RESUMEN:En la actualidad la obesidad es considerada como una pandemia, además de los cambios metabólicos ya sabidos, se producen alteraciones estructurales como las generadas en el sistema ventilatorio. Sin embargo, se ha observado que dependiendo de la forma de categorización de esta condición, puede o no existir alteración sobre este sistema generando así una complicación al momento de la elección de esta herramienta. Por lo tanto el objetivo de esta investigación fue comparar parámetros de la función pulmonar en individuos obesos (O) y normo-peso (Np) según índice de masa corporal (IMC) y porcentaje de grasa corporal (PGC). Se midió la función ventilatoria en 57 participantes, posteriormente estas variables se contrastaron entre los sujetos normo-peso (Np) y obesos (O) según IMC y PGC. Se observó un aumento significativo en el valor de la capacidad inspiratoria (CI) de O versus Np y del volumen de reserva espiratoria (VRE) y capacidad residual funcional (CRF) de Np en relación a O, tanto para la división por IMC como por PGC, en el grupo dividido por IMC las variables de resistencia especifica de las vías aéreas (sRaw) y presión inspiratoria máxima (PImáx) fueron significativamente mayor en los participantes O, asimismo, la conductancia específica de las vías aéreas (sGaw) fue significativamente mayor en sujetos Np. En conclusión, el IMC mostró mayor versatilidad como predictor de la función ventilatoria, sin embargo, se necesitan mayores estudios de los efectos mecánicos de este indicador y del PGC sobre las vías aéreas. Introducción. La diabetes mellitus tipo 2 (DM2) es una enfermedad metabólica que afecta todas las aristas de la vida individual y familiar de la persona que la padece. La pandemia causada por el virus de SARS-COV-2 ha generado un problema relevante a nivel del sistema de salud, provocando una sobrecarga importante y una complejizarían de los servicios para atender la infección. Lo anterior, ha llevado a que muchas personas pierdan sus controles crónicos y no puedan cuidarse de manera adecuada. Métodos. La prueba de Wilcoxon se utilizó para comparar grupos de pacientes y variables continuas. Se aplicó un modelo de regresión lineal para estudiar la asociación entre la glicemia y la hemoglobina glicosilada. Se consideró un valor p <0.05 para aceptar la hipótesis alternativa de las pruebas estadísticas. Resultados. Se encontraron diferencias significativas entre población con niveles inferiores a 9% de hemoglobina glicosilada y niveles superiores o iguales a 9% en variables como glicemia, colesterol total, colesterol HDL, colesterol LDL y triglicéridos. En el modelo de regresión lineal se reportó R2 (0.61) y (0.22) entre la glicemia y la hemoglobina glicosilada con significancia estadística en todos los niveles (valor p<0.05). Conclusiones. Un mal ajuste de los niveles de HBA1C en población con DM2 podría generar una serie de comorbilidades como dislipidemias, hipertensión, enfermedad cardiovascular o infarto agudo al miocardio producto de la glucotoxicidad y lipotoxicidad.
: The Sierra Nevada de Santa Marta is a coastal mountain bio-region - the world's highest — located in the North of Colombia. Its administrative structure includes a complex series of territorial bodies encompassing three states and thirteen municipalities containing two indián reservations and two protected regions which overlap each other. The promulgation of Plans for Territorial Ordering for the municipalities included in the Sierra Nevada Region was the result of the country's restructuring effected by the Political Constitution of 1991 and Law 388 on Territorial Restructuring. It then became necessary to reflect on the conflicts resulting from the simultaneous existence in the massif of several territorial authorities added to the lack of States presence to enforce such ordering, particularly in a region of biological and cultural preservation with a high percentage of native and peasant population. The current panorama of available means to materialize the territorial ordering of the Sierra from a bio-regional standpoint is not encouraging. The major problems that have been identified are : • The lack of articulation between the several territorial and environmental bodies present in the massif (states, municipalities, indián reservations, national parks, autonomous corporations). • The strong presence of the Colombian armed conflict which hinders State intervention and coerces citizens participation. • The inefficacy of the political system and the ensuing wasting of economic, legal and logistic resources. • The technical limitations of planners and the managerial inability of citizens. The strengthening of available mechanisms such as communal action boards, economic incentives to preservation and sustainable development, and community work through NGO's, are some of the means proposed to improve the process of the territorial restructuring of the region. At the regional level, the territorial conflict of the Sierra Nevada de Santa Marta reflects the country's reality and depicts the current situation of the long-term process of national restructuring.
