DiscusiónEl granuloma de células gigantes (GCG) fue descrito por primera vez por Jaffe 1 en 1953 como "granuloma reparativo de células gigantes", si bien actualmente no se refiere a él como reparativo, debido a su carácter localmente destructivo. Es una entidad relativamente rara. Supone un 7% de los tumores de los maxilares (siendo su localización preferente en la región de los incisivos, y más frecuentemente en la mandí-bula que en el maxilar) 2 . Es más habitual en niños y adultos jóvenes, con un ligero predominio en el sexo femenino 2 .Histológicamente se caracteriza 3 por la intensa proliferación de células gigantes multinucleadas y fibroblastos, sobre una densa estroma vascular, con depósitos de hemosiderina. Se clasifica en: a) periférico, si afecta a las extremidades, y b) central, si se desarrolla en la línea media (siendo este tipo menos frecuente). En cualquier caso, siempre afecta a tejidos con soporte óseo.Como factores etiológicos 4 se han relacionado diversos factores, sobre todo irritantes locales (como extracciones dentales o prótesis mal adaptadas) y hormonales 3 (de hecho, ante todo en el GCG debería descartarse la coexistencia de hiperparatiroidismo primario, ya que los clásicos tumores pardos característicos de esta afección tienen una histología prácti-camente indiferenciable de la del GCG). Otra teoría sobre su origen refiere que se trata de una lesión vascular intraósea similar a los angiomas de tejidos blandos 5 .El diagnóstico diferencial se realizará con el granuloma piogénico, el fibroma gingival, el fibrosarcoma y las metásta-sis de tumores a distancia, que clínicamente pueden parecerse al GCG. El diagnóstico definitivo viene dado por la biopsia. Clínicamente se manifiestan como una masa o nódulo de color rojizo (aunque a veces puede ser azulado), de consistencia carnosa y ocasionalmente ulcerada superficialmente. Pueden ser desde asintomáticos, de pequeño tamaño y lento crecimiento, hasta grandes y destructivas lesiones que crecen rápidamente. En cuanto al manejo, clásicamente se recurría
