Несмотря на значительные достижения в лечении пациентов с сердечно-сосудистой патологией, во всем мире эта группа заболеваний остается основной причиной смертности� Пациенты хирургического профиля, страдающие сердечно-сосудистыми заболеваниями, находятся в зоне высокого риска развития послеоперационных осложнений и смерти, а около половины случаев послеоперационной летальности после некардиохирургических вмешательств приходится на сердечно-сосудистые осложнения� Ингибиторы ангиотензинпревращающего фермента, как и блокаторы рецепторов ангиотензина II, являются одними из наиболее часто используемых в кардиологии препаратов, которые доказанно замедляют прогрессирование заболевания, повышают качество жизни и снижают летальность� Однако использование данных препаратов в предоперационном периоде может быть связано с такими неблагоприятными явлениями, как гипотензия и послеоперационная почечная дисфункция� Таким образом, возникает необходимость рассмотреть обоснованность терапии блокаторами ренин-ангиотензин-альдостероновой системы в периоперационном периоде у пациентов кардиохирургического и некардиохирургического профилей� Ключевые слова: ингибиторы ангиотензинпревращающего фермента, блокаторы рецепторов ангиотензина II, сердечно-сосудистая хирургия, общая хирургия
Ингибиторы ГМГ-КоА (3-гидрокси-3-метил-глутарил-КоА) редуктазы, более известные как статины, снижают уровень липопротеидов низкой плотности и общего холестерина плазмы крови� Их эффективность при первичной и вторичной профилактике сердечно-сосудистых заболеваний доказана множеством исследований� Помимо основного механизма действия, статины обладают плейотропными свойствами, включающими противовоспалительную защиту эндотелия, стабилизацию тромбоцитов, снижение концентрации фибриногена и предотвращение нейродегенеративных и онкологических заболеваний� Несмотря на возрастающую в последнее время настороженность в отношении побочных эффектов, статины остаются наиболее часто назначаемыми препаратами в мире� В работе рассматриваются различные стратегии применения статинов в периоперационном периоде у пациентов кардиохирургического и общехирургического профилей�
