ResumenObjetivo: Describir un caso de cistitis enfisematosa y presentar revisión de la literatura como una patología infecciosa caracterizada por la presencia de aire en la luz o pared intravesical. Existen muy pocos casos reportados en la literatura, aunque es posible que su diagnóstico esté subvalorado. Material y métodos: Reportamos el caso de una mujer de 61 años con antecedente de diabetes mellitus de difícil control y episodio previo de pielonefritis enfisematosa con aislamiento de un germen multirresistente, manejado con meropenem. En la hospitalización se reportó el hallazgo por medio de la realización de tomografía axial de abdomen y pelvis de cistitis enfisematosa sin pielonefritis asociada, iniciándose manejo empírico con el mismo antibiótico y colocación de sonda uretral, con posterior mejoría clínica y resolución del proceso infeccioso. Conclusión: La cistitis enfisematosa es una patología infecciosa subvalorada, pero requiere manejo inmediato con una terapia dirigida y específica para lograr un resultado favorable.
Objetivos Discutir y evaluar la tasa libre de cálculos y las complicaciones de la ureterolitotomía endoscópica flexible con láser holmium en el manejo de cálculos ureterales y renales basada en la experiencia de un hospital público de iii nivel en Bogotá.Material and Métodos Estudio retrospectivo en una muestra poblacional en pacientes con litiasis renal y ureteral, tratados con ureteroscopio flexible y láser holmium, evaluándose la tasa libre de cálculos, complicaciones postoperatorias y otros parámetros. El seguimiento se realizó con radiografía de abdomen y urotac al mes del postoperatorio, la tasa libre de cálculos se definió como ausencia de cálculos residuales < 4 mm en la imagen control.Resultados Se realizaron 44 procedimientos en 43 pacientes con edad promedio de 45 años, tiempo quirúrgico promedio 91,2 min, la tasa libre de cálculos fue del 84% (37/44) en un solo evento quirúrgico; solo un paciente requirió una segunda intervención con posterior tasa libre de cálculos del 100%. La tasa libre de cálculos ureterales fue del 92% (24/26), cáliz superior 100% (2/2), cáliz medio 100%(1/1), cáliz inferior 78% (7/9) y calicial múltiple 50% (3/6). El porcentaje de complicaciones fue del 6,8%.Conclusiones La ureterolitotomía endoscópica flexible con láser es un procedimiento efectivo y seguro en el manejo de litiasis renal y ureteral con baja incidencia de complicaciones. Observamos que en la mayoría de los pacientes con fragmentos residuales significativos estos se encontraban en cáliz inferior o se trataba de cálculos en múltiples cálices, explicándose por el difícil acceso al cáliz inferior y la carga litiásica.
Objetivo Realizar el reporte de un caso de linfoma testicular no Hodgkin tipo Burkitt, presentando una revisión de la literatura y documentando las variaciones histológicas típicas, los métodos diagnósticos, el impacto del manejo inicial y el abordaje de las metástasis e infiltraciones tumorales en el pronóstico de esa patología oncológica.
Metodología Se describe el caso de un hombre de 36 años en quien se diagnosticó un linfoma testicular tipo Burkitt, durante el estudio de un síndrome proliferativo, que inició con signos y síntomas neurológicos conllevando a la extensión del estudio y demostrando múltiples lesiones metastásicas asociadas a la infiltración del sistema nervioso central. El tratamiento incluye múltiples modalidades entre las que se encuentra la quimioterapia, radioterapia local, profilaxis intratecal entre otras; sin embargo, el pilar del manejo del compromiso testicular sigue siendo el quirúrgico.
Conclusión El linfoma testicular no hodking es una patología rara, siendo aún más infrecuente el subtipo Burkitt, a pesar del pobre pronóstico, el diagnóstico oportuno y el desarrollo de nuevas terapias aplicadas en combinación, han permitido mejorar la sobrevida y disminuir el riesgo de recaída y progresión.
