Introduction Cardiovascular diseases (CVD) are the main cause of death worldwide, and according to experts, they will continue to dominate the structure of global mortality. Aim The effectiveness of the multimodal low-opioid anesthesia technique in performing coronary artery bypass graft operations with artificial blood circulation. Material and methods Ninety-six patients aged 61.8 ±10.4 years underwent coronary artery bypass grafting under artificial blood circulation. Group I: propofol, sevoflurane, fentanyl, pipecuronium bromide (standard doses). Group II: dexketoprofen trometamol (50 mg), intravenous lidocaine (1% – 1 mg/kg bolus) and continuous lidocaine infusion (1.5–2 mg/kg/h), propofol, ketamine (0.5 mg/kg), magnesia sulfate, minimal doses of fentanyl. Results Average duration of anesthesia – 257.4 ±19.1 min; assisted blood circulation – 55 ±10 min. Mean dose of fentanyl in group I – 4.66 ±1.58 µg/kg/h, in group II – 1.29 ±0.32 µg/kg/h.Standard lab values and stress hormonal changes were within the normal range (mean cortisol: 479.3 ±26.4 nmol/l, lactate 1.61 ±0.2 mmol/l, glucose 6.42 ±0.9 mmol/l). Changes in heart rate within group I had a significant amplitude of dynamics, while in group II, these values were relatively at the same level throughout the entire anesthetic provision. Mean arterial pressure changes in group I were characterized by a significant reduction at the stage of induction, support and sternum reduction, whereas in group II it was relatively at the same level during the entire anesthetic management and significantly differed from baseline only at the stage of induction. Conclusions Multimodal low-opioid anesthesia during coronary artery bypass surgery with artificial blood circulation allows one to ensure adequate analgesia and avoid the intraoperative usage of routine doses of fentanyl, as indicated by the absence of hemodynamic and endocrine-metabolic changes.
З кожним роком прогрес тільки набирає обертів, і все більше серцево-судинних захворювань лікуються із застосуванням міні-інвазивних методів. Це зумовлює стрімкий розвиток медичних технологій, упровадження нових методик лікування й діагностики у сфері кардіохірургії та інтервенційної кардіології. Незважаючи на те що в Україні удосконалилася підготовка лікарів — інтервенційних кардіологів, створений Український реєстр з інтервенційної кардіології, питання анестезіологічного забезпечення інтервенційних процедур залишається дискусійним та недостатньо вивченим. У статті наводиться огляд проблеми анестезіологічного забезпечення в інтервенційній кардіології.
II за ASA) проведено двобічну субарахноїдальну блокаду (ДСБ) 15 мг ізобаричного бупівакаїну у поєднанні з 0,06 мг бупренорфіну при аналогічній операції. ОСБ і ДСБ ефективно забезпечували знеболювання операцій і тривалу післяопераційну аналгезію. При ОСБ гемодинаміка була стабільною внаслідок застосування однобічного, а не двобічного симпатичного блоку на відміну від ДСБ. Покращення якості аналгезії дозволяло проводити ранні активізацію та ентеральне харчування пацієнтів і скорочувало період реабілітації. Субарахноїдальне введення бупівакаїну з додаванням бупренорфіну характеризувалося значно меншою кількістю негативних побічних ефектів порівняно з відповідним уведенням інших опіоїдів.
In the main group (n = 20; 39.4 years ± 4.1 years; 71.4 kg ± 4.3 kg, I–II ASA) asynchronous intrathecal anaesthesia (AIA) 10 mg of hyperbaric bupivacaine in combination with 0.06 mg of buprenorphine was administered in patients with inguinal hernia repair. The control group (n = 20; 42.6 years ± 0.9 years; 75.4 kg ± 3.6 kg, I–II ASA) received synchronous intrathecal anaesthesia (SIA) 15 mg with isobaric bupivacaine in combination with 0.06 mg of buprenorphine during similar operation. The AIA and SIA provided effective pain relief for operations with long postoperative analgesia. In the AIA group, a hemodynamics was stable due to the lateral rather than bilateral sympathetic block, in contrast to the CIA. Improving the quality of analgesia allowed an early activation and enteral nutrition of patients, as well as a shortened rehabilitation period. An intrathecal administration of bupivacaine supplemented with buprenorphine was characterized by a significantly smaller number of negative side effects compared with the corresponding administration of other opioids.
