Introduction: Patients with atopic dermatitis (AD) have several potential stressors including the symptoms of the disease itself, the stigmatization due to their appearance, and emotional and psychological strain. Psychological factors and stress can trigger and exacerbate the symptoms of skin diseases and there is evidence that stress has a relevant clinical effect on the function of skin cells in vivo. Our objective was to evaluate in a pilot study the feasibility, acceptance, and effectiveness of a Mindfulness-Based Stress Reduction (MBSR) programme in AD patients in a clinical setting. Methods: 10 patients took part in an 8-week MBSR programme, which included, e.g., mindful and conscious awareness of the body and bodywork, and seated meditation. We assessed sociodemographics and disease related variables with standardized measures at predefined time points including Score of Atopic Dermatitis, Patient Oriented Eczema Measure, Dermatology Life Quality Index, Perceived Stress Questionnaire, Freiburg Mindfulness Inventory (FMI), and others. Participants also gave qualitative feedback regarding the effects of the intervention. Results: The mean age was 53.10 years (SD = 15.04), seven patients were female, and disease duration was 36.6 years (SD = 25.5). Calculating pre-post effect sizes (Cohen’s d), the FMI indicated significant improvement in the “presence” and “acceptance” subscales. There was also tendency for less stress. This was confirmed by the qualitative statements of the participants. Conclusions: The MBSR programme is feasible and acceptable for AD patients. Considering the long disease history and the severity of disease burden, the effects of this intervention seem promising as an adjunct to conventional treatments for patients with AD.
Zusammenfassung In den letzten 30 Jahren hat das Interesse an einer Verwendung des k?rpereigenen Fettes zur ?sthetischen K?rperformung, speziell zur Augmentation der weiblichen Brust, kontinuierlich zugenommen. Fehlende Fremdk?rperreaktionen, harmonisches Aussehen und ein nat?rliches Gef?hl sind die Vorteile einer autologen Fettzell-Transplantation. Komplikationen wie Nekrosen, Infekte, Zystenbildungen, hohe Graftverluste sowie die Schwierigkeit, gro?e Fettmengen in vern?nftiger Zeit zu gewinnen boten in fr?heren Jahren Ansatz zu Kritik an der Methode des Eigenfett-Transfers. Mit der BEAULI?-Methode steht seit 2007 ein Verfahren zur rationellen Gewinnung und Aufbereitung gr??erer transplantabler Fettmengen zur Verf?gung. Das Ziel dieser Arbeit ist es, die Operationstechnik pr?zise und reproduzierbar zu beschreiben sowie aufgrund eigener klinischer Erfahrungen einen umfassenden ?berblick ?ber den autologen Fetttransfer zu vermitteln. Zwischen dem 1. September 2010 und dem 30. Juni 2012 f?hrte der Autor 96 Eigenfett-Transplantationen an 84 Patientinnen durch. Ausgew?hlt wurden Patientinnen zwischen 18 und 62 Jahren mit dem Wunsch nach einer moderaten Volumenvermehrung und Formung der Br?ste. Die Fettgewinnung erfolgte mittels wasserstrahl-assistierter Liposuction (Bodyjet?) und anschlie?ender Separierung der Fettzellen mit dem LipoCollector?. Postoperativ traten in keinem Fall Komplikationen auf. Die Resultate wurden im Rahmen einer Kontrolluntersuchung, eines Fotovergleichs und in der Beantwortung eines Fragebogens beurteilt. Bez?glich des Volumengewinns bzw. der Formverbesserung der Br?ste waren 35% der Patientinnen mit dem Ergebnis sehr zufrieden, 42% zeigten sich zufrieden, 22% h?tten sich noch mehr Volumen gew?nscht, 1% war unzufrieden. Diese Studie zeigt, dass die autologe Fettzell-Transplantation in die weibliche Brust mit der BEAULI?-Methode eine gute und sichere Methode ist, bei ausgew?hlten Patientinnen eine moderate und harmonische wirkende Brustvolumenvergr??erung sowie eine nachhaltige und nat?rliche wirkende Konturverbesserung zu erzielen. Weitere Studien mit hohen Fallzahlen und Beobachtungszeiten ?ber mehrere Jahre sowie Guidelines der Fachgesellschaften k?nnten dazu beitragen, die Methode des Eigenfett-Transfers bez?glich Einheilungsrate, Effizienz und Sicherheit zu perfektionieren.
