resuMo OBJETIVO: No Brasil, 70% dos idosos possuem ao menos uma patologia crônica e, destes, 60% usam mais de 4 medicamentos regularmente, constituindo a polifarmácia. Medicamentos potencialmente inapropriados (MPI) são utilizados por 40% dessa população. Tanto a polifarmácia quanto os MPI estão associados a desfechos negativos, como maior frequência de interação medicamentosa, quedas, fragilidade, desnutrição e, em alguns casos, mortalidade. MÉTODO: Estudo transversal retrospectivo em que foram incluídos idosos internados por motivo clínico no hospital da Santa Casa de Misericórdia de Maceió (SCMM) entre março de 2015 e fevereiro de 2016. Foi avaliada a presença de polifarmácia e MPI, correlacionando-os com outras variáveis de interesse. RESULTADOS: Foram analisados 456 prontuários eletrônicos de pacientes com idade média de 83 anos, sendo 71,3% do sexo feminino. O índice de comorbidade de Charlson (ICC) médio foi de 2,38, sendo demência a morbidade mais prevalente (36,6%). Polifarmácia esteve presente em 56,5% dos pacientes e 46,4% tinham ao menos um MPI, sendo mais frequente o uso de antipsicóticos (46,2%), seguidos por benzodiazepínicos (33%). Interação medicamentosa foi detectada em 53,5% dos pacientes. A presença de polifarmácia se correlacionou com MPI (p < 0,001). Além disso, polifarmácia e MPI se correlacionaram com interação medicamentosa (p < 0,01), pior funcionalidade (p < 0,01) e maior ICC (p = 0,015). CONCLUSÃO: Na população estudada, a presença de polifarmácia foi de 56,5% e a de MPI, 46,5%. Ambas se correlacionaram positivamente com a presença de interação medicamentosa, pior funcionalidade e maior ICC. INTRODUÇÃO: A subnutrição é um problema frequente em idosos e sua prevalência aumenta entre os mais frágeis, tornando prioritária uma adequada oferta nutricional diante de eventos agudos. Entretanto, tal oferta, em pacientes desnutridos ou com privação calórica subaguda, pode ocasionar alterações hidroeletrolíticas associadas a sintomas neurológicos, respiratórios e cardiológicos, incluindo falência cardíaca. Esses distúrbios ocorrem poucos dias após a introdução da dieta, o que caracteriza a síndrome de realimentação. METODOLOgIA: Avaliou-se retrospectivamente cinco pacientes idosos internados por causa clínica e que desenvolveram síndrome de realimentação durante a internação hospitalar. RESULTADOS: Todos os pacientes analisados eram desnutridos e frágeis, dos quais quatro eram do sexo feminino. A idade variou entre 82 e 85 anos e o tempo de privação alimentar entre 7 e 15 dias. Três pacientes eram portadores de demência grave e foram admitidos com delirium hipoativo. Hipofosfatemia ocorreu em 100% da amostra, seguida por hipocalemia (60%) e hipomagnesemia (40%). Quatro pacientes cursaram com edema periférico (80%), dois apresentaram íleo metabólico e três idosos (os que apresentaram menores níveis de fosfato) faleceram durante a internação. CONCLUSÃO: Todos os casos de síndrome de realimentação analisados cursaram com hipofosfatemia, com desfecho fatal naqueles com menores níveis deste íon. A associação com edema e íleo metabólico foi comum. Dada a escassez de dados disponíveis na literatura, novos estudos são de extrema importância. A avaliação eletrolítica antes e após o início de suporte nutricional é imprescindível em pacientes de risco, auxiliando no reconhecimento e no tratamento precoce dessa grave condição. O aumento da expectativa de vida e da população idosa é fenômeno mundial. No Brasil, teremos 41,5 milhões de idosos em 2030, porém sequer metade das escolas médicas brasileiras incorporam conteúdos sobre o envelhecimento humano. Objetivando o diagnóstico situacional do ensino da Geriatria e da Gerontologia em um curso médico, realizou-se análise documental no Projeto Pedagógico do Curso (PPC) e planos de aula das disciplinas através da (1) definição de categorias de análise e unidades de registro, (2) exploração documental e (3) análise e interpretação dos dados. As unidades de registro foram identificadas nas diretrizes propostas pela Sociedade Brasileira de Geriatria e Gerontologia, totalizando 71 conteúdos mínimos a serem buscados no PPC. Houve previsão para abordagem de 13 dos 71 conteúdos, apenas quatro com abordagem completa. Verificou-se, deste modo, grande lacuna no ensino de conteúdos mínimos em geriatria e gerontologia em um curso médico.