RESUMEN Introducción En los últimos años han aumentado los casos de pacientes de 50 años o menores, con patologías tumorales colorrectales. Actualmente se realizan estudios tempranos, sea por el aumento social de preocupación o bien por pesquisa de patologías. Lo demuestran varias investigaciones publicadas últimamente. Este trabajo pretende averiguar si en el Sanatorio Adventista del Plata ha habido incremento de pacientes operados por tumores colorrectales, causados por cáncer o tumor diverticular, entre otros. Se estudiaron pacientes intervenidos en la institución, servicio de Coloproctología, desde enero de 2013 hasta octubre de 2020. Objetivo Saber si hay aumento de casos en personas de 50 años o menores, entre los pacientes operados por patología colorrectal en el servicio de Cirugía. Conocer cuál es la patología causal más común, estado nutricional prequirúrgico, complicaciones postoperatorias más frecuentes, tipo de cirugía que más se realizó y sexo comprometido predominante. Diseño Retrospectivo, de corte transversal, tipo descriptivo. Material y Métodos Pacientes de 50 años o menores, operados por el servicio de Coloproctología del Sanatorio Adventista del Plata, Entre Ríos, Argentina, entre enero de 2013 y octubre de 2020. Resultados Incremento en pacientes operados con aumento anual de la proporción en pacientes de 50 años o menores. Los más afectados tienen sobrepeso, siguen en frecuencia pacientes con peso normal y tercero, con obesidad grado 1. Adenocarcinoma es más común como causal, seguido de tumores diverticulares. Hemicolectomía izquierda es la más frecuente, seguida de resección anterior de recto o cirugía de Dixon. Predomina el sexo masculino. Conclusiones Incremento anual en la cantidad de cirugías colorrectales efectuadas en el Sanatorio Adventista de Plata. 2. Aumento en el porcentaje de pacientes operados, de 50 años o menores. 3. ABSTRACT Background In recent years there has been an increase in the number of patients aged 50 years or younger with colorectal tumor pathologies. Currently, early tests are performed, due to either the increase of social concern or pathology screenings. This has been shown in several recently published studies. This paper intends to inquire whether there has been an increment in the number of patients operated on for colorectal tumors caused by cancer or diverticular tumors, among others, at the River Plate Adventist Hospital. Patients surgically intervened at the institution, Coloproctology Service, from January 2013 to October 2020, have been studied. Aim To know whether there is an increase in the number of people aged 50 years or younger, among patients operated on for colorectal pathologies in the Surgery department. To know the most common causative pathology, preoperative nutritional status, most frequent postoperative complications, the most common type of surgery performed and, predominant sex involved. Design Retrospective, cross-sectional, descriptive type. Method Patients aged 50 years or younger operated on by the Coloproctology Service in River Plate Adventist Hospital, Entre Rios, Argentina, between January 2013 and October 2020. Result There has been an increase in the number of operated patients with an annual rise in the proportion of patients aged 50 years or younger. The most affected patients are overweight, followed in frequency by patients with normal weight and thirdly, with grade 1obesity. Adenocarcinoma is the most common cause, followed by diverticular tumors. Left hemicolectomy is the most frequent, followed by anterior rectal resection or Dixon's surgery. Male sex predominates. Conclusions Annual increment in the number of colorectal surgeries performed at the River Plate Adventist Hospital. 2. An increment in the percentage of patients aged 50 years or younger surgically intervened. 3. Overweight is present in the majority of the studied sample. Key words: Colorectal tumors.
