RESUMOA alfafetoproteína (AFP) é uma glicoproteína de peso molecular de aproximadamente 69.000 daltons. É sintetizada pelo saco gestacional (vesícula vitelínica), trato gastrointestinal e principalmente pelo fígado fetal 1 . Pequenas quantidades podem ser produzidas pelos rins e placenta. Os níveis plasmáticos fetais atingem o pico entre 10-13 semanas de gestação (atingindo em torno de 2.480 KUI/ml), declinam exponencialmente entre 14 e 32 semanas e ainda mais acentuadamente até o termo. A queda na concentração plasmática fetal da AFP pode ser atribuída ao aumento do volume sangüíneo fetal e à diminuição de sua síntese 2,3 . A AFP passa para o líquido amniótico através da urina fetal. Sua concentração máxima no líquido amniótico é atingida entre 12 e 14 semanas de gestação, diminuindo em torno de 13% a cada semana durante o segundo trimestre e sendo praticamente indetectável ao termo. A presença de sangue fetal, mesmo em pequena quantidade, eleva os níveis no líquido amniótico fornecendo resultados espúrios 2 . A concentração da AFP sérica materna eleva-se a partir da sétima semana de gestação, mas os níveis são muito inferiores aos do líquido amnió-tico; o pico ocorre entre 28 e 32 semanas. A discre- pância entre a curva da AFP no soro materno com relação aos níveis no sangue fetal e no líquido amniótico pode ser explicada pela maior permeabilidade da placenta às proteínas plasmáticas fetais com o progresso da gestação. A contribuição da AFP do líquido amniótico para a concentração sérica materna é desprezível 2 . A função biológica da AFP no feto permanece desconhecida. Especula-se uma função imunológica envolvida na prevenção da rejeição do feto pela mãe 2 . A associação de níveis elevados de AFP no líqui-do amniótico com defeitos abertos do tubo neural é conhecida desde 1972. Diversos estudos estabeleceram o período ideal de amniocentese para dosagem de AFP na avaliação de defeitos abertos do tubo neural entre 16-18 semanas, por haver a menor sobreposição entre a curva normal e a alterada neste período da gestação 2 . Outras causas de elevação da AFP no líquido amniótico incluem condições fetais que permitam a passagem de proteínas plasmáticas do feto para o líquido amniótico através da pele (defeitos da parede abdominal, hidropsia fetal, cistos, seqüência de bandas amnióticas, morte fetal, etc.), do trato urinário (hidronefrose, doença policísti-ca) ou da placenta (sangue fetal, mola hidati-
