Death is one of the major taboos of contemporary Western society and is a forbidden and avoided subject. If in the past it was a public and domestic event, nowadays it is hidden in hospitals crowded with professionals who are unprepared to deal with issues related to the end of life. This unpreparedness is a reflex of excessively technical and non-humanistic training which favors healing over caring. Therefore, it seems medical students are taught to work against death and not with it. In this context, we sought to trace the profile of attitudes towards death of the students graduating from the Medicine course of a public university in Brazil and evaluate how this course has prepared them to deal with issues of death and dying in their perception. We employed quantitative and qualitative methodologies by applying two data collection instruments: a questionnaire to collect sociodemographic and academic data as well as data on the teaching about death, and another to define the profile of attitudes towards death (DAP-R). We analyzed the studied variables descriptively using frequencies and means, and intercorrelated them to infer the hypotheses. We also considered student statements for analysis and discussion of results based on comments on the responses to one of the questionnaires. As results, we observed that the students’ attitude towards death was characterized by a view of death as a natural event of life (neutral acceptance), while their imagination about death was permeated with conflicting feelings; that most students had experienced the death of a patient during the course (75.8%) although most of those received no guidance in these situations (80%); that the humanistic courses concentrated the approaches on death; and that such approaches somewhat contributed to create reflection and develop abilities for managing death. We concluded that teaching about death and dying is still concentrated in few courses of the medical curriculum, mainly in the humanistic ones, and is excessively theoretical, whereas the real experiences with death during the Medicine course are treated with detachment and silence when they could be better utilized for teaching and learning about the end of life.
_________________INTRODUÇÂO: As Diretrizes Curriculares Nacionais propõem um perfil de profissional da saúde com uma formação geral, crítica e reflexiva e que utilize metodologias de ensino-aprendizagem centradas no estudante, em diferentes cenários. Há uma forte discussão acerca dessa reforma curricular nos cursos de Medicina, os quais devem privilegiar a medicina preventiva, humanística e, portanto, o contato desde o início da graduação de seus discentes com a Atenção Primária à Saúde (APS). OBJETIVOS: Relatar a experiência da vivência na comunidade dos estudantes de medicina a partir do primeiro ano de graduação. MÉTODOS: para inserção precoce dos acadêmicos de medicina na Atenção Primária, o curso de graduação tem vínculo com quatro módulos de saúde, nos quais são distribuídos grupos de 6 a 10 estudantes, que, dentre outras atuações, realizam visitas domiciliares semanais. Essas atividades, que permitiram uma primeira vinculação entre estudantes e comunidade, estão relacionadas às disciplinas de Medicina da Família e Medicina Comunitária, ministradas no primeiro e no segundo semestre, respectivamente, e os grupos de alunos acompanhados na comunidade por preceptores médicos do próprio módulo de saúde. RESULTADOS: Percebem-se, na visão dos discente, ganhos e entraves. Ganharam os acadêmicos com a oportunidade de vivenciar e aprender sobre a APS, sobre o SUS e de pôr em prática os saberes aprendidos em sala de aula, principalmente no tocante ao morfofuncional. Além disso, esse tipo de experiência propicia a formação de médicos mais humanos e preocupados com os problemas sociais existentes na comunidade, juntamente com uma co-responsabilização por sua resolução. Também ganharam as famílias e a comunidade, pois, após a realização de um diagnóstico situacional, foi possível a realização de projetos de intervenção, normalmente em grupos, que tinham como foco sobretudo o empoderamento dos participantes. Ademais, percebeuse que as visitas periódicas dos acadêmicos promoveram um aumento do vínculo entre a comunidade e as equipes de saúde. Destaca-se como fator limitante o fato de que algumas famílias visitadas esperavam algum tipo de consulta ou tratamento medicamentoso e pelo fato dos acadêmicos terem pouca ou nenhuma prática semiológica e farmacológica, foram algumas vezes reduzidos ou desacreditados, tanto pelas famílias como por alguns agentes comunitários de saúde, o que talvez seja resquício do predomínio do modelo biomédico de outrora. CONCLUSÕES: o conhecimento adquirido durante a vivência certamente vai além do currículo acadêmico. Provocouse o diálogo mútuo entre serviço, academia e comunidade, e, apesar dos obstáculos ainda enfrentados, entende-se que pelos significativos ganhos, experiências como esta deveriam estender-se a todos os estudantes da área da saúde, não só a título de aprendizado, mas também para desenvolver a criticidade e o despertar da importância da medicina preventiva, com a conseqüente formação de profissionais mais humanizados.
