Placental macrophages (Hofbauer cells) are located close to trophoblastic cells and foetal capillaries, which make them perfect candidates for involvement in regulatory processes within the villous core. Their capacity of producing several cytokines and prostaglandin-synthesising enzymes, and expressing vascular endothelial growth factor, indicate a possible role in placental development and angiogenesis in order to support pregnancy. Common cells to Hofbauer macrophages sharing similar cell surface markers (HLA-A, -B, -C and leukocyte common antigen) have been reported in the stroma, decidua and amnion, indicating additional foetal protection. Yet this is not always the case. Most spontaneous abortions occur before 12 weeks' gestation, and most are due to chromosomal errors in the conceptus. Relatively few truly spontaneous abortions take place between 12 and 20 weeks' gestation. Thereafter, between 20 and 30 weeks, another type of premature spontaneous termination becomes prevalent, which is due to ascending infection. The numbers of cells expressing the various markers of the monocytemacrophage lineage change throughout pregnancy. In the present study, we investigated the immunohistochemical expression of mononuclear infiltrations in paraffin-embedded placentas, from foetuses after spontaneous abortion (8th, 10th and 12th weeks of gestational age), and those after therapeutic abortion at the same time, using a panel of monoclonal antibodies for the identification of leukocytes (CD45/LCA), B-lymphocytes (CD20/L-26), T lymphocytes (CD45RO/UCHL1), CD68 and CD14 cells. Immunologic factors in human reproductive failure are plausible mechanisms of infertility and spontaneous abortion. Approximately 25% of cases of premature ovarian failure appear to result from an autoimmune aetiology. Unfortunately, current therapeutic options for these women are limited to exogenous hormone or gamete substitution. Local inflammations at the sites of endometriosis implants are postulated to mediate the pain and reduced fecundability associated with this clinical syndrome. The recruitment of immune cells, particularly monocytes and T-cells, neovascularisation around foci of invading peritoneal lesions, and the possible development of antiendometrial autoantibodies support an immunologic basis of this disorder. To date, treatment of pain and infertility associated with endometriosis is primarily surgical, although immune-based adjuvants are theoretical possibilities for the future. Finally, although hypotheses supporting immunologic mechanisms of recurrent pregnancy loss have been popular over the past decade, most clinical investigations in this area do not provide compelling evidence for this position. Reputable specialists in reproductive medicine use experimental immunotherapies judiciously in selected cases of repetitive abortion. Aim: To detect whether preeclampsia influences neonatal intrahepatic hematopoiesis, given that an activation of fetal neutrophils and monocytes during the course of this disorder occurs. Experimental design: We examined liver samples from 10 neonates of hypertensive/preeclamptic women at 27 to 28 weeks of gestation delivered by a cessarian section. All neonates were placed in incubators but they all died within 24 hours due to immaturity. The control group comprised 10 fetuses of the same gestational age, after voluntary abortion due to a neural defect. Specific antibodies against CD34, glycophorin C, hemoglobins A and F, myeloperoxidase, CD61, CD68, terminal desoxynucleotidyl transferase and the pax-5/B-cell specific activator protein, were used in each sample. Results: Neonates from hypertensive/preeclamptic women, in comparison with controls, showed: a statistically significant reduction of erythropoiesis by 25% (p=0.015); a statistically significant increase of granulopoiesis (p=0.019); a statistically significant increase in the expression of CD68 positive cells of the monocytic lineage (p=0.017); a statistically significant increase in the expression of CD34 progenitor/stem positive cells (p=0.021). No statistically significant differences were observed in both examined groups, concerning megakaryopoiesis and B lymphopoiesis. Conclusions: Preeclampsia of pregnancy has an impact on neonatal intrahepatic hematopoiesis by increasing granulopoiesis, reducing erythropoiesis and triggering endothelial and stem cell activation. We suggest that these findings reflect a state of persistent inflammation and a loss of red blood cell production possibly contributing to the neonatal morbidity related to this disorder.
Congenital heart rhabdomyomatosis is a rare condition that is associated in most cases with tuberous sclerosis, a genetic syndrome inherited with an autosomal dominant pattern. The last is caused by a mutation in tuberous sclerosis 1 or tuberous sclerosis 2 genes. We report a case of heart rhabdomyomatosis, associated with tuberous sclerosis. The lesion was discovered at 21st week of gestation of a 36-year-old woman.
Background: The thymus provides an optimal cellular and humoral microenvironment for cell line committed differentiation of haematopoietic stem cells. The immigration process requires the secretion of at least one peptide called thymotaxine by cells of the reticulo-epithelial (RE) network of the thymic stromal cellular microenvironment. The thymic RE cells are functionally specialised based on their intrathymic location and this differentiation is modulated by various interaction signals of differentiating thymocytes and other non lymphatic haematopoietic stem cells. Objectives: To study the role of another cell line in fetal thymic haematopoietic proliferation and differentiation in different stages of development: the stromal myoid cells. Design: Fifteen cases of fetal thymic specimens (4 th to 8 th weeks: five cases 16th to 20 th weeks: five cases and 28 th to 32 nd weeks: five cases respectively) were studied. Tissue paraffin samples were stained immunohistochemically using (i) a monoclonal antibody recognising alpha-smooth muscle actin, a contractile microfilament expressed exclusively by smooth muscle cells, myofibroblasts and related cells, (ii) a monoclonal antibody glycophorin C recognising the erythropoietic cells.
