The vegetative dysfunction, having been present since childhood, becomes a premorbid abnormality and harbinger of many diseases. Given the increased frequency of vegetative dysfunction recent years, there is a need to study this problem.
Childhood arterial hypertension (AH) has become a global problem not only for pediatrics, but also for public health in general.Objective. The aim of this study was to determine the prevalence of arterial hypertension among school-age children and diagnostic value of pulse pressure. Materials and methods.In total 848 children in 10-17 years of age (mean age -13.9 ± 0.06 years, 45.4 % boys and 54.6 % girls) from urban and rural areas of Chernivtsi region were examined in schools during screening for elevated blood pressure (BP). School-based ambulatory seated BP was measured by oscillometric automated recording devices with the age selection of cuffs. Elevated systolic (SAP) or diastolic (DAP) pressure was diagnosed in those children with indicators above the 95 th percentile of age norms in accordance with the national normative values. In cases of elevated BP the measurements were verificated by aneroid device.Results. It has been established that the frequency of high blood pressure in the examined school students was 25.2 %, including 17.7 % of arterial hypertension (BP above 95 percentile) and 7.5 % -pre-hypertension (90-95 percentile). The prevalence of hypertension varied with age subgroups and the highest level was in 16 years of age -29.9 % with high blood pressure (21.9 % above 95 percentile and 8.0 % between the 90 th and 95 th percentiles). Increased SAP was combined with an increased DAP in 47.3 % of cases and in the study had a positive correlation with overweight (r = 0.27, p < 0.05) and negative with physical performance (r = -0.21, p < 0.05). The pulse pressure had significant correlations with SAP, AH and overweight and it could be used as additional diagnostic index of AH.Conclusions. The prevalence of elevated BP in our study is higher than in European countries and exceeds 20 % of the child population. The elevated BP in our children has positive correlation with overweight and the negative with physical performance. Introduction. In recent years, there has been anincrease in the prevalence of hypertension in both adults andchildren. The discrepancy in the prevalence of the diseasein children is associated with the absence of a standardizedmethod of measuring blood pressure, uniform criteria forassessing its level, as well as the "hypodiagnostics" ofarterial hypertension in children. Attention should alsobe paid to the relatively low compliance of some childrenand adolescents in the treatment and awareness of the needto maintain a sufficient level of physical activity, healthyeating and prevention of bad habits.The aim of the study was to determine the influence ofmicrosocial factors on the formation of arterial hypertensionin schoolchildren.Material and research methods. 848 school - agechildren were examined and divided into the followinggroups: the first group consisted of 182 children withhypertension, the second group - 78 children with arterialprehypertension and the control group - 588 children withnormal blood pressure.Sociometric survey was conducted by collectinginformation about the peculiarities of diet and quality ofnutrition, physical activity and psychological factors inchildren using questionnaires. Blood pressure level wasmeasured with automatic tonometers with variable cuffs inthe morning on both hands, three times, with an intervalof 2 minutes. The results were evaluated according topercentile tables in terms of age, sex and height. Statisticalprocessing of the obtained data was performed usingMicrosoftOfficeExcel and Statistica 10.0.Results. Analyzing the psychological atmospherewithin the family, it was found that 585 (69.0%) childrencalled the situation within the family good and warm, 100(12.1%) thought that family conditions were bad and 138(16.3%) children did not decide on account of bad or goodrelations within the family.Assessing the peculiarities of nutrition among thesurveyed schoolchildren, it was found that children fromthe 1st group ate the most irrationally. They ate bread andbaked goods more often every day, meat and dairy productsa little less often, and vegetables and fruits even less often.Equally important factors influencing a child’s healthand development are the presence of physical activity, dailyroutine, walks in the fresh air, and so on. Another importantfactor that affects both the general health of the child andhis physical development is regular exercise in sections orclubs.Children from the 1st group have the lowest level ofregular sports, unbalanced diet, and more often havepsychological problems in relationships with peers andparents. Given the data of statistical analysis, thesemicrosocial factors can be regarded as risk factors for thedevelopment of hypertension in schoolchildren.Conclusions. According to the conducted correlativeand cluster analysis, three key groups of microsocial factorsthat affect the level of blood pressure in children can beidentified: the factor of balanced nutrition, psychosocialfactor and the factor of physical activity. The abovefactors are complex, they can be classified to the categoryof risk factors for the development of hypertension inschoolchildren and early prediction of increased bloodpressure in them.

Артеріальна гіпертензія виникає в результаті порушень механізмів нейрогуморальної регуляції діяльності серцево-судинної системи. Найбільш схильні до розвитку артеріальної гіпертензії діти препубертатного і пубертатного віку, що визначається властивими цим періодам дитинства вегетативними дисфункціями і мікроциркуляторними розладами у них. Доведено, що мікроциркуляторні розлади лежать і в основі порушень роботи серцево-судинної системи, зокрема в регуляції артеріального тиску. Стан мікроциркуляції можна оцінити за допомогою інфрачервоної термографії. Мета роботи -вивчити особливості та діагностичну цінність термографії дистальних відділів верхніх кінцівок у дітей шкільного віку з артеріальною гіпертензією. Матеріал і методи. Обстежено 61 дитину шкільного віку. Дітей розподілено на три групи: до 1-ї, основної групи (21 особа), увійшли діти з артеріальною гіпертензією, до 2-ї, основної групи (20 осіб), -діти з передгіпертензією, контрольну групу (20 осіб) складали діти з нормальним рівнем артеріального тиску. У дітей проводилися анамнестичне опитування та загальне клінічне обстеження. Вимірювання АТ виконувалося сертифікованими автоматичними тонометрами (OMRON 705ІТ, фірми OMRON, Японія) у ранковий час, на обох руках, тричі, з інтервалом у 2 хвилини. Середні значення тиску оцінювалися за перцентильними таблицями відносно віку, статі і зросту. Інфрачервону термографію у дітей проводили дистанційно за допомогою сертифікованого тепловізора Flir One Pro. Термографію дітям проводили на руках, на тильній поверхні, особливу увагу приділяли дистальній частині пальців (проекції нігтьових пластинок). Допустима похибка при вимірюванні температури була ±0,15ºС. Результати. За результатами тонометрії отримано такі дані: у 20 дітей (32,8%) рівень артеріального тиску був у межах коридору 90-95%, що оцінено як артеріальну передгіпертензію (2-га основна група); у 21 дитини (34,4%) -перевищував 95%, що розцінено як артеріальну гіпертензію (1-ша основна група); у 20 дітей (32,8%) -знаходився в межах 25-75 перцентильного коридору, що відповідає нормальному рівню (контрольна група). За даними термографії, середня найвища температура тильної поверхні дистальних фаланг у цих дітей становила 29,7±0,8ºС та коливалась від 25,7ºС до 32,1ºС. У дітей з контрольної групи середня найвища температура дорівнювала 30,7±0,7ºС і коливалась від 28,9ºС до 32,1ºС. Найвища температура реєструвалась на фаланзі 3-го пальця у 17 (85,0%) дітей, на фаланзі 2-го пальця -у двох дітей (10,0%) та в однієї дитини (5,0%)на фаланзі 4-го пальця, найнижча -на фаланзі 5-го пальця. У дітей з артеріальною гіпертензією спостерігалося зниження максимальної температури термоемісії на тильній поверхні кисті, та зміщення зони найвищої термоемісії на вказівний палець порівняно з дітьми з контрольної групи, у яких ця зона знаходилася переважно на середньому пальці.
