos tanto a nível individual, quanto social. Foram aprovados e publicados 18 trabalhos, os quais abrangeram diferentes áreas temáticas relacionadas às repercussões negativas do hábito de fumar, demonstrando, dessa maneira, a magnitude dos prejuízos biopsicossociais atreladas a esse vício. À guisa de democratizar o debate e embasar cientificamente tal discussão, os Anais, bem como toda a programação científica do II CONECT, foram cuidadosamente organizados, com o intuito de unificar o engajamento dos profissionais de saúde contra o perene hábito de fumar, que - após a introdução dos dispositivos eletrônicos, progressivamente, tem se tornado mais comum, principalmente entre a população mais jovem. Estes Anais refletem o comprometimento dos estudantes, dos docentes e dos pesquisadores com a prática de prevenção em saúde e com a promoção da qualidade de vida. O CONECT, ainda em sua tenra formação, sustenta o valor do cuidado preventivo, acreditando que a principal fonte de disseminação dos bons hábitos, são os profissionais de saúde.
RESUMOO presente artigo tem como objetivo relatar o caso de um paciente submetido à laringotraqueoplastia a Pearson devido a uma estenose cricotraqueal secundária à intubação orotraqueal prolongada por COVID-19.
A sepse é um quadro dinâmico que rapidamente pode evoluir para estágios graves e cuja a intervenção precoce tem um importante impacto no prognóstico do paciente. Tendo em vista que um tratamento eficaz depende da rápida identificação do patógeno causador do quadro, métodos diagnósticos rápidos ganham importante papel dentro do protocolo. Dentre as várias formas de identificar o agente causador da sepse, o Matrix Associated Laser Desorption-Ionization – Time of Flight (MALDI-TOF) tem grande potencial por sua velocidade nos resultados e confiabilidade. O MALDI-TOF é um método diagnóstico que consiste em colocar o material biológico em placa de matriz polimérica, sendo o material vaporizado por um lazer e coletado por um tubo que possui um detector no final, que avalia o tempo de chegada das moléculas evaporadas da substância. Esses dados são, então, colocados em um gráfico, com picos para cada fungo e bactéria, e então interpretados com um banco de dados já conhecido, concedendo o resultado diagnóstico . Esse trabalho teve como objetivo verificar a utilidade do Maldi-TOF como ferramenta diagnóstica nos protocolos de sepse em comparação com a cultura. Como objetivos específicos, esse trabalho se propôs a verificar se houve uma redução no tempo de diagnóstico microbiológico, internamento, tratamento e uma melhora na evolução do paciente, assim como avaliar se houve concordância entre os resultados apresentados pelo MALDI-TOF e o método convencional. Como metodologia, foi realizado uma revisão integrativa nas bases de dados: SciELO, BVS Salus, Scopus, Web of Science, PUBMED e google academico. A seleção de artigos foi feita por dois pesquisadores, individualmente, resultando em 16 artigos finais, dos quais foram extraídos seus dados e realizada a discussão. Foi obtido como resultados que o uso do Maldi-TOF no protocolo de sepse reduz o tempo até identificação do patógeno permitindo, consequentemente, uma redução no tempo até terapia efetiva, tempo até terapia optima, tempo de estadia hospitalar e na mortalidade. Observou-se que a redução no tempo de estadia, assim como, na melhora no tratamento, tem importante impacto econômico no hospital, causando redução de diversos custos quando comparado ao método convencional. No entanto, o MALDI-TOF tem importantes limitações como a dificuldade na identificação de amostras polimicrobianas, sua dependência dos agentes registrados nos bancos de dados, sua ausência de um protocolo único estabelecido para processamento de amostras diretamente de hemocultura positiva e sua limitação na identificação de bactérias gram positivas. Conclui-se que a inclusão do MALDI-TOF no protocolo de sepse traz importantes benefícios tanto em relação ao tempo ate identificação do agente, quanto em relação à melhora no prognóstico do paciente. No entanto, ainda são necessárias algumas melhorias de forma a contornar as limitações atualmente presentes.
