По МКБ10 на сегодняшний день острый и хронический аденоидит не выделены в отдельную нозологи ческую форму. Острый аденоидит обычно проходит по шифру острого назофарингита (J02) или ОРВИ (J06.9), а хрони ческий аденоидит -по другим хроническим болезням миндалин и аденоидов (J 35.8). Реактивные изменения глоточной миндалины начинаются на 3-5й день от начала ОРВИ, продолжаются и постепенно исчезают в течение 2-3 нед. Фак тически в большинстве случаев острый аденоидит является физиологической реакцией глоточной миндалины как органа регионарного мукозального иммунитета, на антигенную стимуляцию. Не существует единого мнения о длительности воспалительного процесса, при котором данные изменения можно считать хроническими. В отсутствии сопряженных осложнений и значимого влияния на качество жизни ребенка это состояние не является патологией. На практике таким детям чаще всего назначают ирригационную терапию. Учитывая, что во всем мире оториноларингологи не рассматрива ют хронический аденоидит как отдельную нозологическую форму, а выделяют лишь гипертрофию аденоидных вегетаций или, при наличии воспалительных изменений в носоглотке, хронический риносинусит, то корректно говорить о хрониче ском аденоидите при сохранении клинических проявлений заболевания более 12 нед. По преобладающему этиологиче скому компоненту условно можно выделить вирусный, бактериальный или аллергический аденоидит. Определить, какой именно этиологический фактор превалирует на практике, трудно. Все вышесказанное ставит множество вопросов о необходимости назначения как системной, так и топической антибактериальной терапии.
