Resumo O estudo teve por objetivo compreender crenças e emoções acerca do usuário com diagnóstico de esquizofrenia vigentes no imaginário coletivo de enfermeiros inseridos na Atenção Primária à Saúde. Trata-se de estudo de abordagem qualitativa pautado pelo método investigativo psicanalítico. Os participantes foram 15 enfermeiros que atuavam no referido nível de atenção em saúde de uma cidade de médio porte. A coleta de dados foi realizada com o emprego do Procedimento de Desenho-Estória com Tema (PDE-T) e a análise de dados buscou captar campos de sentido mediante a execução dos movimentos técnicos preconizados para a operacionalização do método investigativo psicanalítico. Verificou-se que, de modo não-consciente, o usuário com diagnóstico de esquizofrenia é representado negativamente no imaginário coletivo dos participantes, pois é visto como alguém inconveniente, que cria alvoroço e desperta medo, sobretudo devido à sua “agitação” supostamente típica. Assim, deparar-se com tal usuário corresponde, para os participantes, a uma situação a ser “resolvida” com rapidez. Este estudo, portanto, demarca dificuldades no tocante à atenção em saúde mental oferecida ao usuário com diagnóstico de esquizofrenia, as quais estão relacionadas a elementos não-conscientes da subjetividade grupal que, ao mesmo tempo, possuem caráter social e individual.
Introduzione: nell’anno 2015 in Regione Lombardia ci sono state 111.784 giornate di degenza complessive nelle Unità Operative di Terapia Intensiva ma la mobilizzazione dell’assistito con tubo tracheale non viene attuata per la maggioranza dei casi. L’immobilità è ampiamente documentata in letteratura come causa di aumento di mortalità e di complicanze, quali le lesioni cutanee, effetti avversi legatiai farmaci, infezioni nosocomiali e cadute.Obiettivo: indagare e analizzare: tipologia di assistiti candidati alla mobilizzazione precoce; tipologie di mobilizzazione efficaci; barriere e limitazioni alla mobilizzazione; costi e sostenibilità ; atteggiamenti e percezioni degli infermieri.Materiali e metodi: è stato strutturato il PICO. Sono state interrogate le banche dati: Scopus, Cinahl, Pubmed, Cochrane, Embase. Search terms (free terms, MeSH): early, mobilization, ambulation, intensive care unit, adult. Criteri di inclusione: età pazienti >18 anni, anni pubblicazione dal 2012 al 2016. Criteri di esclusione: presenza di patologia specifica, assenza di ventilazione meccanica. Valutazione della qualità degli articoli: lista di controllo di Dixon-Woods.Analisi e discussione: le tipologie di mobilizzazione, passiva, attiva o una combinazione di entrambe sono associate a: diminuzione dei giorni di degenza, durata più breve di delirium, minor numero di giorni di dipendenza dalla ventilazione meccanica invasiva, maggiore distanza percorsa nella deambulazione e miglioramento dello stato funzionale al momento della dimissione ospedaliera.Conclusioni: i benefici ricavati dagli interventi di mobilizzazione sono molteplici sia per il recupero funzionale nella fase acuta sia per la qualità della vita dell’assistito successiva alla degenza. La figura dell’infermiere è fondamentale per una mobilizzazione in sicurezza per la persona.
The nurse's work can be considered essential for the consolidation of the principles of the Brazilian Psychiatric Reform based on the articulation between mental health and Primary Health Care. There are challenges to be overcome though for its potential will be maximized in this regard. One challenge is the deconstruction of ancient prejudices about "madness" that are still present in various segments of society, including the professional category in question. Paradoxically, it is also up to the nurse to fight these prejudices. This study assumes that the concept of collective imaginary, in its psychoanalytic sense, is capable of helping to circumscribe prejudices in general, especially through the identification of nonconscious beliefs, images and emotions. Thus, the objective was to understand the collective imaginary about the patient diagnosed with schizophrenia by a group of nurses who work in Primary Health Care. This research can be classified as qualitative using the psychoanalytic method. The participants were 15 nurses who work in Primary Health Care in a city in the interior of the State of Minas Gerais. The instrument used was the Thematic Drawing-and-Story Telling Procedure. The data collection took place collectively. The participants were asked to draw a patient with a diagnosis of schizophrenia being attended by a nurse in Primary Health Care and to write a story about their drawing, giving a title. The collected data were submitted to psychoanalytic interpretation, aiming to capture the fields of meaning that support the participants' collective imaginary. Three fields of meaning were delimited. The first field, titled "I assist, I do not deny care, I forward when I can", reveals that, in most participants' collective imaginary, the belief that the diagnosis of schizophrenia in Primary Health Care seems to be the sole responsibility of the professionals and/or health services purportedly specialized in mental health has a central role. The second field of meaning, called The "super-healthcare team", was organized based on the belief that the health teamin which most participants did not include themselves in their storiesrepresents a kind of powerful entity capable of undertaking the appropriate management of patients diagnosed with schizophrenia. "The problem patient", the third and final field of meaning, organized around the negative image on the basis of which the patient diagnosed with schizophrenia is preponderantly represented in the participants' collective imaginary. That patient is seen as someone who creates problems due to agitation, aggressiveness and unpredictability. Therefore, this study supports, specifically with respect to the participants, the outlines of the conceptual-emotional substrate of the behaviors concerning patients diagnosed with schizophrenia and the mental health care they are offered in Primary Health Care.
A crise psiquiátrica constitui um importante desafio ao cuidado psicossocial, mas tem sido tematizada em poucas pesquisas empíricas nacionais. O presente estudo teve como objetivo compreender o imaginário coletivo de um grupo de profissionais de um CAPS acerca do paciente com transtorno mental em crise psiquiátrica. Trata-se de um estudo qualitativo, do qual participaram 10 profissionais lotados em um CAPS. A coleta de dados foi realizada individualmente mediante o emprego do Procedimento de Desenho-Estória com Tema. O corpus foi constituído pela transcrição das verbalizações dos participantes e, secundariamente, por seus desenhos. A análise de dados visou à formulação de interpretações voltadas à demarcação de campos de sentido.
