A síndrome da veia cava superior é a expressão clínica da obstrução ao fluxo sanguíneo na veia cava superior que abrange uma constelação de sinais e sintomas. Dentre várias etiologias, há condições benignas que podem levar a essa síndrome, como a mediastinite crônica fibrosante. Dentre as opções de tratamento cirúrgico, podem-se usar alguns tipos de enxerto, como o criopreservado, PTFE e pericárdio bovino. Neste trabalho, é descrito um relato da abordagem cirúrgica rara da mediastinite crônica fibrosante como tratamento da síndrome da veia cava superior para que seja proporcionado maior conhecimento do tema para a comunidade científica.
