Closed loop systems are the ultimate solution to ensure that optimal therapies are delivered in a timely manner. A concept of a semi-closed loop infusion system for perioperative semi-automated optimisation of blood pressure and haemodilution is proposed. The key variable for the latter objective is the noninvasively and continuously measured blood haemoglobin concentration. However, it lacks reliability in predicting the haemoglobin in large blood vessels. Our proposed statistically biased calibration method for the adjustment of noninvasively measured Hb enabled better prediction of arterial Hb when it was applied to data from our ongoing clinical trial.

Prieš dešimtmetį įsivyravo individualus skysčių skyrimas vadovaujantis į tikslą nukreiptos skysčių terapijos (angl. goal-directed fluid therapy) algoritmais. Nors tuo metu tai buvo pažangus metodas, dabar jo naudojimo pagrįstumas jau sukelia diskusijų. Pavyzdžiui, abejojama, ar yra saugu, tikslinga ir fiziologiška maksimizuoti širdies susitraukimų jėgą. Taip pat įtariama, kad kraujotakos optimizavimas intraveniniais skysčiais gali sutrikdyti audinių hidrataciją. Deja, audinių skysčio tūrio matavimas ir jo pokyčių stebėsena yra neįmanomi klinikinėje praktikoje. Neseniai buvo sukurtas naujas metodas – mažasis infuzinis plazmos atskiedimo (mIPA) mėginys audinių hidracijai netiesiogiai įvertinti. Atliekant mIPA mėginį taikomos nedidelės kristaloidų infuzijos, kurias skiria 5 min. laikotarpiai be skysčių, o pagrindinis tikslinis parametras yra plazmos atskiedimas. Trumpoje apžvalgoje taip pat aptariamos hemoglobino (SpHbTM) neinvazyvaus matavimo galimybės plazmos atskiedimo stebėsenai. Aprašomi mIPA mėginio klinikinio išbandymo rezultatai ir ateities tyrimų hipotezės. The individualized goal-directed fluid therapy became the prevailing method a decade ago. However, now it is surrounded by the concerns and questions such as whether it is appropriate to maximize the cardiovascular performance. Also, there is a justified concern that it can be associated with the deterioration of body hydration status. The new method – minimal volume loading test (mVLT) – has been recently introduced for the indirect evaluation of the whole-body tissue hydration. It implies administration of consecutive relatively small crystalloid boluses separated by 5 minute periods without fluid. The plasma dilution is employed as the target parameter. The applicability of the noninvasive measurement of hemoglobin (SpHbTM) is discussed. The results of the initial mVLT method validation studies are shortly described along with the hypotheses for the future research. Straipsnio tikslas yra apžvelgti kristaloidų ir koloidų savybes, šių preparatų pranašumus ir trūkumus bei derinimo ypatumus perioperacinėje skysčių terapijoje.Racionali skysčių terapija gerina gydymo baigtį po didelių chirurginių intervencijų. Perioperacinei skysčių terapijai yra vartojami kristaloidų ir koloidų tirpalai bei jų deriniai. Nė viena iš tirpalų rūšių nėra pranašesnė už kitą, o nepageidaujamas poveikis labai priklauso nuo dozės. Trumpoje apžvalgoje aprašomos išskirtinės tirpalų savybės ir atsižvelgiant į naujausius mokslinius tyrimus apibūdinamos racionalaus jų dozavimo bei derinimo galimybės.
