40 ResumoUm adolescente de l7 anos deu entrada no hospital com queixas de cefaléia, irritabilidade, sudorese e hipertensão arterial(HA) severa. O quadro clínico e testes bioquímicos iniciais sugeriram feocromocitoma, sendo tratado com nifedipina, clinidina e propranolol. Com informações adicionais de exposição a vapor de mercúrio, dosagens urinária e sangüínea de mercúrio confirmaram a intoxicação. O paciente recebeu terapia de quelação com dimercaprol(BAL) e penicilamina com remissão dos sintomas e normalização da HA em 2 meses.O caso tem relevância para a prática médica devido à semelhança entre intoxicação por mercúrio e HA secundária ao feocromocitoma.J. pediatr. AbstractA 17-year-old boy was admitted to hospital because of severe hypertension (200/130 mmHg), headache, irritability, sweating, etc. Initial biochemical tests suggested pheochromocytoma, being treated with nifedipina, clonidina and propranolol.On reporting exposure to mercury vapour, he underwent twenty-four-hour urine screening and measurement of blood mercury which confirmed intoxication. The patient received courses of chelation therapy with dimercaprol (BAL) and penicillamine with remission of symptoms and normalisation of hypertension after 2 months.This case is relevant to current practice regarding similarity between mercury intoxication and hypertension secondary to pheochromocytoma. Ele se apresenta na natureza em 3 formas químicas: mercúrio elementar (vapor de mercúrio), sais orgânicos e sais inorgânicos de mercúrio. Os sais inorgâ-nicos mercurosos e mercúricos são usados como desinfetantes, antissépticos, pigmentos, em baterias e explosivos. Os sais orgânicos são usados nos diuréticos, inseticidas, fungicidas, em papéis e plásticos 1,2 .O mercúrio elementar (vapor de mercúrio) é utilizado em indústrias de aparelhos eletrônicos e na extração mineral de ouro. É esta forma de mercúrio a responsável por grande número de intoxicações agudas e crônicas com graves sequelas e evoluções, às vezes, fatais 1,3 . As pessoas de maior risco são os garimpeiros que, extraindo o ouro de forma legal ou clandestina, não utilizam o principal meio de proteção no manuseio do mercúrio, o cadinho: instrumento que permite a purificação mineral do ouro evitando a aerosolização do mercúrio e a aspiração deste pelo aparelho respiratório.Descrevemos um caso de intoxicação subaguda por mercúrio elementar, com graves repercussões no sistema circulatório, cuja expressão clínica mais marcante foi hipertensão arterial (HA) severa com marcadores bioquí-micos de ativação simpático-adrenal, simulando a síndro-me do feocromocitoma.
