SumárioA síndrome metabólica (SM) tem gerado enormes controvérsias, a iniciar-se pela terminologia. O próprio autor que sugeriu o termo, Gerald Reaven, recomenda que não o utilizemos, porque, como a síndrome é um apanhado de alterações metabólicas e três dessas alterações podem defini-la, fica sempre difícil saber-se a que conjunto de alterações estamos nos referindo quando conotamos o termo SM. Em crianças, a complicação é ainda maior, já que se fazem diferentes adaptações aos critérios propostos para adultos. De qualquer forma, independentemente da terminologia, os riscos cardiovasculares são bem estabelecidos e fica cada vez mais claro que as crianças, já em tenra idade, podem começar a ter alterações metabólicas preditivas de problemas mais sérios futuramente. Assim, o papel do endocrinologista pediátrico e do pediatra geral é de investigar essas crianças, especialmente se portadoras de sobrepeso ou obesidade, para detectar precocemente e intervir em condições que podem ser prevenidas na vida adulta. Essa é uma nova visão de uma abordagem pediátrica preventiva de problemas que, usualmente, só são diagnosticados em vida adulta. Gerald Reaven himself, the author who proposed the term MS, advised against the use of this terminology because the definition implies in at least three metabolic alterations, and it is never clear to which group of alterations we are referring to when we say that a patient has MS. In children, the problem is even more complicated, since there are many different adaptations to the criteria used in adults. On the other hand, independent of the terminology, cardiovascular risks are well-established and it is very clear that even children may present metabolic disturbances which predict future metabolic problems. The role of the pediatric endocrinologist or the general pediatrician is to investigate, especially in overweight/ obese children, conditions that if treated early, may prevent future complications that today, unfortunately, are being diagnosed only in adult life. A close microanatomical relationship between serotonin-positive mast cells and nerve fibres positive for substance P, calcitonin gene related peptide, vasoactive intestinal polypeptide, and somatostatin has been observed in whole-mount preparations of rat mesentery by an immunofluorescent double-staining procedure. Peptidergic fibres have been shown either to run in close proximity or come in direct contact with mast cells. This supports earlier morphological and immunohistochemical results suggesting an innervation of mast cells and provides a structural foundation for a series of pharmacological studies which outline the influence of various neuropeptides on mast cell secretory activity.

It has become clear that a sexual br ain dimor phism exists between males and females and animal studies have shown specific circuits developing depending on the sex. Since the first studies by Gor ski, in rats, character izing a sexually dimor phic nucleus in the pr e optic ar ea (SDN-POA) it has been accepted that the male sex imprint is done by estradiol, locally conver ted from testosterone thr ough the action of a local ar omatase. The pr esence of estr adiol inhibits the apoptosis of the cells of SDN, making it bigger in the male sex. Other CNS r egions have shown sexual dimor phism and we need a mar ker to allow us to identify these str uctur es and, eventually, apply this infor mation to clinical pr actice. In inter sex patients, it may be of value to know which the br ain sex of the patient is, since we have had many doubts in choosing the sex of r ear ing in many of these patients. It has not been uncommon that sexual inadequacies have occur r ed in some patients, causing a lot of discomfor t and suffer ing for the patient himself as well as to his family. Objetivo: Revisão sobre o uso de inibidores de aromatase, uma nova modalidade terapêutica em pacientes com baixa estatura, numa tentativa de evitar o avanço rápido da idade óssea, dependente da produção estrogênica, mesmo no sexo masculino. Fontes dos dados:MEDLINE, com levantamento dos últimos 10 anos, com os termos inibidor de aromatase, baixa estatura e puberdade precoce, selecionando os textos mais informativos a respeito das indicações, uso, esquemas de tratamento e efeitos colaterais dos inibidores de aromatase. Síntese dos dados:Tem se tornado evidente que o avanço da idade óssea depende da produção estrogênica e da ação desse hormônio sobre a placa de crescimento. Nos meninos, a conversão testosterona para estradiol ocorre pela ação da enzima P450 aromatase. O uso de bloqueadores desta enzima tem se mostrado efetivo em prolongar o tempo de crescimento em crianças com baixa estatura idiopática, atraso constitucional de crescimento e puberdade e mesmo na deficiência de hormônio de crescimento, em que o avanço da idade óssea coloca em risco os resultados da terapia com reposição hormonal com hormônio de crescimento. Não tem havido problemas com efeitos adversos, e os resultados são animadores em termos de melhora efetiva da altura final sempre que a indicação tenha sido pertinente. Skeletal maturation is estrogen-dependent even in male children. Sources:We performed a MEDLINE search of studies published in the last 10 years, including aromatase, short stature, and early puberty as keywords. The most informative articles on indications, dosages, treatment schedules, and side effects of aromatase inhibitors were included in the review. Summary of the findings:It has become increasingly clear that bone age advancement depends on the production of estrogen and its effect on the growth plate. In boys, testosterone is converted to estradiol by the cytochrome P450 enzyme aromatase. The use of aromatase inhibitors has been shown to be effective in prolonging the length of the growth phase in children with idiopathic short stature, constitutional growth delay, delayed puberty, as well as in children with growth hormone deficiency, in which bone age advancement jeopardizes the results of hormonal replacement therapy with growth hormones. As yet, significant adverse effects have not been reported, and results are encouraging in terms of effective increase in height, whenever the indication for the drug is appropriate.
