La hiperplasia suprarrenal congénita (HSC) es la alteración adrenal hereditaria más común en la infancia. Objetivo: describir las características clínicas y de laboratorio de los pacientes pediátricos con HSC y realizar un análisis exploratorio comparando variables clínicas y paraclínicas según los tipos de HSC. Pacientes y Método: Estudio observacional descriptivo longitudinal. Se revisaron historias clínicas de la consulta de endocrinología pediátrica de cuatro instituciones en Medellín Colombia. Se analizaron variables sociodemográficas, clínicas (tipo de HSC, crisis salina, endocrinopatías asociadas), paraclínicas (17-hidroxiprogesterna, testosterona, dehidroepiandrosterona sulfato, androstenediona, cortisol, hormona adrenocorticótropa). Se realizó un análisis estadístico descriptivo. Resultados: Se incluyeron 132 pacientes (65% femenino). La mediana de edad al diagnóstico fue 2 meses, 44,7% diagnóstico neonatal. Se documentaron 79 niños con HSC clásica perdedora de sal, 31 virilizante simple y 22 no clásica. La mediana de 17-OHP al diagnóstico fue 4.820 ng/dl. El 47% presentó desarrollo sexual diferente y el 48% crisis salina (CS) al diagnóstico. La mediana de edad de la primera CS fue 15 días. Noventa y tres pacientes requirieron fludrocortisona y 32 presentaron CS posterior al diagnóstico y tratamiento. Más del 60% presentó androstenediona y/o testosterona elevada en la última cita. Conclusiones: Las características sociodemográficas y clínicas son similares a las reportadas en la literatura. El 48% de pacientes presentaron inicialmente CS. La tamización neonatal es importante ya que permitiría un diagnóstico temprano. Encontramos virilización en el 71% de mujeres de nuestra serie. Se debe sospechar una HSC en un recién nacido con genitales diferentes.
La tecnología en diabetes hace referencia a aquellos softwares o hardware que están diseñados para facilitar y mejorar la vida del paciente con diabetes. Se realizó una búsqueda no sistemática de literatura para artículos en inglés y español con descripción del uso de monitoreo continuo de glucosa (MCG) en pacientes pediátricos con diabetes Mellitus Tipo 1. Se hace un recuento sobre el funcionamiento del MCG, su exactitud y clasificación, y se realiza una síntesis cronológica de la evidencia hasta junio 2020, donde se especifican aspectos del control metabólico con el uso de MCG en tiempo real e intermitente/flash en estudios con población adulta y pediátrica, tales como: reducción de los niveles de HbA1c, diminución de frecuencia y gravedad de las hipoglicemias, disminución de episodios de cetoacidosis y bienestar, y variables como la Frecuencia del uso de MCG, que se han relacionado con la mejoría de los objetivos del control en diabetes. Esta revisión presenta un resumen cronológico de la evidencia del monitoreo flash de glucosa en estudios donde sólo se incluye población pediátrica y aporta un recuento de las recomendaciones de tecnología en diabetes que aplican para población pediátrica de la guía de la American Diabetes Association del 2020, las recomendaciones del International Society for Pediatric and Adolescent Diabetes 2018.