Τις τελευταίες δεκαετίες υπάρχει σταδιακή μείωση τόσο των γεννήσεων στην Ελλάδα όσο και του αριθμού των παιδιών που πρόκειται να αποκτήσει μια γυναίκα κατά την διάρκεια της αναπαραγωγικής της ηλικίας. Η μεγάλη μείωση της φυσικής αύξησης του ελληνικού πληθυσμού ξεκίνησε το 1981, ενώ το 1996 η κατάσταση φτάνει πια στο απροχώρητο: οι θάνατοι είναι περισσότεροι από τις γεννήσεις. Οι γυναίκες προκειμένου να ικανοποιήσουν την ανάγκη τους για υψηλότερη μόρφωση, επαγγελματική καταξίωση και οικονομική ανεξαρτησία συνεχώς αναβάλουν τη γέννηση του πρώτου παιδιού, με αποτέλεσμα να αποκτούν λιγότερα παιδιά και σε μεγαλύτερη ηλικία. Η Θράκη, παρόλο ακριτική περιοχή, δεν έμεινε ανεπηρέαστη από τις εξελίξεις στο δημογραφικό. Ορμώμενοι από τις παραπάνω ανησυχητικές διαπιστώσεις θεωρήσαμε σκόπιμο να επικεντρώσουμε το ενδιαφέρον της έρευνάς μας στη μελέτη των μεταβολών της ηλικίας πρώτης εγκυμοσύνης τα έτη 1987-1996 στην ευρύτερη περιοχή μας. Ακόμη σκοπός μας ήταν να περιγράψουμε και να συγκρίνουμε τα πρότυπα της ηλικίας των χριστιανών και μουσουλμάνων γυναικών. Χρησιμοποιώντας το αρχείο του Πανεπιστημιακού Γενικού Νοσοκομείου Αλεξανδρούπολης, συλλέχτηκαν και αναλύθηκαν στοιχεία σχετικά με την ηλικία, το θρήσκευμα, τον αριθμό του τόκου και τον τόπο διαμονής, 8911 γυναικών που γέννησαν από 1/1/1987 - 31/12/1996. Από το σύνολο των γυναικών 4161 (46,7%) ήταν πρωτοτόκες. Ποσοστό 71,6% (2979/4161) ήταν χριστιανές, ενώ ποσοστό 28,4% (1182/4161) ήταν μουσουλμάνες. Η μέση ηλικία πρώτου τοκετού στο σύνολο, στις χριστιανές και στις μουσουλμάνες γυναίκες είναι 23,6 έτη, 24,7 και 20,8 έτη αντίστοιχα. Οι μουσουλμάνες, αφ ού γεννήσουν το πρώτο τους παιδί κατά μέσο όρο 3,9 έτη νωρίτερα από τις χριστιανές, καθυστερούν να κάνουν τα υπόλοιπα, σε σχέση με τις τελευταίες. Όμως οι χριστιανές γεννούν κάθε παιδί τους σε μεγαλύτερη ηλικία από τις μουσουλμάνες. Στο σύνολο των πρωτοτόκων γυναικών, παρατηρείται μία γραμμική άνοδος της μέσης τιμής της ηλικίας με την πάροδο του χρόνου, η οποία οφείλεται αποκλειστικά στις χριστιανές. Αντίθετα, η μέση ηλικία των μουσουλμάνων πρωτοτόκων γυναικών δεν μεταβάλλεται κατά τη διάρκεια των ετών της μελέτης και παραμένει μεταξύ 20 και 21 ετών. Στο σύνολο των πρωτοτόκων γυναικών, το μεγαλύτερο ποσοστό γεννήσεων συμβαίνει στις ηλικίες 20-24 ετών (39% το 1996 από 51,4% το 1987), με δεύτερη στη σειρά τις έφηβες έως το 1990 που τις ξεπέρασαν οι ηλικίες 25-29 ετών. Στις μουσουλμάνες πρωτοτόκες γυναίκες, το ποσοστό των γεννήσεων στις ηλικίες 20-24 ετών παραμένει σχεδόν σταθερό (45-50%) με δεύτερο στη σειρά στις ηλικίες 15-19 ετών (35-40%). Στις χριστιανές πρωτοτόκες γυναίκες, οι τοκετοί στις ηλικίες 25-29 ετών με μία συνεχή άνοδο ξεπέρασαν τους τοκετούς της ομάδας 20-24 (αντίθετη τάση) το 1993 για να φτάσουν το 37,3% το 1996. Πτωτική τάση εμφάνισαν επίσης και οι τοκετοί στις έφηβες με σταθεροποίηση στο 6,6% στο τέλος της δεκαετίας. Αξιοσημείωτη ήταν η αύξηση των πρωτοτόκων χριστιανών προς το τέλος της δεκαετίας στις ηλικίες >30 ετών, οι οποίες καταλαμβάνουν το 18,5% των τοκετών με τάση για περεταίρω άνοδο.
