Latar belakang: Seiring bertambahnya usia akan menyebabkan penurunan fungsi sistem tubuh, salah satunya penurunan fungsi kognitif. Terganggunya fungsi kognitif pada lansia dapat berupa gangguan berbahasa, kehilangan memori jangka pendek maupun panjang, juga disorientasi waktu dan tempat. Berdasarkan hasil pra survei yang dilakukan di Rojin Home Itoman Teinsagu No Ie Jepang didapatkan bahwa jumlah lansia sebanyak 35 lansia dan sebanyak 18 lansia menderita demensia. Tujuan: Penelitian ini bertujuan untuk mengetahui gambaran fungsi kognitif pada lansia di Rojin Home Itoman Teinsagu No Ie Jepang menggunakan instrument Mini Mental State Examination. Metode: Penelitian ini menggunakan metode pemelitian deskriptif kuantitatif. Teknik pengambilan sampel pada penelitian ini menggunakan teknik total sampling. Terdapat 33 lansia sebagai responden. Hasil: Hasil penelitian menunjukkan sebagian besar responden dengan kategori lanjut usia tua (75-90 tahun) sebanyak 24 responden (72,7%), dominan responden berjenis kelamin perempuan sejumlah 21 responden (63,6%) dengan sebagian besar mengalami penurunan fungsi kognitif sedang sejumlah 19 responden (57,6%). Seluruh responden 33 (100%) benar pada perintah pengulangan kata, sejumlah 29 (87,9%) responden benar pada tentang perintah menutup mata. Hal tersebut berarti bahwa fungsi kognitif lansia baik pada penilaian bahasa tentang pengulangan kata dan perintah sederhana. Tetapi sejumlah 20 (60,6%) responden mendapat nilai 0 pada pertanyaan tentang kalkulasi/perhatian. Kesimpulan: Penurunan fungsi kognitif lansia merupakan penyebab terbesar terjadinya ketergantungan terhadap orang lain untuk merawat diri sendiri akibat ketidakmampuan dalam melakukan aktivitas sehari-hari. Perawat dapat mengkajinya menggunakan MMSE agar lansia segera mendapat terapi. Kata kunci: Fungsi Kognitif; Demensia; Lansia; MMSE
